Colloids versus crystalloids for fluid resuscitation in critically ill patients.

BACKGROUND: Colloid solutions are widely used in fluid resuscitation of critically ill patients. There are several choices of colloid and there is ongoing debate about the relative effectiveness of colloids compared to crystalloid fluids. OBJECTIVES: To assess the effects of colloids compared to crystalloids for fluid resuscitation in critically ill patients. SEARCH STRATEGY: We searched the Cochrane Injuries Group's specialised register, CENTRAL, MEDLINE, EMBASE, the National Research Register, Web of Science and MetaRegister. Bibliographies of trials and review articles retrieved were sear

Tonicity of oral rehydration solutions affects water, mineral and acid-base balance in calves with naturally occurring diarrhoea.

Recommendations for composition of oral rehydration solutions (ORS) for calves, particularly concerning Na+ , glucose, and their combined effect on tonicity, are not in line with guidelines for humans. Thus, this study aimed to determine the effect of ORS tonicity on water, mineral and acid-base balance. Seventy-two calves were selected based on the severity of dehydration and blood base excess (BE) on day 0. Five calves that did not develop diarrhoea were removed post-inclusion from the study. Calves were allocated to blocks of four animals based on blood BE on day 1. Within each block, calve

Syndrome of inappropriate secretion of antidiuretic hormone as a leading cause of hyponatremia in children who underwent chemotherapy or stem cell transplantation.

BACKGROUND: Hyponatremia is a common metabolic disorder in cancer patients. However, little information is available for patients receiving chemotherapy or stem cell transplantation (SCT). We analyzed the frequency, characteristics, and various causes of hyponatremia including routine use of hypotonic fluids in children following chemotherapy or SCT. PROCEDURE: We reviewed the clinical and laboratory data of 63 children who received chemotherapy or SCT at the Department of Pediatrics, Hanyang University Medical Center from July 2005 to July 2008. RESULTS: All 63 patients at admission receive

Effects of chemical stimulation of masseter muscle nociceptors on trigeminal motoneuron and interneuron activities during fictive mastication in the rabbit.

An electrophysiological study was carried out in sixteen decerebrate and paralyzed New Zealand rabbits to determine how a bolus injection of a nociceptor stimulant (hypertonic saline, 5%) into the masseter muscle influences the activity of the trigeminal motor circuitry during fictive jaw movements. Hypodermic needles connected to a syringe held in a computer-controlled infusion pump were inserted into the anterior deep layer of either the right or the left masseter. Twenty-three infusions of 50, 70 or 80 microl saline were made in fourteen animals at constant rates over 1 min. Eight control i

Optometric management of Fuchs's endothelial dystrophy.

Fuchs's endothelial dystrophy interferes with removal of fluid from the cornea by the endothelium. It progresses through stages of corneal guttata, corneal edema, bullous keratopathy, vascularization, and scarring, and complications such as glaucoma or infection. The optometrist can detect this disorder and manage its early stages as well as provide counseling and proper referral. This paper includes a case study and a discussion of the course and treatment of this disease.

Spontaneous acetylcholine release in mammalian neuromuscular junctions.

Spontaneous secretion of the neurotransmitter acetylcholine in mammalian neuromuscular synapsis depends on the Ca2+ content of nerve termina … kinetics, had a lower effect. The increase in MEPP frequency induced by exposing the preparation to hypertonic solutions was affected by neither external Ca2+ concentration nor L-, N-, and P/Q-type VDCC blockers, indicating that extracellular Ca2+ is not necessary to produce hyperosmotic neurosecretion. On the other hand, MEPP frequency was diminished by BAPTA-AM and …

Regulation of water permeability in rabbit conjunctival epithelium by anisotonic conditions.

Effects of unilateral exposure to anisotonic conditions on diffusional water permeability of the isolated rabbit conjunctiva were determined. A segment of the bulbar-palpebral conjunctiva was mounted between Ussing-type hemichambers under short-circuit conditions. Unidirectional water fluxes (J(dw)) were measured in either direction by adding (3)H(2)O to one hemichamber and sampling from the other. Electrical parameters were measured simultaneously. J(dw) were determined under control isosmotic conditions and after introduction of either hyper- or hypotonic solutions against the tear or stroma

Investigating intermittent immersion during osmotic dehydration of mango (Mangifera indica L. Moench). Part A: Determination of optimal conditions for mango (Mangifera indica L. Moench) dehydration impregnation by immersion (D2I) and intermittent immersion (D3I).

This work aimed to determine the optimum conditions for dehydration impregnation by immersion (D2I) and by intermittent immersion (D3I) of mango (Mangifera indica) slices measuring 4 × 1 × 1 cm3. To this end, the Doehlert response surface plan was used, with the following factors for D2I: the volume of D2I solution/fruit mass ratio (6/1-13/1 mL/g), the process time (120-360 min) and the Brix degree of the solution (45-65 °Brix) and with the following factors for D3I: immersion time (20-60 min), process time (60-300 min) and de-immersion time (7-25 min). The temperature was fixed according to l

Intestinal and renal effects of low-volume phosphate and sulfate cathartic solutions designed for cleansing the colon: pathophysiological studies in five normal subjects.

OBJECTIVES: Ingestion of a concentrated low-volume phosphate solution produces copious diarrhea, which cleanses the colon, but it occasionally causes renal failure due to calcium phosphate precipitation in renal tubules. We hypothesized that a concentrated low-volume sulfate solution would be an equally effective cathartic, and that urine produced after sulfate would have less tendency to precipitate calcium salts than urine produced after phosphate. METHODS: Hydrated subjects ingested 75 ml of phosphosoda or an equimolar dose of sulfate salts in a small volume of solution. Four liters of PEG

Effectiveness of chest physiotherapy and pulmonary rehabilitation in patients with non-cystic fibrosis bronchiectasis: a narrative review.

Respiratory physiotherapy and rehabilitation are important therapeutic options in non-cystic fibrosis bronchiectasis (NCFB). The aims of this review of clinical trials were to evaluate the safety and the effects on physiologic and clinical outcomes of airway clearance techniques (ACTs) and rehabilitation in NCFB patients, in comparison to usual care. The search was performed on March 2018 by using PubMed and PeDro databases. 33 studies were selected. The use of ACTs for NCFB were effective in increasing sputum volume although no benefit in quality of life (QoL) or pulmonary exacerbations were

[Selective termination of the development of the defective fetus in discordant twin pregnancies].

Selective termination of the affected fetus in twin pregnancies was performed in the second trimester of seven pregnancies. The malformations included anencephaly/exencephaly (2 cases), hydrocephalus (1 case), thoracoabdominopagus of "B" and "C" cotwins (1 case), urethral obstruction sequence (1 case) and hygroma colli (2 cases). Intrauterine intervention on the affected fetus was done by transabdominal intracardial injection of 20% NaCl solution in the 15--24 weeks of gestation. All cases had dichorionic placentation. Unaffected co-twin infants were delivered at term with normal weight in 4 c

Airway deposition of hygroscopic heterodispersed aerosols: results of a computer calculation.

A new computer model is developed and used to calculate the deposition of inhaled heterodispersed hygroscopic aerosols for mouth breathing in a Weibel symmetric bronchial tree. The model was first validated by obtaining good agreement with recent experimental and theoretical data on regional and total airway deposition of monodispersed and heterodispersed nonhygroscopic aerosols. The model was then used to obtain predictions of regional and total deposition of heterodispersed hygroscopic aerosol particles (droplets of NaCl solutions). Parameters that were varied in the hygroscopic calculations

Discharge properties of mechanosensitive afferents supplying the retroperitoneal space.

Functional properties of lumbar mechanosensitive afferent fibres in the white rami L3 to L4 and the lumbar splanchnic nerves, which supply the retroperitoneal space were investigated. The receptive fields of these afferents, consisting of 1 to 3 mechanosensitive sites, were found on large vessels (aorta, inferior mesenteric artery, segmental arteries), on nerves, on the peritoneum, in fat lobules, on lymph nodes and on the vertebral column. Most afferent units were associated with small vessels. About 50% of the afferents had ongoing activity (about 0.1 to 2 imp/s). Two thirds of the afferents

Effects of three hydrogen-rich liquids on hemorrhagic shock in rats.

BACKGROUND: Hydrogen-rich saline provides a high concentration of hydrogen, which selectively reduces levels of hydroxyl radicals and alleviates acute oxidative stress in many models. We investigated the protective effects and mechanisms of three different hydrogen-rich liquid resuscitation preparations on lung injury-induced uncontrolled-hemorrhagic shock (UHS) in rats. MATERIALS AND METHODS: A UHS rat model was prepared using the method of Capone et al. of arterial bleeding and tail amputation. Healthy male Wistar rats were randomly divided into seven groups (10 per group) to receive: sham
