The exact prevalence of heart failure among children of developing countries is not known, as the data is limited. The relative frequency of different causes of pediatric heart failure varies widely across different countries and even among different parts of large countries like India. Children of developing countries face a double burden of etiologies. Conditions such us congenital heart disease, myocarditis and cardiomyopathies are common causes of pediatric of heart failure. In addition, diseases like rheumatic heart disease, nutritional deficiencies, and other tropical diseases also resul…

The treatment of chronic systolic heart failure has shown important improvements, but there are no evidence-based medical treatment options for patients with diastolic heart failure. A recent elaborate study, the TOPCAT trial, failed to show a beneficial effect of spironolactone on the primary composite endpoint of cardiovascular death or hospital admission for heart failure in patients with heart failure and preserved ejection fraction (HFpEF). After ACE inhibitors and angiotensin receptor blockers, this is the third group of medications that previously showed a beneficial effect in patients …

Devices have become a cornerstone of heart failure (HF) therapy. Implantable cardioverter-defibrillators (ICDs) decrease mortality, and cardiac resynchronization therapy (CRT) improves symptoms and mortality. Most data have been collected in patients with systolic dysfunction. Data in patients with preserved systolic function are scarce, but interesting therapeutic concepts are evolving. Besides therapeutic functions, devices can have add-on diagnostic features such as early detection of fluid overload. In addition, stand-alone diagnostic devices are now also being developed for HF.

Patients with heart failure are frequently limited in their exercise capacity. Although this clinical phenomenon is mostly attributed to the failing myocardium, the effects of skeletal muscle wasting should not be underestimated. Muscle wasting may present in the form of loss of muscle mass and function, termed sarcopenia in healthy aging, or in the form of cachexia. Only cachexia is associated with loss of body weight. The mechanisms involved embrace an anabolic-/catabolic imbalance with increased degradation of myofibrils and myocyte apoptosis. Clinical effects include reduced muscle mass, s…

INTRODUCTION: Despite widespread use of loop diuretics in congestive heart failure (HF) to achieve decongestion and relief of symptoms, as recommended by the current guidelines, there is uncertainty as to their long-term therapeutic efficacy and safety. Their efficacy and safety compared to venous ultrafiltration are currently under investigation in acute decompensated HF patients.
AREAS COVERED: In this article, the authors review current available data related to efficacy and safety of loop diuretics and ultrafiltration in HF.
EXPERT OPINION: The literature review highlights an unmet clini…

Ventricular tachyarrhythmias are common in patients with congestive heart failure. The clinical presentation ranges from an asymptomatic incidental electrocardiographic finding to palpitations, syncope, and sudden cardiac death. Although implantable cardioverter defibrillators successfully prevent sudden cardiac death associated with ventricular fibrillation and ventricular tachycardia, recurrent implantable cardioverter defibrillators shocks remain a clinical management challenge. In this review, we discuss management strategies of ventricular tachycardia in congestive heart failure, includin…

Congestive fetal heart failure, defined as inability of the heart to deliver adequate blood flow to organs such as the brain, liver, and kidneys, is a common final outcome of many intrauterine disease states that may lead to fetal demise. Advances in fetal medicine during the past 3 decades now provide the diagnostic tools to detect and also treat conditions that may lead to fetal heart failure. Fetal echocardiographic findings depend on severity of diastolic and systolic dysfunction of both ventricles. At an advanced stage, findings include cardiomegaly; valvar regurgitation; venous congestio…

In the view of the aging of the general population and particularly the aging of the dialysis population, it is of considerable interest to examine the combined effects of age and uremia on cardiovascular status. The effects of uremia per se interact with the effects of aging in cardiovascular end-organ damage and in the genesis of heart failure (HF) and cardiorenal syndrome (CRS). Structural abnormalities with fibrosis and calcification of the heart and central arteries, along with autonomic nervous system dysfunction, underlie reduced cardiac performance leading to cardiac decompensation and…

Heart failure is the leading cause of death and hospitalization in industrialized countries and a major cause of healthcare costs. It is associated with severe symptoms and its prognosis remains poor. Further improvement is needed beyond the results of pharmacological treatment and devices. The role of nutrition has therefore been studied both in the early stages of heart failure, as a tool for the reduction of cardiovascular risk factors and in symptomatic heart failure, for the prevention and treatment of congestion and fluid overload. In addition, dietary supplements, such as n-3 polyunsatu…

Many signals have risen and fallen in the tide of investigation into mechanisms of myocardial hypertrophy and heart failure (HF). In our opinion, the multifunctional Ca and calmodulin-dependent protein kinase II (CaMKII) has emerged as a molecule to watch, in part because a solid body of accumulated data essentially satisfy Koch's postulates, showing that the CaMKII pathway is a core mechanism for promoting myocardial hypertrophy and heart failure. Multiple groups have now confirmed the following: (1) that CaMKII activity is increased in hypertrophied and failing myocardium from animal models …

Sodium and water retention in cardiac failure and cirrhosis is pivotal in the morbidity and mortality of patients with these disorders. Moreover, the pathophysiology of these edematous disorders is quite similar. Both disorders have activation of the renin-angiotensin-aldosterone system, increased sympathetic activity, and nonosmotic stimulation of arginine vasopressin, which is initiated by unloading of the arterial baroreceptors; this occurs secondary to diminished cardiac output with heart failure and primary systemic arterial vasodilation with cirrhosis. With this common pathophysiology ca…

Cystatin C (CysC) is an antiprotease useful for measuring kidney function. Beyond such property, it carries significant prognostic information in several fields of cardiovascular diseases. We review data that support CysC as a prognostic factor in cardiovascular diseases among healthy elderly, hypertensive and heart failure patients. In addition, it has been speculated that CysC may be an early marker of ventricular remodelling, primarily involved in its pathogenesis, as a local antiprotease. Its role in physiopathology and clinical issues of cardiovascular pathology is yet to be elucidated.

The focus of this review is to explore the controversy between the broad acceptance of loop diuretics as a first-line treatment for heart failure (HF) and the data regarding their association to increased morbidity and mortality in HF. We conclude that existing evidence is sufficient to recognize that diuretics are essential for the management of HF. Selection bias is the likely source of their connection to poor outcomes.

Systemic and pulmonary congestion is a central aspect of both acute and chronic heart failure and directly leads to many of the clinical manifestations of these syndromes. Therefore, diuretic therapy to treat congestion plays a fundamental role in heart failure management. However, although diuretics are the most common drugs prescribed for heart failure, there is limited quality evidence to guide their use. Unlike other components of the heart failure armamentarium, such as beta-blockers and angiotensin-converting enzyme inhibitors, diuretics (with the exception of aldosterone antagonists) ha…

Congestion causes the majority of hospitalizations for heart failure and contributes to heart failure progression and mortality. Intravenous loop diuretics reduce the signs and symptoms of congestion. Loop diuretics, however, may be associated with increased morbidity and mortality due to deleterious effects on neurohormonal activation, electrolyte balance, and cardiac and renal function. Ultrafiltration, an alternative method of sodium and water removal, safely improves hemodynamics in heart failure patients. Recent clinical trial data suggest that ultrafiltration may also reduce rehospitaliz…
