Regional Anesthesia for Hip Arthroscopy.

Pain after hip arthroscopy can be severe, yet we lack a consensus method for non-narcotic analgesia. Here we describe anatomic elements of hip arthroscopy and our current understanding of the relevant sensory innervation as a prelude to the evaluation of locoregional analgesic techniques. Many regional nerve blocks and local anesthetic infiltration techniques are reviewed, including 2 newer ultrasound fascial plane blocks. Further study of targeted, motor-sparing approaches, either ultrasound-guided or under direct surgical visualization is needed.

Best Practices: Hip Femoroacetabular Impingement.

OBJECTIVE. Imaging plays a critical role in the assessment of patients with femoroacetabular impingement (FAI). With better understanding of the underlying pathomechanics and advances in joint-preserving surgery, there is an increasing need to define the most appropriate imaging workup. The purpose of this article is to provide guidance on best practices for imaging of patients with FAI in light of recent advances in corrective FAI surgery. CONCLUSION. Pelvic radiography with dedicated hip projections is the basis of the diagnostic workup of patients with suspected FAI to assess arthritic chan…

[Scanning Technique in Hip Ultrasonography].

This guide to ultrasound imaging of the hip describes the basic scanning planes and contains corresponding highresolution ultrasound images. The patient and probe positionings are explained in a simplified step-by-step manner using schematic drawings. This standard and simple examination technique seems to be an appropriate way to improve the learning curve of novice musculoskeletal sonographers. In this manuscript, an emphasis is put on hip effusion detection and on how to diagnose tendon and muscle injuries. Key words: tendons, hip tendinopathy, hip joint, athletic injuries, musculoskeletal,…

Trends in total hip arthroplasty.

This article discusses developments in total hip arthroplasty related to the use of highly cross-linked polyethylene (HXLPE) liner material as the new standard of care for acetabular cup articulation surfaces. The pathologic implications of metal-on-metal hip prostheses in asymptomatic patients also are discussed.

What's New in Pediatric Hip?

BACKGROUND: Developmental dysplasia of the hip (DDH), which encompasses a wide spectrum of disease from mild dysplasia to frank dislocation, is one of the most common developmental deformities of the lower extremities and one of the leading causes of future osteoarthritis and hip arthroplasty. Legg-Calvé-Perthes disease (LCPD) results from a vascular insult to the growing femoral epiphysis, which in turn can create permanent morphologic changes to the hip joint. Slipped capital femoral epiphysis (SCFE) occurs when the proximal femoral physis fails allowing the epiphysis to displace in relation…

Canine Hip Dysplasia: Diagnostic Imaging.

Diagnostic imaging is the principal method used to screen for and diagnose hip dysplasia in the canine patient. Multiple techniques are available, each having advantages, disadvantages, and limitations. Hip-extended radiography is the most used method and is best used as a screening tool and for assessment for osteoarthritis. Distraction radiographic methods such as the PennHip method allow for improved detection of laxity and improved ability to predict future osteoarthritis development. More advanced techniques such as MRI, although expensive and not widely available, may improve patient scr…

Capsular Management in Hip Arthroscopy.

The hip capsule is a highly complex anatomic structure, which influences normal hip motion and biomechanics. A dynamic stabilizing capsular contribution exists in the iliocapsularis and gluteus minimus, among other musculotendinous structures crossing the joint. Variable types and sizes of capsulotomy are necessary to sufficiently visualize and address the bony and soft tissue pathologic source of symptoms. Unrepaired capsulotomies may leave the hip significantly unstable to variable degrees. Capsular closure is a necessary part of a comprehensive arthroscopic hip preservation procedure. Great…

Arthroplasty for Pertrochanteric Hip Fractures.

The mainstay of treatment of pertrochanteric fractures is internal fixation using a sliding hip screw or a cephalomedullary device. However, in patients with ipsilateral hip osteoarthritis or avascular necrosis of the femoral head, or inflammatory arthritis, arthroplasty should be considered as the primary treatment modality to reduce the likelihood of a secondary procedure. Unstable fracture patterns with concomitant poor bone quality represent a challenge for internal fixation, with high rates of lag screw cut-out and hardware failure. Prosthetic replacement for unstable pertrochanteric frac…

Dermal burn during hip arthroscopy.

Radiofrequency devices are often used during arthroscopic surgery, most commonly of the shoulder and knee, and increasingly in hip arthroscopy. The most commonly described complication is elevation of joint temperature, leading to capsular shrinkage, chondrolysis, and nerve damage. A less commonly reported complication is that of dermal burns from the heated irrigation fluid. There are several case reports describing dermal burns after shoulder arthroscopy; however, to the authors' knowledge, there are none describing the complication in hip arthroscopy that is often performed by surgeons doin…

Hip disease in ankylosing spondylitis.

PURPOSE OF REVIEW: Hip disease occurs in about one-third of patients with ankylosing spondylitis (AS) and can often be disabling, necessitating total hip replacement in young adults. There have been recent articles on a number of aspects of this problem, including the epidemiology and pathology. The most recent studies on diagnosis, prognosis and therapeutic management are reviewed here. RECENT FINDINGS: Several large studies have evaluated the prevalence and outcome of hip involvement in AS. Hip involvement can be diagnosed clinically, radiologically, by MRI or by ultrasonography. These exam…

Anesthesia for hip replacement surgery.

Hip replacement surgery is a common and effective procedure for the relief of pain and loss of function. The number of procedures is increasing and great interest is shown for the manner of outcome improvement following hip replacement surgery. Last decade (2001-2010) is declared as the Bone and Joint Decade and has been characterized by many innovations in hip replacement surgery including minimally invasive technique but also by improvements in anesthetic technique. However there is no consensus about most appropriate anesthetic and analgesic techniques to use. Total hip replacement is proce…

Infection in periprosthetic hip fractures.

Fracture around the acetabulum and femur in total hip arthroplasty is a possible complication, sometimes with difficult surgical solution, namely when a sepsis is present. Periprosthetic hip fractures were classified according to a modified Vancouver classification. We treated 112 patients (67 men and 45 women) with periprosthetic hip fractures: 105 femoral fractures (94%) and seven acetabular fractures (6%). Mean follow-up was 10.6 years. For Type A (seven cases - 7%), B1 (10 cases - 10%) or C (13 cases - 12%) fractures around well fixed femoral stems we only fixed the fractures. For Type B2 …

[Hip arthroscopy - indications and limits].

Through continuous improvement and advancement of technology over the last 10 years, hip arthroscopy has developed into a standard orthopedic procedure and this has greatly expanded the range of indications. The main indications for hip arthroscopy include any disorder of the synovial membrane, damage to the labrum acetabulare, lesions of the ligament of the head of the femur, loose bodies, degenerative and traumatic cartilage lesions and especially disorders of the joint mechanism caused by femoroacetabular impingement. Increasingly, endoscopic procedures additionally allow the treatment of p…

[Neonatal hip dysplasia--differential diagnosis].

INTRODUCTION: Hip dysplasia is the elementary form of the developmental dysplasia of the hip (DDH). The diagnosis may be made by the ultrasound examination (types II a-, II b). OBJECTIVE: The aim of the authors was to define the congenital structural neonatal hip dysplasia, and to differentiate hip hypoplasia, hip dysplasia and teratogenic anomalies. METHODS: In all the cases, the uniform clinical and ultrasound tests were performed. All the patients were examined in the neonatal period of life (the first six weeks). The following clinical tests were used: Le Damany-Ortolany, Coleman-Barlow-…

Total hip replacement pseudotumoral osteolysis.

We report three cases of pseudotumoral lesion secondary to total hip replacement using metal on polyethylene bearings, presenting two distinct macroscopic aspects: (a) classic inflammatory granuloma and, in one case, (b) onset of hematoma associated with gluteal vessel lesions, probably affected by the pseudotumoral process. Diagnosis was radiographic, with CT-scan serving to confirm and, more importantly, to reveal extension and analyze surrounding tissue. Arteriography is needed when the lesion is liquid, and biopsy may be envisaged depending upon the clinical situation. In all three cases, …
