H1-antihistamine is generally a well-tolerated and safe drug. However, in resemblance with all other drugs, H1-antihistamines can also prompt adverse drug reactions (ADRs). We recently encountered the very unusual ADR of H1-antihistamine-induced gynecomastia. Men referred for breast imaging most frequently present with a unilateral palpated breast lump or breast enlargement. In the vast majority of these cases, the cause is benign and the most common etiology is gynecomastia. This pictorial review illustrates the appearance by full field digital mammography and digital breast tomosynthesis of gynecomastia as well as additional findings in the male breast including sternalis muscle and hypertrophied pectoralis muscle, lipoma, intramammary lymph node, fat necrosis, breast cancer, and atypical ductal hyperplasia.

The aim of this study is to introduce the endoscopic subcutaneous mastectomy without skin excision as the standard surgical technique for grade IIB and III gynecomastia. Endoscopic subcutaneous mastectomy was performed successfully in 125 breasts of 65 patients with Simon's grade IIB and III gynecomastia. Ten pubertal boys treated with tamoxifen for gynecomastia for more than 3 months were evaluated after 2.5-7 years (mean 4.6 years) to determine the side effects of this therapy. We assessed the safety of long duration in adolescents with pubertal gynecomastia treated with tamoxifen. We did not find any serious side effects of tamoxifen in these patients.

METHODS: All identifiable environmental exposures were investigated for estrogenic and antiandrogenic activity.
RESULTS: A high incidence of gynecomastia was observed among Haitian refugees in five detention centers in the United States. Myofibroblastoma of the breast is a recently recognized benign mesenchymal mammary tumor that typically occurs as a unilateral, solitary lesion. Myofibroblastomas are well-circumscribed, unencapsulated tumors characterized by spindle cells in fascicles which exhibit varying degrees of myogenic and fibroblastic differentiation. Our case reports a mammary myofibroblastoma occurring in an 82-year-old male with gynecomastia and reviews the reported incidence of this benign spindle cell tumor in the world literature.
