Feasibility of Laparoscopic Transabdominal Preperitoneal Hernioplasty for Incarcerated Inguinal Hernia.

BACKGROUND/OBJECTIVES: An incarcerated inguinal hernia (IncIH) is defined as an irreducible inguinal hernia. The problems caused by long-term incarceration are adhesion between the incarcerated organ and hernial sac and difficulty in reduction of the edematous incarcerated organ. The present study was performed to evaluate the feasibility of laparoscopic transabdominal preperitoneal (TAPP) hernioplasty to treat chronic IncIH. METHODS: This retrospective study included 154 patients aged >20 years who were treated for IncIH from January 1, 2015 to October 31, 2020. Patients were categorized int…

Civil Rights Litigation Involving the Dental Care of Incarcerated Persons.

Individuals incarcerated in jails, prisons, and detention centers are unique because they have an entitlement to health care under the U.S. constitution; however, the dental care they receive is highly variable in scope and quality. We identified federal civil rights lawsuits related to dental care by incarcerated individuals that were decided in federal district courts in 2018 and courts of appeals since 1976. Judges dismissed or denied the majority of dental claims. Courts have little guidance to inform decisions relating to dental care because cases rarely have a dental expert and the denta

Association Between Social Support and Mental Health of Incarcerated Individuals.

Mental illness occurs more frequently in incarcerated individuals than in the general population. This study examined whether social support during incarceration is associated with improved mental health outcomes. Data were used from the 2004 Survey of Inmates in State Correctional Facilities; the analytic sample was 3,451 incarcerated individuals. Linear regression models predicted current mental health functioning from frequency of visits and phone calls while incarcerated. Controlling for demographic characteristics, incarcerated individuals with more frequent contact from family and friend…

Subtotal Appendectomy to Treat Gangrenous Appendicitis Incarcerated in an Obturator Hernia.

Obturator hernias are quite rare in the world of hernias, and one that contains an acutely inflamed appendix becomes a very rare case indeed. An obturator hernia containing the appendix has been reported only 5 times in the surgical literature. Three of those cases were in elderly women with delayed presentations and were fatal. The most recent case was in a young woman with prompt laparoscopic appendectomy and no complications. We present a case in a 25-year-old man presenting with acute appendicitis and found to be incarcerated within the obturator canal during laparoscopy. The patient was t

MICRO-ELIMINATION OF HEPATITIS C IN THE INCARCERATED POPULATION: IS IT REALLY POSSIBLE?

According to the World Health Organization, 71 million people live with chronic hepatitis C. The treatment of this disease requires assistance from specialized physicians and a highly complex health care system. The prison population has been recognized as being at a high risk of acquiring confinement-related infections, including viral hepatitis. Hepatitis C virus (HCV) infection is a primary cause of death owing to liver disease among liberty-deprived individuals. Generally, prisons do not have adequate isolation wards for persons with communicable diseases, and overcrowding is a risk factor

Angle grinder as surgical tool for incarcerated penoscrotal steel ring.

Penile strangulation with concomitant scrotal entrapment by a steel ring is an extremely rare urological emergency that requires immediate intervention. Any delay may lead to irreversible complications. Metal rings increase penile engorgement and are usually associated with an attempt to improve sexual pleasure or to maintain a prolonged erection. The removal of steel rings can be challenging and may require a multidisciplinary approach. We present a unique case report of an 18-year-old male with a penoscrotal steel ring retained for 24 hours that was safely removed using an angle grinder as a

Incarcerated gallbladder in inguinal hernia: a case report and literature review.

BACKGROUND: Treating hernias is one of the oldest challenges in surgery. The gallbladder as content in the case of abdominal hernias has only been reported in a few cases in the current literature. Cholecyst has only been described in the content of an inguinofemoral hernia in one case to date. CASE PRESENTATION: A 73-year-old female patient was admitted to the Emergency Department due to complaints in the right inguinal area, which had started 1 day earlier. The patient complained of cramp-like abdominal pain and nausea. Physical examination confirmed an apple-sized, irreducible hernia in th

Cognitive Training for Very High Risk Incarcerated Adolescent Males.

OBJECTIVE: Persistent violent and antisocial behavior, as manifested in conduct disorder (CD) traits, are associated with a range of cognitive deficits. Individuals with more severe cognitive deficits are more likely to commit violent crimes. Currently, no treatments target improving cognition in high-risk CD youth. This pilot study tests the feasibility and efficacy of delivering intensive tablet-based cognitive training (CT) to adolescent males incarcerated in a youth maximum-security prison. METHODS: Participants were fourteen adolescent males, diagnosed with CD. All participants completed

'I took care of my kids': mothering while incarcerated.

BACKGROUND: Little is known about how incarcerated mothers make meaning of their parenting role and relationship with their children prior to incarceration and during custody. The aims of this project were to explore the experiences of mothering prior to incarceration and during custody using the Gendered Pathways Perspective and to examine how mothering intersects with incarcerated women's health and health outcomes to facilitate prevention and intervention strategies. This secondary data analysis used qualitative methods and grounded theory to identify themes related to mothering from 41 inc…

Emergency laparoscopic repair of an incarcerated obturator hernia, a case report.

Obturator hernias are rare hernias with high morbidity and mortality if not diagnosed and treated promptly. Various minimally invasive techniques have been reported in literature. Here we describe a novel technique of repairing an obturator hernia complicated by small bowel incarceration and obstruction, via a laparoscopic trans-abdominal approach.

Housing, Relationships, and Substance Use among Formerly Incarcerated Females.

The need for housing is an important concern for most women about to be released from prison, especially by those that need substance use treatment. This study explored the association between housing settings, relationships, and substance use. Two hundred adult females that had endorsed previous incarceration within the past two years participated in the initial baseline study. This study used multi-level modeling to analyze if housing settings and/or relationships within a living condition are associated with substance use. Results indicated that participants in homeless, and mutual settings…

Predicting quality of life during and post detention in incarcerated juveniles.

PURPOSE: Besides reducing recidivism, juvenile justice institutions aim to rehabilitate juvenile delinquents, in order for them to reintegrate in society. As such, improving quality of life (QoL), especially post detention, is an important treatment goal. However, research is primarily focused on recidivism as an outcome measure for juvenile detention. The aim of the current study is therefore to describe and predict QoL of detained young offenders up to 1 year after an initial assessment, and to examine whether QoL differs between youth who are still detained versus released. METHODS: A samp

Externalizing Behavior Among Youth With a Current or Formerly Incarcerated Parent.

This study examined associations between parental incarceration and youths' externalizing behaviors (e.g., damage to property, fighting, theft, etc.). Data were drawn from the 2016 Minnesota Student Survey, a statewide sample of 126,868 youth in public schools. Logistic regression analyses examined associations between youths' experience of parental incarceration and their self-reported externalizing behaviors, controlling for key demographic characteristics. Youth with a currently or formerly incarcerated parent reported significantly more externalizing behaviors compared with youth who never…

Reproductive justice for incarcerated mothers and advocacy for their infants and young children.

The United States has seen unprecedented growth in the number of incarcerated women, most of whom are mothers with minor children. Major public health concerns relate to the reproductive health of women in prisons and jails and the well-being of their infants and young children. In the current article, we use a reproductive justice framework to examine the intersection of incarceration and maternal and child health. We review (a) current research on the reproductive health of incarcerated women, (b) characteristics and experiences of pregnant incarcerated women, (c) outcomes of infants and you…

Who Should Make Decisions for Unrepresented Patients Who Are Incarcerated?

The United States has a high incarceration rate. Incarceration is associated with increased risk for cancer, chronic illness, serious mental illness, and substance use disorder. People who are incarcerated are less likely to be offered or participate in advance care planning, less likely to document their treatment preferences, and might not have a surrogate if one is needed. This article explores medical decision making for patients who are incarcerated and unrepresented and considers advantages and disadvantages of different classes of decision makers for them.
