Parathyroid gland growth is a major cause of secondary hyperparathyroidism in renal failure. It is well known that high serum phosphate levels, low serum calcium levels and vitamin D deficiency are the three promoters of parathyroid hyperplasia in renal failure. Recent studies have investigated in depth the potential role of growth factors (transforming growth factor alpha) and their receptors (epidermal growth factor receptor) in the pathogenesis of parathyroid cell hyperplasia in chronic renal failure. The identification of molecular mechanisms involved in calcium, phosphate and vitamin D ma…

Surgery for primary hyperparathyroidism has become one of the least invasive and safe procedures with the recent advances in the diagnosis and surgical technique. However, it is still difficult to prospect detailed post-operative conditions of the patient from the data of pre-operative examination. Because many factors related each other to make the preoperative conditions complicated. Thus, integrated interpretation of the patient is necessary before surgery, including general condition, coincidental disease, renal function, bone mineral density and its turnover, size and location of the lesi…

OBJECTIVE: Coeliac disease (CD) may present in its classical form with diarrhoea and weight loss, but also with atypical symptoms that are both related and unrelated to malabsorption. Osteomalacia or osteopenia following malabsorption of calcium and vitamin D is known to occur in patients with CD, and in such cases secondary hyperparathyroidism (SHP) caused by low serum calcium levels is frequently found. However, the prevalence of CD in subjects with SHP has not been reported.
MATERIAL AND METHODS: In the Tromsø study 2001, serum parathyroid hormone (PTH) and calcium were measured in 7954 su…

Secondary hyperparathyroidism (SHPT), a frequent complication of chronic kidney disease, develops in response to an imbalance in the serum levels of calcium, phosphorus and vitamin D as a result of altered metabolism. Raised serum levels of parathyroid hormone (PTH) and calcium-phosphorus product have a major effect on morbidity and mortality in dialysis patients. The new Kidney Disease Outcomes Quality Initiative (K/DOQI) guidelines, formulated by the National Kidney Foundation in the USA, propose strict targets for the control of serum levels of PTH, calcium and phosphorus. Meeting these tar…

Preliminary data are available on the coexistence of primary hyperparathyroidism and breast carcinoma. To further understand the association between hyperparathyroidism and breast cancer, we conducted a record-linkage study in Sweden using the Swedish Cancer Registry from 1958-1997. A total of 9,835 women who underwent surgery for primary parathyroid adenoma were followed to evaluate the hypothesis that a history of primary hyperparathyroidism increases the risk of subsequent breast cancer. During 99,929 person-years of follow-up, 331 cases of newly diagnosed breast cancer were reported. The n…

Nearly 50,000 new cases of primary hyperparathyroidism (PHPT) are diagnosed annually in the United States. Most information about the disease focuses on the white population. We evaluated African American (AA) and white patients at our tertiary care university medical center to determine whether there was a racial difference in presentation of PHPT. A retrospective chart review of patients treated surgically for PHPT between 1997 and 2002 was performed. Demographic data, laboratory values, objective symptoms, surgical procedure, and histologic findings were recorded. The AA participants were m…

The prevalence of hyperparathyroidism as increased in the last 20 years due to a longer number of biochemical determinations, in which we observe in many cases hipercalcemia by casualty. The simptomatology of the primary hyperparathyroidism is very subtile and sometimes none in an 80% of the patients. Between november 1996 and march 2003 we have operated 55 parathyroidectomies. 36 patients (65,4%) had primary hyperparathyroidism, 16 (29,1%) secondary hyperparathyroidism and the rest (5,4%) tertiary hyperparathyroidism. Our hospital has a device of rapid detection of the parathyroid hormone lev…

OBJECTIVE: To review the mechanism of action, development, and clinical application of the calcimimetic compounds being investigated for the treatment of primary and secondary hyperparathyroidism (HPT).
DATA SOURCES: A MEDLINE search (1990-April 2004) was performed to identify all published articles related to calcimimetics. Published abstracts over the previous 5 years from various scientific meetings (American Society of Nephrology, American Society for Clinical Pharmacology and Therapeutics, American Society of Bone and Mineral Research) were also searched for reports regarding investigati…

Calcimimetic agents function as allosteric activators of the calcium-sensing receptor (CaSR). In parathyroid tissue, they decrease the threshold for CaSR activation by extracellular calcium ions and diminish parathyroid hormone (PTH) secretion directly. Results from small clinical studies in hemodialysis patients with secondary hyperparathyroidism have shown that single oral doses of calcimimetic compounds abruptly decrease plasma PTH levels within 1 to 2 hours in a dose-dependent manner. Sustained decreases in plasma PTH levels can be achieved when daily oral doses are given for as long as 18…

Primary hyperparathyroidism (HPT) has catabolic effects on cortical bone and anabolic effects on cancellous bone with overall deleterious effects on skeleton. Primary HPT is associated with increased fracture risk both at the cancellous bone-enriched spine and the cortical bone-enriched distal one third of the radius. This risk is reversed by parathyroidectomy.

Classic pathogeneses of secondary hyperparathyroidism (2HPT), hyperphosphatemia, vitamin D deficiency, and hypocalcemia, have been treated by the administration of phosphorus binders and vitamin D derivatives. However, these therapies have not brought about a successful result. The main reason could be attributed to hypercalcemia resulting from the administration of calcium salts as a phosphorus binder and the calcemic action of vitamin D. To prevent hypercalcemia, non-calcium-containing phosphorus binders and vitamin D analogues, which suppress parathyroid hormone (PTH) secretion with minimum…

AIMS: To determine whether, in secondary and tertiary hyperparathyroidism (HPT), quick parathyroid hormone (PTH) assay can be used to prevent persistent or recurrent HPT. Another point was to determine, considering the PTH decrease, the cut-off point at which the operation could be considered well performed.
METHODS: A retrospective study was performed evaluating all cases operated on since 1975 until 2002, 679 patients, and particularly the analysis of two groups of patients that underwent surgical treatment for secondary and tertiary HPT in the period 1995-2002. In the first group, (January…

Parathyroidectomy is effective in avoiding complications of primary hyperparathyroidism (pHPT) during pregnancy, because in about 90% of cases, it is caused by parathyroid adenomas. We present a case of severe pHPT during pregnancy of unknown etiology and without detectable parathyroid adenoma so that surgery was not a therapeutic option. The patient was a 29-year-old gravida 1 who presented with pHPT at 11 weeks of gestation. Although her serum calcium levels were elevated throughout pregnancy (>3.05 mmol/l; reference range: 2.05-2.65), a vigorous premature infant was delivered at 35 weeks of…

A cat was presented for anorexia and vomiting. Hyperthyroidism and primary hyperparathyroidism were diagnosed. A thyroid adenoma and parathyroid adenoma, respectively, were responsible for these conditions. Unilateral thyroidectomy and parathyroidectomy successfully resolved both disorders.

The aim of this study was to assess the spectrum of radiographic findings in primary hyperparathyroidism (PHPT). The study group consisted of 16 women and 7 men whose Ca levels were at least two or three times higher than normal. The average age was 55.3 in women and 49.4 in men. We detected carcinoma in 1, hyperplasia in 1, multiple adenomas in 4, single adenoma in 17 patients. The most common finding in the skeletal system was the decreased bone mineral density (BMD) and the complete loss of the lamina durae dentium. BMD was found lower in women than in men. This result attributed the increa…

As it becomes common for hyperparathyroidism to be detected earlier in the course of the disease, the bony manifestations of the disease are rarely seen. Presented here is a rare case of brown tumor of the maxilla as the presenting symptom of primary hyperparathyroidism.
