Ipsilateral fractures of the distal radius and scaphoid treated by Herbert screw and external skeletal fixation. A report of two cases.

Two patients with comminuted, displaced fractures of the distal radius associated with ipsilateral, undisplaced scaphoid fractures were treated by internal fixation of the scaphoid fracture with a Herbert screw in association with external fixation of the distal radial fracture. One of the patients had a limited open reduction of the distal radius combined with bone grafting. Both patients had satisfactory results. Internal fixation of the scaphoid is indicated if distraction is applied to the carpus to treat an associated fracture of the distal radius, even if the scaphoid fracture is undispl

Reduction of Acute Zygomatic Arch Fractures With Intraoperative Ultrasound: An Underutilized Technique for Resource Scarce Settings.

BACKGROUND: Zygomatic arch (ZA) fractures are a common facial fracture, and reduction is typically performed blind via a Gillies or Keen approach. Postoperative confirmation of reduction thus requires advanced imaging, which may not be readily available in all settings. Thus, there exists a need for an effective, low-cost imaging paradigm to employ in these clinical scenarios. Herein, we introduce the ultrasonic arch reduction (USA Reduction) for ZA fractures. METHODS: All consecutive patients with ZA fractures undergoing a USA Reduction at a single public hospital were reviewed. Patients wer

New Linked-Wire-Type External Fixator (the Ichi-Fixator System) for Metacarpal Fractures: Case Series and Literature Review.

Background: External fixator is an effective treatment option in the comminuted or unstable metacarpal fractures. The new linked-wire type of external fixator (the Ichi-Fixator System) developed for hand and wrist fractures enables fine adjustment of the fixation under fluoroscopic inspection through small screws inside the fixator fitted externally. This technique is designed to reinforce the stability and rigidity of conventional percutaneous Kirchner wire fixation. The aim of this study is to assess the effectiveness of the fixator for comminuted or unstable metacarpal fractures through the

Epidemiology and Treatment of Radial Head Fractures: A Database Analysis of Over 70,000 Inpatient Cases.

PURPOSE: We aimed to study the epidemiology of radial head fractures within a single national registry database while analyzing trends in available treatments. METHODS: A retrospective analysis of data from 2007 to 2016 provided by a national inpatient database registry was performed using the International Classification of Diseases code for radial head fractures and associated German Procedure Classification (OPS) codes. All surgical interventions were evaluated according to fracture type, patient sex and age, and distribution differences over the last decade to detect changes in the treatm

Intracompartmental pressure changes after anterolateral bridge plating of tibial fractures.

OBJECTIVE: Compartment syndrome is one of the most devastating complications in orthopedics both for the patient and the treating physician. Among the many causes, trauma and its treatment are the most common reasons for compartment syndrome, which most frequently occurs in the lower leg following tibial fractures. Since bridge plating of difficult metadiaphyseal tibial fractures is becoming increasingly popular, serious concerns have been raised about the increased intracompartmental pressures and possible compartment syndrome. METHODS: This study investigated the intracompartmental pressure

Nonoperative treatment of humeral shaft fractures revisited.

PURPOSE: The purpose of this study was to examine the union rate of humeral shaft fractures treated nonoperatively and to establish whether a particular fracture type is more likely to go on to nonunion. METHODS: Radiographs and patient records of 207 humeral shaft fractures occurring during 5 years were retrospectively reviewed. All patients were initially managed nonoperatively and placed in a U-slab on diagnosis in the emergency department; this was converted to a functional humeral brace at 7 to 10 days after injury. Fracture location, morphology and comminution were assessed radiological

Staged Reconstruction and Salvage of a Small Finger With a Corticocancellous Distal Radius Autograft: A Case Report.

Finger reconstruction in the aftermath of severe trauma poses significant challenges in restorative surgery, particularly when addressing extensive soft tissue damage, comminuted fractures, and simultaneous neurovascular complications. This study presents a case of a patient with a complicated proximal phalangeal fracture who pursued digit salvage. A 31-year-old Hispanic woman sustained severe injury to her right fifth digit during an all-terrain vehicle (ATV) rollover resulting in an exposed, comminuted fracture of the proximal phalanx with extensive structural and neurovascular compromise. Endoscopically assisted zygomatic fracture reduction and osteosynthesis revisited.

In comminuted fractures of the zygoma open reduction of the malar arch is essential for correct anatomic repair. Avoiding exposure of this landmark may result in severe functional and aesthetic impairment. Exposure of the malar arch necessitated traditionally a coronal incision. However, recently several authors reported good results in malar fracture repair with minimal incisions and endoscopic assistance. To establish this technique a cadaver study was performed. Different approaches to the malar arch, fracture reduction and internal fixation were evaluated. After establishment of a satisfac

Fractures of the base of the middle phalanx treated by a dynamic external fixation device.

Intraarticular fractures of the base of the middle phalanx are difficult to treat, especially if the fragments are comminuted. A spring dynamic traction device is described and the results in the first 14 patients presented. Patient acceptance has been high and the results comparable to other methods of treatment.

[Bone disease with Pain. Colles' fracture].

The distal radius is one of the most commonly fractured long bone. Colles' fracture results from a fall on the dorsiflexed and pronated hand. The dinner-fork deformity is the typical deformity of the Colles' fracture. For patients with no or a little displacement, conservative treatment is applied. The non-bridge type external fixator is applied for patients without an intra articular fracture. For patients with a comminuted fracture, the locking plate (volar approach) is recommended. During the healing period, shoulder, elbow and finger exercise should be insisted.

The use of bioabsorbable plate fixation for nasal fractures under local anaesthesia through open lacerations.

Nasal fractures are the most common facial fractures. The majority of nasal fractures have been discussed as minor injuries and managed by closed reduction and intranasal packing. However, in cases of a nasal fracture with open laceration on fracture site, the plates fixation may be accomplished with definite reduction through the open laceration. But, metallic reconstruction plates can sometimes produce palpable irregularity at the site of fixation in nasal bone fractures. We performed rigid fixation through open laceration wound with bioabsorbable plate and screws under local anesthesia. Sat

Unstable intertrochanteric fractures treated with the Variwall reconstruction nail.

Forty consecutive patients with unstable intertrochanteric fractures were treated with the Variwall reconstruction nail. Twelve of these had subtrochanteric extensions to their fractures, while most were highly comminuted grade 5 fractures. This nail was found to be relatively easy to use with a mean operative time of 58 min. Mean follow up was 5.8 months. Clinically, 86 per cent had occasional pain or none at all. There were no external rotation deformities, and only one case of shortening. Radiologically there were no non-unions and no case of shaft fractures. Failure occurred in one case, b

Zygomaticomaxillary complex fracture repair with intraoperative CBCT imaging. A prospective cohort study.

The aim of this study was to evaluate the value of intraoperative conebeam computed tomography (CBCT) imaging in the treatment of zygomaticomaxillary complex (ZMC) fractures. A prospective single center cohort study was performed. Included were consecutive patients who underwent surgery for a unilateral ZMC fracture. An intraoperative CBCT scan was performed after reduction of the ZMC fracture. Revision reduction was performed of the ZMC and/or orbital floor (OF) on indication. The preoperative and postoperative asymmetry of the outer surface of the ZMC was measured on digital 3D-models of CBC

Complete Recovery of Iatrogenic Radial Nerve Palsy in a Child: A Case Report.

Supracondylar humerus fractures are among the most common paediatric injuries. Although neurologic complications are often seen in these fractures, iatrogenic nerve injuries following closed reduction and percutaneous pinning remain relatively uncommon. Radial nerve palsy is particularly rare in this context, and most cases represent neuropraxic injuries that recover spontaneously. This report describes a rare case of iatrogenic high radial nerve palsy following closed reduction and K-wire fixation of a comminuted supracondylar humerus fracture in a child, with complete functional recovery und

Radial head arthroplasty.

In conclusion, radial head fractures with 3 or more fragments have a high incidence of complications when treated with ORIF including hardware failure, malunion, nonunion, and the need for re-operation. Radial head arthroplasty has demonstrated good success in the treatment of complex, comminuted radial head fractures which are not amenable to non-opeative treatment or ORIF. Success can be optimized by diligent surgical dissection, avoiding inadvertent nerve injury, placement of an appropriately sized implant, repair of associated injuries, and early protected motion.

A floating knee treated conservatively: a case report.

Ipsilateral fractures of the femur and tibia are usually considered a primary indication for internal fixation. A case of severely comminuted, open fracture of the distal femur and proximal tibia involving the knee which was managed conservatively is described here. This case is reported to make a plea for the trial of conservative management before opting for primary internal fixation of such complicated fractures. The conservative approach is strongly advocated when the essential requirements for internal fixation cannot be rigidly met.

The fractured patella: a long-term follow-up study.

One hundred and three patients with 104 patellar fractures were examined over a follow-up period ranging from three to 11 years. Patients were assessed subjectively and objectively. The fractures were classified into three groups, transverse, comminuted and polar. Four methods of treatment were used, conservative, internal fixation, partial excision and total excision. In general, the results in the majority of patients were satisfactory, those patients who underwent total patellectomy faring at least as well as any others requiring surgical intervention.

Intrafocal pin plate fixation of distal ulna fractures associated with distal radius fractures.

Subcapital ulnar fractures in association with distal radius fractures in elderly patients increase instability and pose a treatment challenge. Fixation of the ulnar fracture with traditional implants is difficult due to the subcutaneous location, comminution, and osteoporosis. We describe an intrafocal pin plate that provides fixation by a locking plate on the distal ulna and intramedullary fixation within the shaft. The low profile and percutaneous technique make this device a useful alternative for treatment of subcapital ulna fractures in the elderly.

Operative treatment of intra-articular distal humerus fractures.

Intra-articular distal humerus fractures can be among the most challenging injuries treated by orthopedic surgeons. The goals of surgical treatment are anatomical restoration of the articular surface and stable fixation of the fracture fragments to allow for early motion. However, the bone stock of the distal humerus is limited, and stable fixation may be difficult to achieve in the case of a low fracture pattern, comminution, or osteoporosis. In this article, we provide practical recommendations for surgical management of these complex fractures.

[Patellectomy as a salvage operation].

One hundred seven patients were examined who had undergone patellectomies between 1965 and 1983 (113 patellectomies). The mean follow-up time was 10.5 years (3-17.5 years) and the average age of the patients 42.6 years. There were three distinct groups of operative techniques: (1) the purse-string technique in 40 patellectomies; (2) the vastus medialis technique in 24 patellectomies; (3) other techniques in 49 patellectomies. The indications for patellectomy were: chondromalacia, 56 cases; comminuted patellar fractures, 32 cases; arthritis, 17 cases; recurrent patellar dislocations, 8 cases. T
