Anterolateral ligament injury has a synergic impact on the anterolateral rotatory laxity in acute anterior cruciate ligament-injured knees.

PURPOSE: To investigate the prevalence of the anterolateral ligament (ALL) injuries and its role in rotatory laxity in acute anterior cruciate ligament (ACL)-injured knees. METHODS: Two-hundred and ninety-six consecutive patients with acute ACL injuries were evaluated retrospectively, excluding those with other ligament injury and undetectable path of ALL in MRI. Patients were divided into two groups based on the degree of ACL injury in arthroscopy (complete versus partial group). Logistic regression and discriminant analysis were performed to assess the risk of pivot shift test. RESULTS: A

Changes in biomechanical knee injury risk factors across two collegiate soccer seasons using the 11+ prevention program.

The 11+ injury prevention program effectively reduces injuries in high school-aged female soccer player, but the mechanism of the 11+ is unknown, particularly whether it impacts biomechanical risk factors associated with knee injuries. The purpose of this study was to report the changes in hip and knee biomechanics with use of the 11+ over two soccer seasons. Two collegiate women's soccer teams performed the 11+ for two soccer seasons. A control team was followed for one season. Athletes performed motion analysis of a drop vertical jump during preseason and postseason. Both groups had meaningf…

Knee function among elite handball and football players 1-6 years after anterior cruciate ligament injury.

The aim of the study was to describe objective and self-reported knee function for athletes who have returned to elite handball and football play after an ACL injury, comparing these to non-injured players at the same level. A total of 414 handball and 444 football players completed baseline tests from 2007 through 2014, examining lower extremity strength, dynamic balance, knee laxity, and knee function (KOOS questionnaire). Measures were compared between injured and non-injured legs and between injured legs and legs of controls. Eighty (9.3%) of the 858 players reported a previous ACL injury,

Anterior cruciate ligament injury after more than 20 years. II. Concentric and eccentric knee muscle strength.

The long-term consequences on knee muscle strength some decades after rupture of the anterior cruciate ligament (ACL) are not established. The aims of our study were to examine peak torque more than 20 years after ACL injury and to compare their knee muscle strength to that of healthy controls. We tested 70 individuals with unilateral ACL injury 23 ± 2 years after injury, whereof 33 (21 men) were treated with physiotherapy in combination with ACL reconstruction (ACLR ) and 37 (23 men) with physiotherapy alone (ACLPT ). These were compared with 33 age- and gender-matched controls (21 men). A Ki

Perioperative nerve injury after total knee arthroplasty: regional anesthesia risk during a 20-year cohort study.

BACKGROUND: Perioperative nerve injury (PNI) is one of the most debilitating complications after total knee arthroplasty (TKA). Although regional anesthesia (RA) techniques reduce pain and improve functional outcomes after TKA, they may also contribute to PNI. The objective of this study was to test the hypothesis that PNI risk differs among patients according to RA use during TKA. METHODS: All patients aged at least 18 yr who underwent elective TKA from January 1988 to July 2007 were retrospectively identified. The primary outcome variable was the presence of a new PNI documented within 3 mo

Physical therapy activities in stroke, knee arthroplasty, and traumatic brain injury rehabilitation: their variation, similarities, and association with functional outcomes.

BACKGROUND: The mix of physical therapy services is thought to be different with different impairment groups. However, it is not clear how much variation there is across impairment groups. Furthermore, the extent to which the same physical therapy activities are associated with functional outcomes across different types of patients is unknown. OBJECTIVE: The purposes of this study were: (1) to examine similarities and differences in the mix of physical therapy activities used in rehabilitation among patients from different impairment groups and (2) to examine whether the same physical therapy

Restoration of stance phase knee flexion during walking after spinal cord injury using a variable impedance orthosis.

A hybrid neuroprosthesis (HNP) combines lower extremity bracing with functional neuromuscular stimulation (FNS) to restore walking function and enhance the efficiency of ambulation. This report details the development of a novel HNP containing a variable impedance knee mechanism (VIKM) capable of supporting the knee against collapse while allowing controlled stance phase knee flexion. The design of a closed loop, finite state controller for coordination of VIKM activity with FNS-driven gait is presented. The controller is verified in testing during able bodied gait. The improved functionality

Biomechanics laboratory-based prediction algorithm to identify female athletes with high knee loads that increase risk of ACL injury.

OBJECTIVE: Knee abduction moment (KAM) during landing predicts non-contact anterior cruciate ligament (ACL) injury risk with high sensitivity and specificity in female athletes. The purpose of this study was to employ sensitive laboratory (lab-based) tools to determine predictive mechanisms that underlie increased KAM during landing. METHODS: Female basketball and soccer players (N=744) from a single county public school district were recruited to participate in testing of anthropometrics, maturation, laxity/flexibility, strength and landing biomechanics. Linear regression was used to model K

Autologous chondrocyte implantation versus microfracture for knee cartilage injury: a prospective randomized trial, with 2-year follow-up.

The objective was to evaluate the functional performance over a 2-year period following autologous chondrocyte implantation (ACI) in an open knee procedure compared to microfracture. Objective functional outcome was studied as secondary analysis in a subgroup of patients, in a randomized clinical trial, with concealed allocation and independent evaluators. Sixty-seven patients with local cartilage defect, with a mean size of 2.4 cm(2) (SD 1.5) of the femoral condyle of the knee were included. Thirty-three patients underwent the microfracture and 34 the ACI procedure. An identical rehabilitatio

Medial knee injury: Part 2, load sharing between the posterior oblique ligament and superficial medial collateral ligament.

BACKGROUND: There is limited information regarding directly measured load responses of the posterior oblique and superficial medial collateral ligaments in isolated and multiple medial knee ligament injury states. HYPOTHESES: Tensile load responses from both the superficial medial collateral ligament and the posterior oblique ligament would be measurable and reproducible, and the native load-sharing relationships between these ligaments would be altered after sectioning of medial knee structures. STUDY DESIGN: Descriptive laboratory study. METHODS: Twenty-four nonpaired, fresh-frozen adult

Peroneal nerve injury in three patients with knee trauma: MR imaging and correlation with anatomic findings in volunteers and anatomic specimens.

The aim of this article is to report on three patients with injuries of the peroneal nerve along the posterolateral aspect of the knee. Injuries in this area are less common than the injuries occurring at the level of the fibular head. In this article we report on three patients with posterolateral knee trauma who had peroneal nerve dysfunction. To better understand the precise location of the nerve on MR images, we performed MR imaging in five volunteers, and studied the position of the nerve on anatomic dissection ( n=1) and anatomic slices ( n=1). The common peroneal nerve is easily depicte

Muscle transposition and skin grafting for salvage of below-knee amputation level after bilateral lower extremity thermal injury.

Thermal injury to the lower extremity sometimes necessitates amputation around the knee joint. Knee function is so critical to prosthetic rehabilitation that every attempt should be made to salvage the knee joint. This report presents an unusual case of bilateral lower extremity flame burn requiring amputations. While the distal two-thirds of the legs and both feet were totally necrotic, the thermal damage was limited to skin and subcutaneous tissue sparing muscle and bone in the proximal one-third of the legs and posterior thighs. The below-knee amputation level was salvaged by muscle transpo

Prevalence of cyclic changes in limb volume (volumotion) of male patients with knee injury and the effects of ischemia/reperfusion due to tourniquet.

During surgery of limbs tourniquet up to a maximum of 2 h is frequently applied which may cause ischemia/reperfusion injury (IRI). During this condition the presence of vasomotion may have consequences for the perfusion and nutritive state of the tissues. We used a noninvasive plethysmographic method to investigate periodic changes in limb circumference (volumotion) in healthy male patients (n = 24) undergoing surgery for knee injury. To facilitate surgery a tourniquet was applied to the thigh, which caused an IRI of the leg. Results are given as mean of all values +/- SEM. Immediately after t

Generalised joint hypermobility and excess knee hyperextension are associated with an increased risk for second ACL injury, but not primary ACL injury, in female football players: A 5-year follow-up.

PURPOSE: This study aimed to investigate the association between generalised joint hypermobility, knee hyperextension, knee laxity, and static standing alignment with the risk of anterior cruciate ligament (ACL) injury in a cohort of female football players with an ACL-reconstructed (ACLR) knee and in knee-healthy controls. METHODS: We prospectively followed 117 female football players with ACLR (age, mean ± standard deviation, 20 ± 2 years; average 19 ± 9 months after ACLR) and 119 knee-healthy players (age, 19 ± 3 years) for 5 years. At baseline, all players were assessed for generalised jo

High knee abduction moments are common risk factors for patellofemoral pain (PFP) and anterior cruciate ligament (ACL) injury in girls: is PFP itself a predictor for subsequent ACL injury?

BACKGROUND: Identifying risk factors for knee pain and anterior cruciate ligament (ACL) injury can be an important step in the injury prevention cycle. OBJECTIVE: We evaluated two unique prospective cohorts with similar populations and methodologies to compare the incidence rates and risk factors associated with patellofemoral pain (PFP) and ACL injury. METHODS: The 'PFP cohort' consisted of 240 middle and high school female athletes. They were evaluated by a physician and underwent anthropometric assessment, strength testing and three-dimensional landing biomechanical analyses prior to thei

Intraosseous Vancomycin for Primary Total Knee Arthroplasty Is Associated With a Lower Rate of Postoperative Acute Kidney Injury Than Intravenous Vancomycin.

BACKGROUND: Perioperative vancomycin administration can be difficult to time and can lead to adverse systemic effects. Intraosseous vancomycin (IOV) is becoming increasingly utilized for total knee arthroplasty (TKA) prophylaxis. The primary purpose was to compare the rate of acute kidney injury (AKI) following primary TKA that utilized IOV or intravenous vancomycin (IVV). METHODS: A single-institution retrospective review was conducted on 4,184 primary TKAs from July 2016 to November 2024 that received IOV (1,297 cases) or IVV (2,887 cases) as part of antibiotic prophylaxis. Effect of Aquatic Proprioception Training, Strengthening, and Balance Exercises on Knee Joint Stability and Functional Performance in Anterior Cruciate Ligament Injury.

Background Anterior cruciate ligament (ACL) injuries frequently induce symptomatic knee instability and functional decline among active individuals. This pathology extends beyond mechanical insufficiency, involving significant deficits in muscle force production, balance, and proprioception. Collectively, these neuromuscular disruptions impair joint stability and represent critical factors that can delay the recovery of full functional status. While conventional land-based rehabilitation remains the standard approach, aquatic rehabilitation has gained attention due to its ability to reduce joi
