Mucopolysaccharidoses.

The MPSs are a heterogeneous group of disorders caused by the deficiency of one of ten lysosomal enzymes and the resultant accumulation of glycosaminoglycans in tissues and organs. The phenotypic variations of each disorder are continuing to be expanded, while the biochemical explanation of these variations needs to be defined. Mucopolysaccharidoses should not be diagnosed solely on clinical grounds, since laboratory confirmation by specific enzyme assay in now available. Prenatal diagnosis is possible for MPSs by amniocentesis. Chorionic villus sampling offers the possibility of first trimest

Resuscitation algorithm for management of acute emergencies.

Assuming that unrecognized or inadequately corrected hypovolemia results in higher mortality and morbidity rates, we developed a systematic approach to resuscitation that would: 1) identify criteria to aid in the recognition of hypovolemia and ensure the expeditious correction of this defect without interfering with diagnostic workup and management; 2) define criteria to prevent fluid overload which may jeopardize the patient's course, and 3) express these criteria in an explicit, systematic, patient care algorithm, ie, protocol, useful to both the resident and the practicing physician. We are

Health services research related to performance indicators and benchmarking in Europe.

OBJECTIVE: Measuring quality of care through performance indicators and subsequently using these to compare, learn, and improve (benchmarking) has become a central component of health care policy. This paper aims to identify the main themes of health services research in this area and focuses on opportunities for improving the evidence underpinning performance indicators. METHODS: A literature survey was carried out to identify research activities and main research themes in Europe in the years 2000-09. Identified literature was categorized into sub-topics and for each topic the main methodol

Factors influencing motivation and job satisfaction among supervisors of community health workers in marginalized communities in South Africa.

BACKGROUND: Management and supervision of community health workers are factors that are critical to the success of community health worker programmes. Yet few studies have explored the perspectives of supervisors in these programmes. This study explored factors influencing motivations of supervisors in community health worker programmes. METHODS: We conducted qualitative interviews with 26 programme staff providing supervision to community health workers in eight community-based organizations in marginalized communities in the greater Durban area of South Africa from July 2010 to September 20

An Observational, Cross-Sectional Study to Investigate Whether Room Air Ventilators, Used in the Community Setting, Are Colonised by Potential Airborne Pathogens (IPAP Study).

Background/Objectives: Long-term ventilation (LTV) is a widely used treatment for the management of patients with chronic respiratory failure. As use increases, it generates further questions about aspects of care. One issue is the potential risk of contamination within the device itself and the potential risk of respiratory tract infections in a subsequent user. Using an observational cross-sectional study design, the primary objective of this study was to identify whether airborne bacterial and fungal pathogens are present within a NIPPY 3+ (Breas Medical Ltd., Stratford Upon Avon, UK) room

The effectiveness of artificial intelligence health education accurately linking system on self-management in non-specific lower back pain patients.

BACKGROUND: Integrating artificial intelligence (AI) with mobile health is revolutionizing chronic disease management. Non-specific lower back pain (NSLBP), a leading worldwide disabling condition, negatively impairs patient quality of life and psychological status. Standard treatments, mostly pharmacological and physiotherapies, do not offer long-term support for self-management. Consequently, we developed the AI-Health Education Accurately Linking System (AI-HEALS) to investigate its application in improving self-management, alleviating pain, and enhancing overall life quality for NSLBP pati

Characterizing particle size in agricultural runoff to inform stormwater basin design.

Upper US Midwest sediment-basin design commonly relies on parent-soil texture classes and texture-based settling-velocity defaults (Natural Resources Conservation Service Conservation Practice Standard Code 350), yet conservation-managed fields can export fine particles that bypass gravity settling. Runoff from 13 edge-of-field watersheds in Wisconsin and Indiana showed systematic divergence between particle size distribution representations: volume-weighted (mass-based) D50 ≈ 30 µm (coarser classification) versus number-weighted (count-based) D50 ≈ 3.5 µm (clay-rich suspension). A mixed-effec

Dermal Exposure to Heavy Metals in Urban Green Space Soils: A Review of Bioavailability, Toxic Mechanisms, and Precision Risk Assessment.

Urban green spaces (UGSs) provide essential ecological services but also accumulate heavy metals (HMs) in their soils, posing a paradoxical health risk through dermal exposure. Traditional risk assessments, based solely on total HM concentrations, often overestimate threats by ignoring bioavailability (the fraction actually absorbed by organisms) and dynamic skin microenvironment factors. This review synthesizes recent advances to propose a precision assessment framework that integrates bioavailability. The framework advocates for the incorporation of bioaccessibility (the fraction of pollutan

Evaluating the utility of the Allplex STI Essential Assay to determine the occurrence of urogenital sexually transmitted infections among symptomatic and asymptomatic patients in Cape Town, South Africa.

BACKGROUND: Sexually transmitted infections are among the most commonly occurring infections globally, with countries in sub-Saharan Africa exhibiting disproportionately higher prevalence rates. Numerous reports indicate the need for accurate detection, epidemiological characterisation, and appropriate management of these infections. This prospective observational laboratory study sought to determine the occurrence of STI, using a validated molecular assay as a diagnostic and surveillance tool in our setting. METHODS: Urogenital swabs from symptomatic and asymptomatic patients, submitted to t

Arrhythmias in Patients With Valvular Heart Disease: Gaps in Knowledge and the Way Forward.

The prevalence of both organic valvular heart disease (VHD) and cardiac arrhythmias is high in the general population, and their coexistence is common. Both VHD and arrhythmias in the elderly lead to an elevated risk of hospitalization and use of health services. However, the relationships of the two conditions is not fully understood and our understanding of their coexistence in terms of contemporary management and prognosis is still limited. VHD-induced left ventricular dysfunction/hypertrophy and left atrial dilation lead to both atrial and ventricular arrhythmias. On the other hand, arrhyt

What happens after the funding ends?: A qualitative sustainability investigation of emergency department-based peer support programs implemented as part of Indiana's opioid state targeted response initiative.

INTRODUCTION: In 2017, funding disseminated through the US Substance Abuse and Mental Health Services Administration's Opioid State Targeted Response (STR) program accelerated the expansion of peer recovery support services across several states to engage emergency department patients presenting with opioid use disorder. While there is some literature on the initial implementation of these programs, little is known about their sustainability after the STR funding's end. Identifying what happened to these programs is a key component of understanding their ultimate impact and can inform future a

Defining and assessing vulnerability within law enforcement and public health organisations: a scoping review.

BACKGROUND: Historically, police departments focused solely on criminal justice issues. Recently, there has been a dynamic shift in focus, with Law Enforcement professional groups assuming more responsibility for tackling mental health and distress-related issues (that may arise because of mental health related problems and learning disabilities) alongside Public Health departments. While Law Enforcement has become a 'last line of support' and an increasing partner in mental health support, there is partnership working between law enforcement, psychology, and health professions in training and
