Pediatric systems medicine: evaluating needs and opportunities using congenital heart block as a case study.

Medicine and pediatrics are changing and health care is moving from being reactive to becoming preventive. Despite rapid developments of new technologies for molecular profiling and systems analysis of diseases, significant hurdles remain. Here, we use the clinical setting of congenital heart block (CHB) to uncover and illustrate key informatics challenges impeding the development of a systems medicine approach emphasizing the prevention and prediction of disease. We find that there is a paucity of useful bioinformatics tools enabling the integrative analysis of different databases of molecula

Premature atherosclerotic cardiovascular disease in the MENA region: economic impact, barriers to care, and policy recommendations for an escalating crisis.

PURPOSE OF THE REVIEW: Premature atherosclerotic cardiovascular disease (ASCVD) is increasingly recognized as a major public health challenge in the Middle East and North Africa (MENA) region, where cardiovascular events occur at younger ages than in Western populations. Despite ongoing policy reforms and improvements in healthcare infrastructure, premature ASCVD remains underrecognized and insufficiently addressed. This review summarizes the epidemiology, economic burden, policy initiatives, and current barriers related to premature ASCVD in MENA, while proposing targeted interventions to sup

Use of the nominal group technique to identify stakeholder priorities and inform survey development: an example with informal caregivers of people with scleroderma.

OBJECTIVES: The nominal group technique (NGT) allows stakeholders to directly generate items for needs assessment surveys. The objective was to demonstrate the use of NGT discussions to develop survey items on (1) challenges experienced by informal caregivers of people living with systemic sclerosis (SSc) and (2) preferences for support services. DESIGN: Three NGT groups were conducted. In each group, participants generated lists of challenges and preferred formats for support services. Participants shared items, and a master list was compiled, then reviewed by participants to remove or merge

Sí se puede: using participatory research to promote environmental justice in a Latino community in San Diego, California.

Community-based participatory research (CBPR) increasingly is seen as a potent tool for studying and addressing urban environmental health problems by linking place-based work with efforts to help effect policy-level change. This paper explores a successful CBPR and organizing effort, the Toxic Free Neighborhoods Campaign, in Old Town National City (OTNC), CA, United States, and its contributions to both local policy outcomes and changes in the broader policy environment, laying the groundwork for a Specific Plan to address a host of interlocking community concerns. After briefly describing th

Overcoming Logistical Challenges in Kidney Disease Treatment and Workflow Management in Disasters.

Sustaining dialysis and kidney care services in emergencies is highly dependent on the resilience of the health system and the quality and breadth of predisaster planning. Emergencies tend to occur in regions where health systems are not strong. This multiplies patient vulnerability. Patients with kidney diseases should be prepared in how to react in emergencies to best protect their health and their lives. The logistics of maintaining uninterrupted dialysis and transplant care should not be underestimated. Meeting these needs safely and fast can be enhanced through transparent and constructiv

The role of surveillance systems in confronting the global crisis of antibiotic-resistant bacteria.

PURPOSE OF REVIEW: It is widely accepted that infection control, advanced diagnostics, and novel therapeutics are crucial to mitigate the impact of antibiotic-resistant bacteria. The role of global, national, and regional surveillance systems as part of the response to the challenge posed by antibiotic resistance is not sufficiently highlighted. We provide an overview of contemporary surveillance programs, with emphasis on gram-negative bacteria. RECENT FINDINGS: The WHO and public health agencies in Europe and the United States recently published comprehensive surveillance reports. These hig

[Transformation of Epidemiology in the Age of Artificial Intelligence].

Epidemiology has been fundamental for analyzing health problems and supporting decision-making in healthcare systems and public health. However, traditional epidemiological methods, designed fundamentally to identify causal associations at the population level through aggregate data measures, present inherent limitations in capturing individual heterogeneity in response to specific exposures. This population-based approach hinders personalized prediction of outcomes in a particular individual whose risk factors may manifest differently from the group average, particularly when multiple context

Delphi consensus validation of an integrated climate sensitive diabetes management model for climate vulnerable health systems in Iran.

Type 2 diabetes mellitus (T2DM) is rising rapidly in Iran, where climate-related hazards such as heatwaves, sand and dust storms, and disruptions to food and medicine supply chains challenge continuity and quality of care. We sought to validate a climate-sensitive, health system-oriented management model for T2DM using a Delphi consensus process. Eighteen experts in diabetes, noncommunicable diseases and health systems rated 51 statements derived from the preliminary Integrated Iranian Climate-Sensitive Diabetes Management Model (ICDMM), mapped onto six domains aligned with WHO health system b

Experiences of Black and Latinx health care workers in support roles during the COVID-19 pandemic: A qualitative study.

Black and Latinx individuals, and in particular women, comprise an essential health care workforce often serving in support roles such as nursing assistants and dietary service staff. Compared to physicians and nurses, they are underpaid and potentially undervalued, yet play a critical role in health systems. This study examined the impact of the coronavirus disease 2019 (COVID-19) pandemic from the perspective of Black and Latinx health care workers in support roles (referred to here as HCWs). From December 2020 to February 2021, we conducted 2 group interviews (n = 9, 1 group in English and

Developing a community-based neonatal care intervention: a health facility assessment to inform intervention design.

Community-based interventions are an important way of improving health in low-income countries. A necessary prerequisite for the design of such interventions is an understanding of the local health system. This will inform intervention design, help ensure the community-intervention forms part of a continuum of care, and provide information about health system strengthening activities that may be necessary for success. Such formative research processes, however, are seldom reported in the literature. We present the results of a health facility assessment used in the design stage of Newhints, a

Attributable fraction of congenital syphilis due to the lack of prenatal care.

INTRODUCTION: Although congenital syphilis is preventable, its incidence has increased in Brazil. METHODS: In this ecological study, a Bayesian spatio-temporal model was used to estimate the attributable fraction of congenital syphilis cases due to the lack of prenatal care recorded between 2010 and 2015 in the state of São Paulo, Southeast Brazil. RESULTS: For the year 2016, it was estimated that between 79.4% and 95.3% of the congenital syphilis cases among women who did not have prenatal care could have been prevented. CONCLUSIONS: A significant proportion of congenital syphilis cases ca

Assessment of population exposure to PM10 for respiratory disease in Lanzhou (China) and its health-related economic costs based on GIS.

BACKGROUND: Evaluation of the adverse health effects of PM10 pollution (particulate matter less than 10 microns in diameter) is very important for protecting human health and establishing pollution control policy. Population exposure estimation is the first step in formulating exposure data for quantitative assessment of harmful PM10 pollution. METHODS: In this paper, we estimate PM10 concentration using a spatial interpolation method on a grid with a spatial resolution 0.01° × 0.01°. PM10 concentration data from monitoring stations are spatially interpolated, based on accurate population dat

Socio-environmental determinants of the leptospirosis outbreak of 1996 in western Rio de Janeiro: a geographical approach.

The environmental and social context in which a leptospirosis outbreak took place during the summer of 1996 in the Rio de Janeiro Western Region was examined by using spatial analysis of leptospirosis cases merged with population and environmental data in a Geographical Information System (GIS). Important differences were observed between places where residences of leptospirosis cases are concentrated and other places in the region. Water supply coverage, solid waste collection, sewerage system coverage and flood risk area were the main determining variables from an initial list of ten. The in
