BACKGROUND: Palliative support has been shown to improve the quality of end-of-life care provided in long-term care facilities (LTCF). In this context, a practice improvement plan, which included creating a withholding and withdrawing life-sustaining treatment (WLST) protocol, has been developed and implemented in our LTCF. We aimed to compare professional practices before and after the implementation of this improvement plan.
METHODS: The study population included all individuals who died in 2018 and in 2021. A clinical audit noted if WLST was decided and which palliative interventions were

Familial caregivers are often directly involved in treatment of patients with chronic wounds, however, less is known about their personal impairment, and specific support is lacking for these important members of the therapeutic team regarding wound care. The aim of this study was to investigate the influence of wound care provided by family members on their quality of life, and to create a suitable questionnaire to describe the affected personal aspects. A five-part questionnaire, named ELWA, was created by the authors and answered by 30 familial caregivers of 30 respective patients with chro

BACKGROUND: Cancer and circulatory diseases are overrepresented among the annual deaths in Sweden. Specialized palliative care (SPC) is considered complex, especially by cancer patients who have undergone long-term treatment. Relatives caring for a person with cancer within the home are faced with many demanding tasks.
PURPOSE: To describe and map relatives' experiences of the end-of-life care given in specialized palliative home care from a Swedish national perspective.
METHODS: A register study was conducted with a mixed-method design using qualitative and quantitative descriptive data fro

INTRODUCTION: The symptom and treatment of pancreatic ductal adenocarcinoma (PDA) at an advanced stage significantly deteriorate patients' quality of life (QOL). Therefore, multidisciplinary long-term and palliative care is needed to maintain patients' physical, social, and psychological well-being and ultimately improve survival.
CASE PRESENTATION: An octogenarian residing in a care center, which can provide multidisciplinary long-term care with nutritional, physical, and social support, as well as pain management through home medical services, was diagnosed with Stage IV PDA. She received m

Guidance on psychosocial-spiritual care for people experiencing homelessness (PEH) is limited. Through a descriptive phenomenological analysis of interviews with interdisciplinary professionals, chart documentation, and input of a practicing homeless palliative care (HPC) social worker, this study provides exploratory guidance on administering psychosocial-spiritual care in a street outreach-based setting. Analysis produced four themes: (1) Building intimacy and consistency, which illustrates strategies for meaningfully meeting client need; (2) Assessment and treatment planning, in which provi

BACKGROUND: In low- and middle-income countries, the incidence of cancer is rising, with diagnoses frequently made at advanced stages, limited access to treatments, and high mortality rates. Palliative care may alleviate suffering, enhance quality of life, and reduce costs. In this review, we aimed to explore the development of palliative care in low- and middle-income settings and assess if and how this contributes to mitigating or alleviating the human suffering of people with cancer.
METHODS: A best-fit framework synthesis approach was employed, and seven databases were systematically sear

Perinatal palliative care (PnPC) is a relatively new branch of pediatric palliative care (PPC), which focuses on providing holistic care in the antenatal, delivery, and neonatal settings. In this paper, we address previously unexplored justice-based ethical questions related to the provision of PnPC. We examine why some families who receive the diagnosis of a potentially life-limiting condition in their baby before or after birth receive PnPC support whilst others do not. We describe current inequities in the access to, and delivery of, PnPC. Drawing on philosophical theory (the Capabilities A

Each year, millions of people around the world experience serious health-related suffering from life-threatening and life-limiting illnesses and require palliative care. Palliative care is a human right to adequate and timely interventions for the well-being of patients with serious illnesses and their families. The World Health Assembly incorporated palliative care as a component of universal health care, reinforcing the World Health Organization's mandate to support governments in integrating the service into primary health care. Yet, fewer than half of the countries across the globe have re

BACKGROUND: Mental illnesses can have fundamental negative effects upon people's quality of life. Whilst people with mental illness require mental health support, their physical health can often be neglected. As a result, they are often denied high-quality palliative care to which they are entitled. This study aimed to identify the facilitators of providing high-quality palliative care for patients with mental illness from the perspective of staff working in mental health services in Iran.
METHODS: A descriptive qualitative study was conducted. A total of 32 people participated, including nur

BACKGROUND: Caregivers of persons living with Alzheimer's disease (AD), dementia with Lewy bodies (DLB), and Parkinson's disease dementia (PDD) are faced with numerous challenges. However, little is known about the caregiving experience across different dementias.
OBJECTIVE: The aims of this cross-sectional study were to examine the differences in the caregiver experience between DLB, PDD, and AD.
METHODS: Respondents were caregivers (N = 515; 384 DLB, 69 AD, 62 PDD) who completed a 230-question survey including sociodemographics, disease severity, neuropsychiatric symptoms, and measures of

Background: Patients with motor neuron disease (MND) often do not experience the full survival and quality of life benefits of noninvasive ventilation (NIV). Successful delivery of NIV is challenging to multiple healthcare professionals involved in the respiratory care patient journey and considering their perspectives is crucial in order to understand how to deliver optimal care. Objective: To identify the factors that influence NIV delivery in MND from a healthcare professional perspective and understand how obstacles can be overcome to optimize care. Movement and bodily autonomy are integral to life and participation in meaningful activities. Current advance care planning (ACP) and advance directive processes do not consistently elicit discussions or documentation about these topics. The problem is especially serious when continued movement increases the risk of falling or other negative health events. This review article integrates available evidence and the authors' clinical experience to argue that ACP should include functional goals and that rehabilitation professionals should be regular members of end-of-life care teams. We address cl

OBJECTIVE: We conducted this study to investigate how physicians in a pediatric intensive care unit (ICU) currently make decisions to withdraw and withhold life support. Consultation with the patient's primary caregiver often precedes decisions about withdrawal and limitation of life support in chronically ill patients. In these scenarios, the patient's primary caregiver was the pediatric oncologist. To evaluate the influence of subspecialty training, we compared the attitudes of the pediatric intensivists and the oncologists using scenarios describing critically ill oncology patients.

OBJECTIVE: This article outlines the physical, psychological, social, and spiritual issues that people with neuromuscular disease may face as the disease progresses and how neuropalliative care can help in the assessment and management of these issues.
LATEST DEVELOPMENTS: Understanding of the genetic influences on progressive neuromuscular disease is increasing, and new treatments are being developed that may offer the possibility of slowing or even halting disease, although the treatments may be very expensive. The use of technology will allow some areas of care, particularly communication,

As the number of prison inmates facing end-stage chronic illness grows, more prisons across the U.S. must address the need for end-of-life care. Many will likely need to develop a plan with potentially limited resources and external support. This case study presents one long-running model of care, the Louisiana State Penitentiary Prison Hospice Program. Based on field observations and in-depth interviews with hospice staff, inmate volunteers and corrections officers, we identify five essential elements that have contributed to the long-term operation of this program: patient-centered care, an

Palliative and end-of-life care aim to minimize suffering and optimize the quality of life for individuals nearing the end of their lives. Osteopathic manipulative medicine (OMM) represents a potentially valuable nonpharmacological approach within this integrative care framework. Emerging evidence from clinical observations, small-scale studies, and foundational osteopathic principles suggests that OMM may help support patient comfort, alleviate certain symptoms, and contribute to dignity in care. Techniques such as myofascial release, cranial manipulation, and lymphatic drainage have been rep
