Statistical resolution of genetic heterogeneity in familial disease.

If a disease can be split into two or more groups on any criterion (clinical, biochemical, physiological or statistical) then the grouping can be tested to establish if genetically independent forms of the disease have been identified. The data required are simply the frequencies of the two disease groups in relatives of probands for each of the disease groups. A systematic search for such distinct groups is proposed in searches for genetic heterogeneity in familial diseases. In disease forms with overlapping, correlated genetic liabilities, the method of Falconer (1967) can be used to estimat

Hydrogen cyanide smell sensitivity in some Indian populations.

The ability to smell HCN has been examined in a sample of 2619 subjects from Hyderabad, Andhra Pradesh, belonging to different Indian ethnic groups. One single antimode, between concentrations of 5% and 10%, has been found. No sex nor ethnic differences were noted. Previous claims of possible sex-linked inheritance could not be confirmed.

Consecutive medical exome analysis at a tertiary center: Diagnostic and health-economic outcomes.

The utility of whole exome analysis has been extensively demonstrated in research settings, but its clinical utility as a first-tier genetic test has not been well documented from diagnostic and health economic standpoints in real-life clinical settings. We performed medical exome analyses focusing on a clinically interpretable portion of the genome (4,813 genes) as a first-tier genetic test for 360 consecutive patients visiting a genetics clinic at a tertiary children's hospital in Japan, over a 3-year period. Bioinformatics analyses were conducted using standard software. A molecular diagnos

Oesophageal atresia, related malformations, and medical problems: a family study.

Oesophageal atresia (OA) and tracheo-oesophageal fistula (TOF) are life-threatening malformations of generally undefined cause. Previous reports of familial cases suggest a genetic contribution. The pattern of inheritance appears non-Mendelian, i.e., multifactorial. Individuals with OA/TOF often have other malformations and medical problems. The aim of this study was to determine the association in OA/TOF cases and healthy control subjects of associated malformations, midline defects, and medical conditions. We also investigate the relationships of these conditions in the relatives of the case

Whole exome sequencing unveils novel pathogenic variants in an Iranian cohort with retinal dystrophies: Implications for genetic diagnosis and counseling.

BACKGROUND: Retinal dystrophies (RD) are a heterogeneous group of genetic disorders leading to progressive vision loss. The notable gap in the genetics of RD is primarily due to unidentified disease genes and variants. Advanced genotyping technologies present significant opportunities for the identification of causative variants, thereby contributing to improved disease management. METHODS: This study investigates the genetic basis of RD in 12 families from Iran. Comprehensive ophthalmic evaluations were conducted for each participant, including visual acuity testing, slit-lamp examinations,

Molecular diagnosis of inherited platelet disorder via a targeted whole-exome virtual gene panel: a 5-year institutional experience.

BACKGROUND: Inherited platelet disorders represent a prevalent but inadequately understood subset of bleeding diatheses, often with elusive causative genetics. The low rate of molecular diagnoses highlights the critical need for advanced genetic tools to enhance risk stratification and therapeutic personalization. OBJECTIVES: To address this, we retrospectively reviewed our institutional experience using a whole exome sequencing-based platelet virtual gene panel in patients with suspected inherited platelet disorders to determine the frequency of variant identification, rate of diagnosis assi

Uterine serous carcinoma and germline genetic testing: patterns of referral, completion and pathogenic variant detection.

BACKGROUND: Current guidelines recommend consideration of germline genetic testing for patients with uterine serous carcinoma (USC) but real-world data are limited on completion rates of testing and pathogenic variant (PV) identification. This study aimed to evaluate the rate of genetic testing referral and completion in a cohort of patients with USC, determine the prevalence of clinically meaningful PVs found on testing and explore factors associated with genetics referral and testing completion. METHODS: We retrospectively examined the medical records of all individuals diagnosed with USC b

Exome copy number variant detection, analysis, and classification in a large cohort of families with undiagnosed rare genetic disease.

Copy number variants (CNVs) are significant contributors to the pathogenicity of rare genetic diseases and, with new innovative methods, can now reliably be identified from exome sequencing. Challenges still remain in accurate classification of CNV pathogenicity. CNV calling using GATK-gCNV was performed on exomes from a cohort of 6,633 families (15,759 individuals) with heterogeneous phenotypes and variable prior genetic testing collected at the Broad Institute Center for Mendelian Genomics of the Genomics Research to Elucidate the Genetics of Rare Diseases consortium and analyzed using the s

Glioblastoma in pregnant patient with pathologic and exogenous sex hormone exposure and family history of high-grade glioma: A case report and review of the literature.

BACKGROUND: Glioblastoma (GBM) incidence is higher in males, suggesting sex hormones may influence GBM tumorigenesis. Patients with GBM and altered sex hormone states could offer insight into a relationship between the two. Most GBMs arise sporadically and heritable genetic influence on GBM development is poorly understood, but reports describing familial GBM suggest genetic predispositions exist. However, no existing reports examine GBM development in context of both supraphysiologic sex hormone states and familial predisposition for GBM. We present a case of isocitrate dehydrogenase (IDH)-wi

Genetic evaluation of patients and families with concern for hereditary endocrine tumor syndromes.

Hereditary endocrine tumor syndromes are rare conditions with overlapping features. It is imperative that healthcare providers differentiate between these syndromes for proper patient care. Advances in genetic testing technologies have increased utilization of genetic counseling and testing in this field; however, few endocrine cancer genetics clinics exist. Two years ago, a genetic counselor (GC) specializing in endocrine cancer genetics was added to the multidisciplinary team of the James Neuroendocrine/Thyroid Clinic at The Ohio State University. Here, we report on this experience. In total

Does heredity determine the allergy manifestation or the sensitisation to a specific allergen?

BACKGROUND: The role of genetics in allergy development is well accepted. However, studies could not delineate the mode of inheritance or what is specifically being inherited. The purpose of this study was to determine the effect of genetics on the development of allergy manifestation, serum IgE level, and sensitization to specific allergens. METHODS: Fifty-eight twin sets (age 7 months to 11 years) were evaluated for allergy by medical history, family history, physical examination, serum total IgE level, and percutaneous testing to selected common allergens. RESULTS: In 25 monozygotic (MZ)

Telomere dysfunction and mucociliary impairment drive idiopathic pulmonary fibrosis susceptibility: insights from a Sardinian whole-exome study.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a progressive interstitial lung disease in which both environmental exposures and genetic predisposition contribute to disease susceptibility. Studying the burden of rare variants in a genetically homogeneous founder population may help identify disease-associated alleles that are difficult to detect in more heterogeneous populations. METHODS: Whole exome sequencing was performed on 123 patients with IPF and 1110 unrelated controls from Sardinia (Italy). Variant prioritisations were conducted according to the American College of Medical Genet
