No sign of weakness: a systematic review and meta-analysis of hip and calf muscle strength after anterior cruciate ligament injury.

OBJECTIVE: We aimed to determine hip and lower-leg muscle strength in people after ACL injury compared with an uninjured control group (between people) and the uninjured contralateral limb (between limbs). DESIGN: Systematic review with meta-analysis. DATA SOURCES: MEDLINE, EMBASE, CINAHL, Scopus, Cochrane CENTRAL and SportDiscus to 28 February 2023. ELIGIBILITY CRITERIA: Primary ACL injury with mean age 18-40 years at time of injury. Studies had to measure hip and/or lower-leg muscle strength quantitatively (eg, dynamometer) and report muscle strength for the ACL-injured limb compared with

Bilateral peroneal nerve paralysis in agricultural workers: three case reports.

The peroneal nerve can be injured due to direct trauma or non-traumatic causes, with the injury mostly occurring between the fibular head and neck and mostly unilateral. This injury mostly results from prolonged squatting, cross-legged sitting or sitting with one foot tucked under the other leg. In this report, we present three agricultural workers with a history of prolonged squatting who developed bilateral foot drop due to bilateral peroneal nerve injury, two of whom recovered almost completely after physical therapy, and one remained disabled. Agricultural workers should be advised to avoi

Lower extremity compartment syndrome. When to suspect acute or chronic pressure buildup.

Acute compartment syndrome is a surgical emergency. A high index of suspicion is needed in cases of severe contusion, especially if patient complaints seem to outweigh physical findings. Acute surgical fasciotomy is an effective treatment and should be carried out expeditiously if compartment pressures are elevated over 70 mm Hg in order to avoid complications such as chronic disability due to contractures or rhabdomyolysis with acute renal failure. Chronic exertional compartment syndrome most often occurs in the anterior or the lateral compartment. Patients have only exercise-related symptoms

[Pretibial epidermolysis bullosa. A rare form of epidermolysis bullosa simplex].

INTRODUCTION: Pretibial epidermolysis bullosa had been classified as a rare localized form of autosomal dominant dystrophic epidermolysis bullosa. OBSERVATION: We report a sporadic case of a patient suffering from bullous lesions induced by minor trauma on pretibial skin. The lesions healed with atrophic scars. No milia formation was observed. The mapping of dermoepidermal junction by LH 7:2 and GB3 monoclonal antibodies was normal. By electron microscopy, numerous perinuclear vacuoles were observed and the cleavage occurred within the basal keratinocytes. DISCUSSION: This patient had clinic

Management of lower limb vascular injuries.

Primary amputation in vascular trauma should be rare in the absence of prolonged ischemia, massive soft-tissue loss, and multiple fractures. Repair of the artery and vein, soft-tissue coverage, and rigid skeletal fixation is associated with an excellent limb salvage rate. Fasciotomy should be performed in cases with compartmental syndrome, and in complex injuries the use of temporary intravascular shunts is recommended for better overall patient management.

Traumatic arteriovenous fistula--a case report.

Traumatic arteriovenous fistula is rare in children. We describe one case resulting from a small glass-fragment stab wound in the left leg. There was an associated false aneurysm of the anterior tibial artery. The patient was cured after multiple ligation and excision of the aneurysm.

Posttraumatic Soft Tissue Coverage of the Lower Leg for the Orthopedic Surgeon.

Soft tissue reconstructive techniques are powerful tools for the orthopedic surgeon caring for lower extremity trauma. This article seeks to inform orthopedic surgeons about useful techniques for skin closure, secondary wound closure techniques, and rotational flaps of the lower leg. Split thickness skin grafting, piecrusting, and the use of negative pressure wound therapy for skin closure, as well as rotational gastrocnemius, soleus, and reverse sural artery flaps are discussed with emphasis on techniques for the nonvascular and nonmicrovascular orthopedic surgeon.

Popliteal fossa reconstruction with a medial sural artery perforator free flap using the medial sural vessel as the recipient.

BACKGROUND: Reconstruction of the popliteal fossa using a free flap is challenging. Therefore, to facilitate easier free flap reconstruction of the popliteal fossa, we use a medial sural artery perforator (MSAP) free flap with a medial sural vessel as the recipient vessel, as it provides several advantages based on many reports. METHODS: This report describes the authors' experience and outcomes with this technique. Between October 2010 and January 2015, 10 patients with medium-sized defects in their popliteal fossa underwent MSAP free flap reconstruction using the medial sural vessel as the

Effects of muscle injury severity on localized bioimpedance measurements.

Muscle injuries in the lower limb are common among professional football players. Classification is made according to severity and is diagnosed with radiological assessment as: grade I (minor strain or minor injury), grade II (partial rupture, moderate injury) and grade III (complete rupture, severe injury). Tetrapolar localized bioimpedance analysis (BIA) at 50 kHz made with a phase-sensitive analyzer was used to assess damage to the integrity of muscle structures and the fluid accumulation 24 h after injury in 21 injuries in the quadriceps, hamstring and calf, and was diagnosed with magnetic

[Substitution of soft tissue defects on the lower leg and foot in septic cases].

After the spontaneous epithelialisation of the defects of soft parts so called unstable scars remain which, because of ulceration, are sources of infection, endangering both soft parts and bones. Substitution with soft tissues of adequate blood supply enables the complete sanation of the septic foci and by this the rapid healing of the patient with full function. Beside this a better cosmetic result can be achieved. In septic environment, if possible, local muscle flaps and free flaps are used, depending on the extent and localization of the defect. Experiences, gained in these operations are

[Vascular reconstruction following trauma. Experience in Basel 1980-1988].

From 1980-1988 twenty patients with vascular trauma were treated at the Kantonsspital Basel, Switzerland. The incidence of 1% in open fractures is low. Nerve lesions in combination with vascular trauma are very frequent, specially at the upper extremity. Nearly all patients were treated with the interposition of a venous graft from the vena saphena magna. We deplore 3 amputations after successful vascular reconstruction, mainly due to extensive bone and soft tissue damage. Our long-term control shows very satisfying vascular results, but poor neurological results. We therefore conclude that th
