BACKGROUND: The role of low-molecular-weight heparin (LMWH) in preventing respiratory failure among patients with hypertriglyceridemia-associated acute pancreatitis (HTG-AP) remains uncertain. We aimed to examine the association between LMWH use and respiratory failure in HTG-AP.
METHODS: This post-hoc analysis was based on a multicenter, prospective cohort. Adult patients with HTG-AP (plasma triglycerides >11.3 mmol/L) admitted within 72 h of onset between February 2020 and July 2023 were included. Patients receiving LMWH during the first week were classified as the LMWH group, and those wit…

BACKGROUND: Antiphospholipid syndrome (APS) is a prothrombotic disorder that presents significant challenges during cardiovascular surgery due to its high risk of both thrombotic and hemorrhagic complications. Perioperative management of anticoagulation, particularly with heparin, requires careful balancing to prevent catastrophic outcomes such as thromboembolism or excessive bleeding.
CASE PRESENTATION: The patient was a 66-year-old man. Two years prior, he was diagnosed with deep vein thrombosis (DVT), and direct oral anticoagulant (DOAC) therapy was initiated. Further evaluation in the car…

Heparin-induced thrombocytopenia (HIT) is a life-threatening prothrombotic disorder with substantial clinical mortality, characterized by pathologic antibody formation against platelet factor 4 (PF4)-heparin complexes. This study identified early pathologic mechanisms that initiate HIT immune complex formation and regulate thrombosis development. We found that repeated heparin administration over five days markedly enhanced platelet activation which implies an additional activating mechanism. Through plasma proteomic analysis, we observed that histidine-rich glycoprotein (HRG) levels were sign…

Background/Objectives: Conventional surgical sutures can promote local coagulation after implantation, potentially compromising vascular patency. Heparin is an effective anticoagulant, but systemic administration may cause bleeding and thrombocytopenia. This study aimed to develop an electrospun heparin-loaded polycaprolactone (PCL) suture that combines structural support with sustained local heparin delivery and antithrombotic activity. Methods: Heparin was incorporated into electrospun PCL sutures at nominal heparin-to-PCL feed ratios of 0.05 and 0.50 wt%. Fiber morphology, chemical characte…

OBJECTIVES: The detection of low-range heparin activity is important to correctly assess heparin reversal and rebound, especially after cardiopulmonary bypass. Current parameters are either not available at point-of-care (anti-Xa activity [aXa] and activated partial thromboplastin time [aPTT]), insensitive (kaolin-activated clotting time [kACT]), or expensive (ROTEM viscoelastic test). We aimed to assess the performance of a recently proposed parameter from the Sonoclot viscoelastic test: the slope-45. We aimed to assess the effects of a range of low-dose heparin and protamine and their intera…

Early recognition and treatment of heparin-induced thrombocytopenia (HIT) are crucial to prevent severe complications. Although immunoassays offer rapid diagnosis, their sensitivity and specificity are suboptimal. Sequential combinations of quantitative immunoassay results improve their diagnostic accuracy. We aimed to validate 2 Bayesian approaches combining 2 rapid immunoassays and to compare their diagnostic performance with 2 other diagnostic approaches (Hamilton and TORADI-HIT algorithms). We included 1194 patients with suspected HIT, of whom 6.0% had confirmed HIT. HemosIL Acustar HIT-Ig…

Heparin, a clinically essential anionic biomacromolecule, is widely used as an anticoagulant and an antithrombotic agent. Real-time monitoring of heparin concentration is critical for precise dosage adjustment to minimize side effects. In this study, we developed two water-soluble cationic conjugated polyelectrolytes, PCBO-NMe3+ and PCBE-NMe3+, based on polycarbazole backbones, for sensitive and selective heparin detection. Synthesized via Suzuki and Sonogashira coupling polymerizations, followed by quaternization, these polymers demonstrated excellent fluorescence stability in buffer solution…

Glycosaminoglycan (GAG) molecules on the surface of red blood cells play an important regulatory role in the invasion of merozoites of apicomplexan protozoa. Heparan sulfate, a type of GAG molecule, has been identified as an important receptor facilitating the invasion of red blood cells by these parasites. Proteins in the parasite that exhibit strong affinity for heparin may play a pivotal role in this invasion process. This study aims to use proteomics to identify Babesia microti proteins with high binding affinity to heparin. Bioinformatics was utilized to analyze the subcellular localizati…

A young woman presented with portal vein thrombosis and thrombocytopenia and developed further extensive thrombosis following treatment with heparin. Despite no previous heparin exposure, she was found to be strongly positive for anti-platelet factor 4 (PF4) antibodies. She had acute cytomegalovirus (CMV) infection and a monoclonal gammopathy. Commentary on: Nicolson et al. Anti-PF4 mediated thrombocytopenia and thrombosis associated with acute cytomegalovirus infection displays both HIT-like and VITT-like characteristics. Tuberculosis (TB) is a major global health threat, with the current Bacillus Calmette-Guérin (BCG) vaccine having limited efficacy against adult pulmonary disease. Trained immunity (TI) is a form of innate immune memory that enhances antimicrobial defense. It is characterized by the epigenetic and metabolic reprogramming of innate immune cells and holds promise as a promising approach to prevent TB. In this study, we investigated the capacity of heparin-binding hemagglutinin (HBHA), a methylated antigen of Mycobacterium tuberculosis, to induce TI in murine RAW264.7 macrophages, human-derived T…

This report presents a case of an 81-year-old male with acute respiratory distress syndrome secondary to aspiration pneumonia who developed heparin-induced thrombocytopenia (HIT). His platelet count remained persistently low despite discontinuing unfractionated heparin and initiating intravenous argatroban. Multiple thromboembolisms, including a new aortic mural thrombus in the descending aorta, were observed on contrast-enhanced computed tomography (CT), resulting in a diagnosis of autoimmune HIT (aHIT). Subsequent high-dose intravenous immunoglobulin (IVIG) therapy substantially improved the…

BACKGROUND: Cardiac surgery is a high-risk setting for heparin-induced thrombocytopenia (HIT). However, large differences in its incidence, rate of thrombotic complications, and mortality have been reported in this context. Few studies address the pharmacologic management of HIT specifically in this setting.
OBJECTIVES: To describe the incidence, outcomes, and management of patients with HIT in our cohort and to compare them with patients presenting platelet factor 4/heparin antibodies but without platelet-activating capacity.
METHODS: A retrospective observational study was conducted over a…

BACKGROUND: Continuous renal replacement therapy (CRRT) is used in hemodynamically unstable patients with acute kidney injury (AKI). Heparin, the most commonly used anticoagulant, has a significant bleeding risk and is associated with heparin-induced thrombocytopenia. Regional citrate anticoagulation is an alternative anticoagulation strategy in CRRT.
MATERIALS AND METHODS: A randomized controlled trial was conducted in the Intensive Care Unit over one year, from October 2020 to September 2021, in patients with AKI requiring CRRT. Fifty-two patients were randomized into two groups: group 1 re…

Heparin is an important anticoagulant drug, and microbial heparin biosynthesis is a potential alternative to animal-derived heparin production. However, effectively using heparin synthesis enzymes faces challenges, especially with microbial recombinant expression of active heparan sulfate N-deacetylase/N-sulfotransferase. Here, we introduce the monosaccharide N-trifluoroacetylglucosamine into Escherichia coli K5 to facilitate sulfation modification. The Protein Repair One-Stop Service-Focused Rational Iterative Site-specific Mutagenesis (PROSS-FRISM) platform is used to enhance sulfotransferas…

Low Molecular Weight Heparins (LMWHs) are well-established for use in the prevention and treatment of thrombotic diseases, and as a substitute for unfractionated heparin (UFH) due to their predictable pharmacokinetics and subcutaneous bioavailability. LMWHs are produced by various depolymerization methods from UFH, resulting in heterogeneous compounds with similar biochemical and pharmacological properties. However, the delicate supply chain of UFH and potential contamination from animal sources require new manufacturing approaches for LMWHs. Various LMWH preparation methods are emerging, such…

INTRODUCTION: Unfractionated heparin is a widely used anticoagulant in critically ill patients. It has a well-established safety profile and remains an attractive option for clinicians due to its short half-life and reversibility. Heparin has a unique pharmacokinetic profile, which contributes to significant inter-patient and intra-patient variability in effect. The variability in anticoagulant effect combined with heparin's short half-life mean close monitoring is required for clinical efficacy and preventing adverse effects. To optimize heparin use in critically ill patients, effective monit…

INTRODUCTION: Heparin, a commonly used anticoagulant in cardiac surgery, binds to antithrombin III (ATIII) to prevent clot formation. However, heparin resistance (HR) can complicate surgical procedures, leading to increased thromboembolic risks and bleeding complications. Proper diagnosis and management of HR are essential for optimizing surgical outcomes.
METHODOLOGY: Diagnosis of HR involves assessing activated clotting time (ACT) and HR assays. Management strategies were identified through a comprehensive review of the literature, including studies exploring heparin dosage adjustments, ant…
