Obliteration of the Epitympanum and Mastoid Results in Reduced Cholesteatoma Recidivism: Long-term Outcomes and Technique.

OBJECTIVE: To compare long-term rates of cholesteatoma recidivism between cases undergoing tympanomastoidectomy with obliteration and without obliteration. STUDY DESIGN: Retrospective case-control. SETTING: Tertiary referral center. PATIENTS: Adults and older children with pars flaccida cholesteatoma. INTERVENTION: Tympanomastoidectomy with or without obliteration of the epitympanum and mastoid. MAIN OUTCOME MEASURE: Recidivistic cholesteatoma (ie, residual and/or recurrent cholesteatoma) determined by otoscopy, revision surgery, or imaging. METHODS: Retrospective chart review of pars fl…

[Imaging and clinical significance of tubular cell in lateral facial recess].

Objective:To observe the position and morphology of tubular cell in lateral facial recess by CT scanning and surgical finding, and its clinical significance. Methods:Thirty patients(32 ears) with cholesteatoma and/or adhesive otitis media requiring radical mastoidectomy and tympanoplasty were included in this study. To observe the morphology of the tubular cell in the lateral facial recess cavity through temporal bone HRCT and surgery, and to analyze its relationship with the facial nerve. Results:Tubular cell were found in 59.4%(19/32) ears by temporal bone HRCT and through surgeon. The lengt…

Effect of the Fascia Graft Size on Healing Rate of Cavities in Patients Undergoing CWD Tympanomastoidectomy: A Clinical Prospective Study.

OBJECTIVES: To evaluate the effects of size of temporal fascia graft on healing of the mastoid cavity in patients undergoing canal wall down (CWD) procedures. MATERIALS AND METHODS: The study included 32 patients (ages from 10 to 69 years) who were performed CWD tympanomastoidectomy from 2016 to 2018. Patients were divided into 2 different groups randomly based on size of fascia used in the operations. Group 1 consisted of 19 patients with temporal muscle fascia large enough to extend up to antrum by passing through over the facial ridge. Group 2 consisted of 13 patients with fascia of a size…

Evaluation of an Infant Temporal-Bone Model as Training Tool.

OBJECTIVE: Evaluation of the face validity of a new artificial model of an infant temporal bone (TB) suitable for surgical training, includi … c details, drilling experience, and surgical landmarks were rated. The surgical procedures included mastoidectomy, posterior tympanotomy, cochleostomy, and insertion of a cochlear electrode. RESULTS: Overall ratings were similar (3.9) for both the infant and the adult TB models, with ranges of 3.47 to 4.47 (infant model) and 3.5 to 4.33 (adult model). Ratings …

Iatrogenic intracranial venous hypertension treated with intracranial venous stenting: illustrative case.

BACKGROUND: Venous sinus injury resulting in thrombosis is a possible complication of skull base surgery and neck dissection. Although usually asymptomatic, sinus thrombosis can obstruct the dominant cranial venous outflow pathways, leading to a cycle of increased intracranial pressure secondary to venous congestion, which further compresses the remaining sinuses in a positive feedback loop. This can present with symptoms resembling idiopathic intracranial hypertension. OBSERVATIONS: A patient underwent a left mastoidectomy for chronic mastoiditis, complicated by injury of the ipsilateral dom…

It mimics various malignant, infectious, and i … esion in the right middle ear cavity extending to the mastoid antrum. He underwent a right cortical mastoidectomy with decompression of the facial nerve. Histopathology and immunohistochemistry of granulation tissues from the middle ear and the mastoid revealed evidence suggestive of probable IgG4 disease. Veria Operation. II. Surgical results from 101 cases.

The 'Veria operation' is a new technique for cochlear implantation. It is a non-mastoidectomy technique and uses the endaural approach for the cochleostomy and a direct tunnel drilled through the supero-posterior bony canal wall for the electrode. Two special instruments have been developed for this technique: a special perforator for the drilling of the direct tunnel and a safety electrode forceps for the insertion of the electrode. The method has been used in 101 cases with an age range from 2.5 to 75 years. 78 of them were primary operations and 23 revision cases. From the revisions, 18 wer…

Impact of surgical approach on taste dysfunction and quality of life in middle ear surgery: a systematic review and meta-analysis.

PURPOSE: This systematic review and meta-analysis evaluated how middle ear surgical approach and chorda tympani nerve (CTN) handling influence postoperative taste disturbance and related quality of life. METHODS: Following PRISMA 2020 guidance, databases and grey literature were searched (2000-2025) for randomized and observational studies reporting taste outcomes after tympanoplasty, stapes surgery, cholesteatoma/mastoidectomy, or cochlear implantation. Risk of bias was assessed with RoB 2.0 and Newcastle-Ottawa scales, and certainty with GRADE. CTN manipulation was standardised using a four…
