Suspected fibrosing cholestatic hepatitis in an immunocompetent child with hepatitis C: Case report.

A 12-year-old boy presented to our center with elevated aminotransferases, jaundice, right upper quadrant tenderness, and hepatomegaly. Evaluation revealed a highly elevated blood hepatitis C virus (HCV) ribonucleic acid (RNA) level, and liver biopsy showed canalicular and intrahepatic cholestasis, feathery degeneration of hepatocytes, and periportal/perisinusoidal fibrosis. Though there were atypical biopsy features, including absence of bile ductular reaction, we diagnosed suspected fibrosing cholestatic hepatitis (FCH). FCH secondary to HCV infection has been reported exclusively in immunos…

Stigma related to hepatitis B virus: A barrier to global viral hepatitis elimination.

Chronic hepatitis B virus (HBV) infection remains a major global health challenge, affecting over 300 million people worldwide and contributing substantially to liver-related morbidity and mortality. Despite the availability of effective vaccination, diagnostic tools, antiviral therapy, and surveillance strategies, progress toward the World Health Organization's hepatitis elimination targets has been slower than anticipated. HBV-related stigma has emerged as a critical and under-recognized barrier undermining efforts across the entire HBV care cascade. Stigma associated with HBV is multi-facet…

Risk of Hepatitis B Reactivation Among ALL Patients With Occult Hepatitis B Infection.

PURPOSE: Anticancer therapy increases the risk of hepatitis B virus (HBV) reactivation in patients with occult hepatitis B infection (OHBI), particularly in those who receive rituximab or stem-cell transplantation. However, the exact risk of HBV reactivation in patients with ALL/lymphoblastic lymphoma (LBL) receiving prolonged, intensive myelosuppressive chemotherapy is not known and thus the role of antiviral prophylaxis is not clearly established. PATIENTS AND METHODS: This prospective observational study was conducted at a single tertiary cancer center in India (October 2017 to August 2021…

Hepatitis B Reactivation After Kidney Transplantation in Hepatitis B Surface Antigen-Positive Recipients.

OBJECTIVES: Hepatitis B virus reactivation in HBsAg -positive kidney transplant recipients sometimes results in liver failure and death. Although current guidelines recommend continuing antiviral therapy for ≥6 months after the last dose of immunosuppressive therapy, in transplant recipients, duration is uncertain because immunosuppressive therapy is lifelong. Here, we evaluated the incidence and effects of hepatitis B virus reactivation in kidney transplant recipients with hepatitis B virus infection. MATERIALS AND METHODS: We reviewed patients who underwent kidney transplant between 2012 an…

Diagnosis and Management of Autoimmune Hepatitis After Hepatitis A in a Young Woman.

BACKGROUND Hepatitis A (Hep A) is a common cause of viral hepatitis in developing nations. Autoimmune hepatitis is characterized by subacute to chronic liver inflammation, which can lead to liver cirrhosis if untreated. Both conditions remain uncommon in the United States. Development of autoimmune hepatitis following viral Hep A is extremely rare. However, viral infections, including Hep A, have been implicated in triggering autoimmune diseases through immune system activation, molecular mimicry, and inflammatory cytokine surges. Early recognition of this phenomenon is crucial for timely inte…

[Hepatitis diagnostics : Differential diagnosis of viral hepatitis from other causes of elevated transaminases].

Liver biopsies in unclear hepatitis are required to assess etiology together with the clinico-serological findings to differentiate comorbidities. In addition, inflammatory activity should be graded, extent of necrosis estimated, and the stage of fibrosis determined in order to estimate pre-existing parenchymal damage. In liver biopsies from patients with acute liver failure, drug-induced liver injury, most often in idiosyncrasy is most common. While a liver biopsy is required for the diagnosis of autoimmune hepatitis, the diagnosis of viral hepatitis A-D is usually made clinically and serolog…

Chronic hepatitis E and hepatitis E seroprevalence in immunosuppressed patients: a prospective study.

BACKGROUND: Hepatitis E virus (HEV) is the commonest cause of acute viral hepatitis in Western countries. Immunocompromised patients may develop chronic hepatitis. This study aims at determining the seroprevalence and the prevalence of chronic HEV infections in a single tertiary center in Belgium. METHODS: From May 2022 to April 2023, we prospectively screened for HEV [immunoglobulin G (IgG), immunoglobulin M (IgM), and polymerase chain reaction (PCR)] all patients transplanted from a solid organ transplantation (SOT) and patients suffering from a lymphoma or followed for a bone marrow transp…
