Agent-based modeling of macrophage-fibroblast interactions in the immune response to biomaterials.

Foreign body reaction (FBR) denotes the reaction to the implantation of a biomaterial into the body. It triggers cascades of responses in the tissue and involves different cell types including, among others, macrophages, fibroblasts and endothelial cells. Macrophages regulate the inflammatory and healing processes. They exhibit a variety of functional phenotypes (or states) induced by the stimulus they receive and the microenvironment. This polarization process is governed by chemical mediators, known as cytokines, that are secreted by the macrophage itself and induce cellular activation and r

Properties of Electrospun Fibers That Influence Foreign Body Response Modulation.

Improving the utility of biomedical devices implanted in subcutaneous tissue by modulating the innate immune response common to these implants is of great interest to improve their utility. Uncontrolled, most biomedical devices produce an immune reaction known broadly as the foreign body response (FBR), which ultimately isolates the device from the native tissue. The use of electrospun fibers to create a porous surface that promotes tissue in-growth and regeneration represents a new paradigm in FBR modulation. A vast number of parameters can be adjusted in the electrospinning process to tune t

Clinical challenges and opportunities related to the biological responses experienced by indwelling and implantable bioelectronic medical devices.

Implantable electrodes have been utilized for decades to stimulate, sense, or monitor a broad range of biological processes, with examples ranging from glucose monitoring devices to cochlear implants. While the underlying science related to the application of electrodes is a mature field, preclinical and clinical studies have demonstrated that there are still significant challenges in vivo associated with a lack of control over tissue-material interfacial interactions, especially over longer time frames. Herein we discuss the current challenges and opportunities for implantable electrodes and

Allometrically scaling tissue forces drive pathological foreign-body responses to implants via Rac2-activated myeloid cells.

Small animals do not replicate the severity of the human foreign-body response (FBR) to implants. Here we show that the FBR can be driven by forces generated at the implant surface that, owing to allometric scaling, increase exponentially with body size. We found that the human FBR is mediated by immune-cell-specific RAC2 mechanotransduction signalling, independently of the chemistry and mechanical properties of the implant, and that a pathological FBR that is human-like at the molecular, cellular and tissue levels can be induced in mice via the application of human-tissue-scale forces through

Thermogel Loaded with Low-Dose Paclitaxel as a Facile Coating to Alleviate Periprosthetic Fibrous Capsule Formation.

Medical-grade silicones as implants have been utilized for decades. However, the postoperative complications, such as capsular formation and contracture, have not yet been fully controlled and resolved. The aim of the present study is to elucidate whether the capsular formation can be alleviated by local and sustained delivery of low-dose paclitaxel (PTX) during the critical phase after the insertion of silicone implants. A biocompatible and thermogelling poly(lactic acid- co-glycolic acid)- b-poly(ethylene glycol)- b-poly(lactic acid- co-glycolic acid) triblock copolymer was synthesized by us

[Exogenous lipoid pneumonia associated with pulmonary and hepatic sarcoidosis].

INTRODUCTION: In a patient with basal alveolar shadowing the diagnosis of exogenous lipoid pneumonia (ELP) requires a past history of chronic ingestion of liquid paraffin and the presence of numerous macrophages containing oil droplets in the bronchial lavage (BL). Additional radiological abnormalities suggest an associated disease, notably infection or cancer, as has been described in the literature. CASE REPORT: We report the case of a 50 year old woman presenting with alveolar shadowing in the left lung associated with ipsilateral mediastinal nodes and a pleural effusion in addition to two

[Systemic spread of wear debris--an in-vivo study].

AIM: The aim of this study was to elucidate whether there is a systemic spread of wear debris from peripherally applied stainless steel and titanium particles into the blood and subsequently to parenchymatous organs. Furthermore, we report on histological findings at the implantation site. METHOD: In Syrian Gold hamsters we implanted 2 mm3 wear debris of stainless steel and titanium into the dorsal skin fold chamber. Over a period of 2 weeks we took blood samples and afterwards explanted the implant area, the heart, lung, liver and spleen. One half of the organs and the implant area were used

Clinical and histologic evaluation of an indwelling, inflatable, long-term laryngeal stent in the canine model.

OBJECTIVE: The goal was assessment of a revolutionary approach to laryngeal stenting. A radically designed, atraumatic, inflatable, low-pressure, high-volume, endoscopically inserted laryngeal stent for effective intralaryngeal support or long-term prevention of aspiration was prospectively evaluated in a canine model to reveal any significant short- and long-term anatomic and functional changes in the larynx. METHODS: A self-sustaining, tube-free, long-term flap tracheostomy was performed in 14 dogs, followed by endoscopic laryngeal stent insertion. The stents were inflated, and their intral

Urethral healing in rabbits.

We studied urethral healing in New Zealand white rabbits by histological examination after insult (urethral catheter) or injury (urethrotomy) specifically for acute and chronic inflammation, fibrosis, fistulas, squamous metaplasia, foreign body giant cells and urethral dilatation. Urethral catheterization resulted in increased inflammation and fibrosis compared to noncatheterized animals. Skin closure techniques and materials resulted in an inflammatory response that may extend to and involve the urethra. Minor differences in suture size were not an important variable but the persistence of su

Autologous fat injection for vocal cord medialization in the canine larynx.

This study examined the use of autologous fat as an alternative to Teflon and collagen as the implantable material in vocal cord medialization. Five animals underwent left recurrent laryngeal nerve sections with subsequent fat harvest and implantation into the left true vocal cords. Three animals were killed after 48 hours and 2 after 3 weeks; their larynges were examined with light microscopy. The results of the 48-hour samples show mode-rate acute inflammation and few areas of focal necrosis. The 3-week samples show no necrotic foci, minimal foreign-body reaction, and maintenance of structur…

[Histologic reactions following implantation of a "lyodura-soft" band--animal experiment studies of an abdomino-vaginal sling operation].

Abdomino-vaginal sling operations are favoured for the treatment of female stress incontinence recidive. An actual material for the sling ribbon is the Lyodura-soft. Histological reactions between one week and one year are described after experimental subcutaneous implantation of the Lyodura-soft ribbon in rats. After the initial inflammatory reaction in the surrounding connective tissue of the graft during the first 1-2 weeks, a capsule of fibre rich connective tissue is produced by the organism. After 6 months to 1 year the capsule has almost vanished. A reduction of the graft has not taken

Macroscopic and histologic tissue reaction to polydioxanone, a new, synthetic, monofilament microsuture.

A major contributing factor to failure in reconstructive tubal surgery is adhesion formation. It can be due to the presence of a foreign body primarily represented by suture material. A new, synthetic, absorbable suture material, Polydioxanone (PDS), was compared to polyglactin -910 (Vicryl). Microsurgical anastomosis of the bicornuate rat uterus served as a model. The right horn was anastomosed with 6-0 or 8-0 PDS and the left horn with Vicryl of the same diameter. At 3, 7, 14, 30 and 60 days after the procedure three to four animals were killed and the histologic reaction estimated and compa

[Biological compatibility of polyurethan (PUR) (author's transl)].

After implantation of a disc (thickness: 0.25 mm, diam: 1.5 mm) of PUR in the anterior chamber of rabbits through a lancet incision, the reactions of iris, aqueous, and cornea were observed for six months at regular intervals using slit-lamp photography. 'Steeping' materials were as compatible as nonsteeping materials. At the place where the disc touched the iris one could see during a half-year period hyperemia iridis and foreign-substance granuloma of the iris. The cause of these reactions is a toxin in the PUR. Mechanical irritants as a cause could be excluded through the method utilized in

Clinical and histologic evaluation of a new injectable implant: hydrophilic polyacrylamide gel.

Hydrophilic polyacrylamide gel is a new, jellylike transparent implant that consists of 95% water and 5% polyacrylamide, and it has been applied in many countries for many years as a sort of injectable implant. However, there are no data regarding human tissue reaction to hydrophilic polyacrylamide gel to date. Thirty-one patients who had undergone the injection of hydrophilic polyacrylamide gel for soft-tissue augmentation from September 1998 to November 2001 wanted to remove the implant. Hematoxylin-eosin-stained sections from each cyst of tissue sample were investigated and analyzed with li

Silicone-Induced Granulomatous Reaction Causing Severe Hypercalcemia: Case Report and Literature Review.

A 67-year-old woman presented to the hospital with complaints of abdominal pain. Physical exam was significant for signs of severe dehydration, mild epigastric tenderness and multiple non-tender hard nodules over her arms and thighs. Incidental finding of severe hypercalcemia led to negative workups for hyperparathyroidism, vitamin D intoxication, and malignancy. However, elevated levels of 1,25-hydroxy vitamin D raised the possibility of granulomatous diseases. Imaging and patient report revealed silicone-induced foreign body granulomatous reaction as the cause of hypercalcemia. Use of silico
