A near miss: an uncommon injury following a common mechanism.

Subtalar dislocation is an uncommon injury involving the simultaneous dislocation of the talocalcaneal and talonavicular joints. Radiographic images can be difficult to interpret for the inexperienced clinician because of the obliquity of the foot and the overlap of tarsal bones. The authors describe the case of a 24-year-old male who presented to the emergency department (ED) with a painful left foot and ankle following a twisting injury. He was examined by a junior member of the ED team and diagnosed with a left ankle sprain. Preparations were underway for discharge home when the radiographs

Reconstruction of the medial patellofemoral ligament for the treatment of habitual or recurrent dislocation of the patella in children.

We investigated the clinical outcome of a reconstructive procedure of the medial patellofemoral ligament for the treatment of habitual or recurrent dislocation of the patella in four children (6 knees), with a minimum follow-up of four years. The technique involves transfer of the tendon of semitendinosus to the patella using the posterior one-third of the femoral insertion of the medial collateral ligament as a pulley. There was no recurrence of dislocation after surgery. The mean Kujala score at follow-up was 96.3 points. Radiological assessment showed that the congruence angle, the tilt ang

Imaging of the skeleton and soft tissue in children.

Normal metaphyses in infants contain a step-off not to be confused with child abuse. Extension of physeal lucency into the metaphyses can be a sign of healing child-abuse fracture of experimental physeal fracture, as well as deferoxamine overdose. Sonography can reveal costochondral dislocation, the pattern of healing fracture callus, the patellar tendon in jumper's knee, synovium of rheumatoid arthritis, sequestrum in advanced osteomyelitis, the unossified dislocated patella, and, even after 2 years of age, the dislocated femoral head. Further developments concern bone scanning, MR imaging, C

Anomalous origin of the sural nerve in a patient with tibial-common peroneal nerve anastomosis.

Recognition of anomalous innervation patterns may be important in dealing with peripheral nerve injuries. We describe a patient who suffered injury to the common peroneal nerve (CPN) at the knee when he laterally dislocated his knee. Intraoperative nerve conduction studies during surgical exploration revealed that nerve fibers from the tibial nerve crossed over to join the CPN immediately distal to a neuroma incontinuity. The sural nerve was also found to arise totally from the CPN.

Reconstruction of medial patellofemoral ligament for chronic patellar instability.

BACKGROUND: Disruption of the capsule, medial patellar retinaculum, and/or vastus medialis obliqus has been associated with recurrent patellar instability. Biomechanical studies have shown that the medial patellofemoral ligament (MPFL) is the main restraint against lateral patella displacement and reconstruction of the MPFL has become an accepted surgical technique to restore patellofemoral stability in patients having recurrent patellar dislocation. We report a prospective series of patients of chronic patellar instability treated by reconstruction of medial patellofemoral ligament. MATERIAL

Popliteal pterygium knee contracture: treatment with the Ilizarov technique.

INTRODUCTION: Various treatment options are in use to address severe knee flexion contractures in children. Their success depends on an adequate selection of the proper one applying to each individual anatomical situation. HYPOTHESIS: Applied to limb deformity, the Ilizarov technique combines progressive correction, to joint structures flexibility restitution in case of severe knee flexion contracture. We review a continuous series of popliteal pterygium syndrome patients managed with this technique. PATIENTS AND METHODS: Medical records of eight children (11 knees), consecutively treated be

Presentation and Repair of Serial Misdiagnosed Congenital Patellar Dislocation.

Congenital patellar dislocation (CPD) is a rare deformity in children that involves a laterally displaced patella. While potentially identified in early childhood using diagnostic imaging techniques, it is often misdiagnosed at birth, creating pain and mobility issues as the child grows. Dislocation of the patella is permanent and manually irreducible, often manifesting with flexion contracture of the knee, genu valgum, external tibial torsion, and foot deformity. Surgical correction is the treatment of choice in order to prevent future sequelae. We herein present a case of CPD in a four-year-

Imaging of patellofemoral disorders.

Anterior knee pain is a common symptom, which may have a large variety of causes including patellofemoral pathologies. Patellofemoral maltracking refers to dynamic abnormality of patellofemoral alignment and has been measured using plain film, computed tomography (CT) and magnetic resonance imaging (MRI) using static and kinematic techniques. Patellar dislocation is usually transient, but specific conventional radiographic and MRI features may provide evidence of prior acute or chronic dislocation. In addition, chondromalacia patellae, osteochondritis dissecans, patellofemoral osteoarthritis,

Incidence and risk factors of acute traumatic primary patellar dislocation.

PURPOSE: The purpose of this study was to investigate incidence, nature, and risk factors of primary traumatic patellar dislocations. METHODS: We identified acute first-time traumatic patellar dislocations from a national hospital discharge register. Patients with previous patellar dislocations, subluxations, or knee traumas were excluded. The sample consisted of 128,714 Finnish male conscripts (median age 20). Background risk factor data were obtained from a Finnish conscript service database. The dislocators were observed during their service period for a short-term outcome. RESULTS: From

In vivo movement analysis of the patella using a three-dimensional computer model.

We used three-dimensional movement analysis by computer modelling of knee flexion from 0 degrees to 50 degrees in 14 knees in 12 patients with recurrent patellar dislocation and in 15 knees in ten normal control subjects to compare the in vivo three-dimensional movement of the patella. Flexion, tilt and spin of the patella were described in terms of rotation angles from 0 degrees . The location of the patella and the tibial tubercle were evaluated using parameters expressed as percentage patellar shift and percentage tubercle shift. Patellar inclination to the femur was also measured and patel

Nail-patella syndrome in Saudi Arabia with new features and surgical procedures: the first described study.

The purpose of this study was to reveal the occurrence of nail-patella syndrome (NPS) in Saudi Arabia together with the detection of abnormal attachment of lateral meniscus in the left knee and new surgical procedures applied to the right and left knee, reported for the first time in this study. This was a case study of a 23-year-old young man presenting with bilateral knee pain, giving way and locking since the age of 15 years. Clinically, most of the NPS features were noted, including ocular problems. The complex features affected both knees, especially the previous attempted surgeries for r

Acute Management of Combined Floating Hip and Knee Injury: A Case Report.

INTRODUCTION: Surgical management of isolated acetabular fracture dislocations, femoral shaft fractures, and tibial shaft fractures are well-documented, but there is limited literature on the acute management of these injuries when they occur concomitantly in the same patient on the ipsilateral extremity. We present a case of the acute treatment of a 33-year-old patient with a right ipsilateral floating hip and knee injury secondary to a motor vehicle accident. We offer a unique surgical technique for the urgent management of this rare combined injury. CASE PRESENTATION: A 33-year-old female

Lower Limbs Alignment in Patients with a Unilateral Completely Dislocated Hip.

BACKGROUND: Severe hip osteoarthritis is known to lead to secondary osteoarthritis of the knee joint. It is not clear whether contracture or a leg length discrepancy is more important in determining the knee alignment. METHODS: In this study, 48 hips in 48 patients with a unilateral completely dislocated hip (Crowe IV) were recruited. The patients were divided into two groups (Crowe IVa and IVb). The Crowe IVa group had completely dislocation with psudo-articulation, and the Crowe IVb group had completely dislocation without psudo-articulation. The lower limb alignment was divided into three

Kinematic assessment of hip movement when retrieving an object from the floor.

BACKGROUND: Activities that require extreme hip movement can dislocate hip implants in the early post operative phase. One such activity is retrieving an object from the floor. The aim of this study was to assess hip movement using four different techniques to accomplish this task. This assessment would identify the techniques least likely to cause a hip dislocation. METHODS: An electromagnetic tracker was used to measure the movement of 50 hips in 25 normal subjects. Sensors were attached over the iliac crest and the mid-shaft of the lateral thigh. Data was then collected for 3 repetitions o

The influence of coronal plane deformity on mediolateral ligament status: an observational study in varus knees.

Most surgeons believe that varus deformity leads to progressive tightness of the medial soft tissue envelope and laxity on the lateral side. It is, however, unclear at what stage of the deformity such ligament alterations occur, and whether these are the consequence of intrinsic alterations in the ligaments themselves, or rather due to extrinsic factors such as osteophytes, adhesions to the underlying bone, or other factors which may cause a tightening effect. Thirty-five varus knees that were scheduled for TKA were investigated. Ligament status was evaluated after temporary correction of alig

Surgical management of congenital and habitual dislocation of the patella.

Twelve patients with congenital dislocation of the patella (CDP) and 23 patients with habitual dislocation of the patella (HDP) were followed for 2-15 years after surgical stabilization of the patella. The underlying pathology in both conditions was contracture of the quadriceps mechanism, which was more severe in CDP. Surgical stabilization in most cases included an extensive lateral release, medial plication, and transfer of the lateral half of the patella tendon. Lengthening of the rectus femoris tendon was required in many cases. With appropriate operative procedures, satisfactory results
