This is a case of a woman with breast cancer, who developed mediastinal sarcoid-like reaction, depicted on PET/CT, which was histologically confirmed, following treatment with trastuzumab and pertuzumab. The development of noncaseating granulomas in patients who do not fulfill the criteria for systemic sarcoidosis is known as sarcoid-like reaction, having been described in association with trastuzumab in a few case reports, but none with pertuzumab. A 26-year-old man presented with a retroperitoneal mass with left urinary tract obstruction and hydronephrosis recently. 18F-FDG PET/CT showed the retroperitoneal mass had intense and homogeneous FDG avidity. In 68Ga-FAPI PET/CT, the retroperitoneal mass showed intense radioactivity without other organ involvements. The patient was finally diagnosed with idiopathic retroperitoneal fibrosis. After treatment, the follow-up CT showed significant decrease in size of the retroperitoneal mass. The current case indicated that idiopathic retroperitoneal fibrosis had active uptake of 68Ga-FAPI.

Restricted lower limb vasculitis is a type of localized muscle vasculitis limited to the lower limbs. The usefulness of fluorodeoxyglucose-positron emission tomography (FDG-PET) for the diagnosis of this entity has not yet been reported. A 28-year old man, with previous history of chronic abdominal pain, was referred to our nuclear medicine center for an F-FDG PET/CT following the fortuitous discovery of a 48-mm right adrenal gland lesion of heterogeneous density at the CT scan. The PET/CT showed an isolated heterogeneous significant uptake evocative of neoplasia. Surprisingly, the anatomopathological analysis after surgery revealed an adenoid tumor of the adrenal gland. This case of a rare benign tumor can be a source of false-positive and mimicking malignancies in the exploration of adrenal lesions.

Myxoid leiomyosarcoma is a malignant tumor that originates from the mesenchymal tissue with extensive mucoid degeneration. It usually occurs in the uterus; occurrences from other tissues are extremely rare. Pancreatic carcinoma metastasizing to the ovary is rare. We report the case of a 69-year-old woman with ovarian metastasis as initial presentation. Ultrasonography revealed a multicystic mass in the right adnexa, which was considered a malignancy. FDG PET/CT performed to detect possible metastasis revealed a right adnexal mass with mild activity and an additional FDG focus in the tail of the pancreas, which later proved to be ovarian metastasis from pancreatic carcinoma.

In the absence of hepatic metastasis in abdominal cancers, an isolated malignant portal vein thrombus is very rare. The presence of a malignant thrombus has clinical significance for determining the stage, treatment, and prognosis. 18F-FDG PET/CECT is a noninvasive modality for discriminating between malignant and benign thrombi. We present a case of primary sigmoid colon carcinoma for which 18F-FDG PET/CT showed, in addition to the 18F-FDG-avid primary lesion, an 18F-FDG-avid filling defect in the portal vein, likely malignant thrombus.

PURPOSE: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder with on average a 1-year delay between symptom onset and diagnosis. A 65-year-old woman underwent FDG PET/CT for presumed hepatosplenic malignancy suggested by an abdominal CT. The images revealed multiple foci of intense FDG activity in both the liver and the spleen. However, a splenic biopsy result revealed no malignant cells, and either tuberculosis or sarcoidosis was proposed. Following an incomplete antituberculosis therapy, a repeat FDG PET/CT showed resolution of the abnormal activity in the liver and the spleen.
