Metallic Lateralized-Offset Glenoid Reverse Shoulder Arthroplasty.

BACKGROUND: Metallic lateralized-offset glenoid reverse shoulder arthroplasty (RSA) for cuff tear arthropathy combines the use of a metallic … a regional interscalene nerve block. Subscapularis tenotomy and capsular release are performed, the humeral head is dislocated, and any osteophytes are removed. An intramedullary cutting guide is placed for correct humeral resection. The osteotomy of the humeral head is performed in the anatomical neck with an inclination of 135° and a retroversion of 20° to 4 …

The lateral ligament is injured preferentially in posterolateral dislocation of the elbow joint.

AIMS: The purpose of this study was to evaluate the relationships between the degree of injury to the medial and lateral collateral ligaments (MCL and LCL) and associated fractures in patients with a posterolateral dislocation of the elbow, using CT and MRI. METHODS: We retrospectively reviewed 64 patients who presented between March 2009 and March 2018 with a posterolateral dislocation of the elbow and who underwent CT and MRI. CT revealed fractures of the radial head, coronoid process, and medial and lateral humeral epicondyles. MRI was used to identify contusion of the bone and collateral

Open reduction through a posterior incision in the surgical treatment of shoulder posterior dislocation associated with proximal humeral fractures.

BACKGROUND: We explored the surgical technique of reducing the humeral head and repairing the fractures through a combined approach in the treatment of this complex injury. METHODS: Six patients with posterior shoulder dislocations associated with proximal humerus fractures were enrolled in this study. The posteriorly dislocated head was first reduced through a shoulder posterior incision and the ruptured posterior capsular tissues were repaired simultaneously using a suture anchor. The fractures were then reduced and fixed with a PHILOS through a deltopectoral approach. The affected shoulder…

Percutaneous balloon humeroplasty for Hill-Sachs lesions: a novel technique.

INTRODUCTION: Hills-Sachs lesions are commonly associated with anterior shoulder dislocations and can be a source of recurrent instability. … he void created by the balloon. We utilized computed tomography (CT) to collect volume data of each humeral head pre- and post-procedure. From this data, we calculated the volume of the Hill-Sachs defect and the percent corrected. A paired t test was performed to analyze the data statistically. RESULTS: The average prereduction Hill-Sachs defect volume was 15 …

Mechanical assessment of local bone quality to predict failure of locked plating in a proximal humerus fracture model.

The importance of osteoporosis in proximal humerus fractures is well recognized. However, the local distribution of bone quality in the humeral head may also have a significant effect because it remains unclear in what quality of bone screws of standard implants purchase. The goal of this study was to investigate whether the failure of proximal humerus locked plating can be predicted by the DensiProbe (ARI, Davos, Switzerland). A 2-part fracture with metaphyseal impaction was simulated in 12 fresh-frozen human cadaveric humeri. Using the DensiProbe, local bone quality was determined in the hum…

[Biomechanical investigations for the development of a SLAP-II-lesion].

The superior labral-biceps-tendon-complex forms an anatomical and functional unit and combines static and dynamic elements of shoulder stability. At present, only theoretical hypotheses exist on the etiology of the microtraumatic SLAP-II-lesion. To gain further insight into this, an instrument was developed to simulate throwing motions such as the late-cocking/early acceleration phase as well as deceleration/follow-through. Sixteen freshly frozen shoulder specimens were tested, varying the loads on the biceps tendon (25 N, 50 N, 100 N) and the compression of the humeral head against the glenoi…

Region-specific evaluation of Hounsfield units on elbow computed tomography as a predictor of osteoporosis in patients at risk.

BACKGROUND: Assessment of osteoporosis is crucial for favorable outcomes after trauma surgery, but a formal dual energy X-ray absorptiometry (DXA) scan may not be available preoperatively. The aim of the present study was to assess standardized measurements of Hounsfield units (HU) on computed tomography (CT) images of the elbow as a predictor for the presence of osteoporosis. METHODS: Patients who underwent both a CT scan of the elbow and a DXA measurement within a period of two years were included. Standardized HU measurements with a diameter of 3 mm each were carried out in defined regions

Biomechanical basis of transfers for shoulder paralysis.

Elevation of the scapulohumeral joint can be executed by two different and distinct systems, both capable of separately performing this move … fficient than those inserted on the humeral diaphysis. Vertical and horizontal stabilization of the humeral head in the glenoid fossa is the result of a complex balance between several muscles and passive factors. Trapezius and serratus anterior are essential for the rotation of the scapula around its centers of rotation.

Subscapularis Peel for Open Reduction and Internal Fixation of Proximal Humerus Fractures With a Head Split.

Proximal humerus fractures involving an articular head split are rare and complex injuries. In the elderly population, arthroplasty is the o … duction and internal fixation through a deltopectoral approach with a subscapularis peel to improve humeral head visualization, reduction, and fixation. The improved reduction may lead to better long-term outcomes and reduce the need for additional surgical procedures. In addition, there were no cases of avascular necrosis in this series.

Bilateral anterior glenohumeral dislocation and coracoid processes fracture after seizure: acute MRI findings of this rare association.

We report a rare case of bilateral anterior shoulder dislocation associated with coracoid processes fracture after a seizure episode in a 37-year-old woman. This combination of findings is rare, especially by the presence of bilateral coracoid processes fracture. Possible mechanisms related are direct trauma of humeral heads over the coracoid processes or sudden pull of biceps short head attachment site during uncontrolled contractures. Few reports published similar combination of injuries, and to our knowledge, this is the first to demonstrate its acute features by magnetic resonance imaging.…

From reduction to replacement: a reconstructibility-based framework for chronic locked shoulder dislocations.

PURPOSE: Chronic locked glenohumeral dislocation is uncommon, and treatment algorithms have traditionally relied heavily on defect-size thre … h worse functional outcomes following this procedure. Anatomical reconstruction may preserve viable humeral heads with larger defects, whereas arthroplasty is considered when reliable reconstruction is unlikely. CONCLUSION: Treatment should integrate defect morphology and size with chronicity, joint viability, glenoid bone stock, cuff and subscapularis integr …
