A Case Report on Surgical-Orthodontic Correction of Skeletal Class III Malocclusion with Severe Prognathic Mandible and Retrognathic Maxilla.

Mandibular prognathism combined with a retrognathic maxilla is a skeletal discrepancy that is difficult to correct. We report a case of a 25-year-old Saudi male patient with skeletal class-III malocclusion due to severe prognathic mandible who was referred to an orthodontist at Prince Sultan Military Medical City. Complete clinical examination, radiographic assessment, and study models revealed class-III malocclusion due to anteroposterior deficiency of the maxilla and severe prognathic mandible. Orthognathic surgery was performed 18 months after the presurgical orthodontic phase. A 10-mm LeFo…

Evaluation of the correlation between facial index and the cortical bone thickness of the maxilla and mandible-A computer tomography based study.

OBJECTIVES: cortical bone thickness confines the position of teeth and thus can be a limiting factor for orthodontic tooth movement. The study is aimed to correlate the cortical bone thickness of the maxilla and mandible with that of different face types, viz; dolichofacial, brachyfacial, and mesofacial. METHODS: Forty CT scans were analysed for the face types and were grouped into dolichofacial (13), brachyfacial (13) and mesofacial (14) face types. The scans were measured for the right and left side buccal and lingual cortical bone thickness at the central incisor, first and second molar re…

Malignant peripheral nerve sheath tumor of the maxilla: Case report and review of the literature with emphasis on its poor prognosis.

Malignant peripheral nerve sheath tumor (MPNST) is a spindle cell sarcoma with poor prognosis. Although patients with neurofibromatosis type 1 (NF1) have a higher risk of MPNST, it can also occur in the sporadic setting and may rarely arise centrally within bone. In this study, we present an extremely rare case of intraosseous MPNST of the maxilla arising in a 38-year-old female with no history of NF1. Despite radical surgery and postoperative radiotherapy, the patient died due to multiple distant metastases 1 year after treatment. According to the results of the literature analysis performed …

Continuous tooth eruption adjacent to single-implant restorations in the anterior maxilla: aetiology, mechanism and outcomes - A review of the literature.

INTRODUCTION: Dental trauma and congenital anodontia are common causes of anodontia in the anterior maxilla. The proposed restorative treatment constitutes a challenge for many dentists, particularly if it is a question of a young patient who has not yet completed skeletal and dental development. Current treatments for anterior maxillary anodontia include: fixed or removable partial dentures; orthodontic closure of interdental spaces; and dental implants. Dental implants do not move with the dento-alveolar complex during the growth period of the maxilla. Therefore, many researchers maintain th…

Aneurysmal bone cyst of maxilla existing concurrently with a giant cell granuloma of mandible: a case report of an unusual presentation.

Aneurysmal bone cysts (ABCs) are relatively rare but well-described expansile, osteolytic psuedocysts are most commonly seen in the long bones, pelvis, and vertebra. Although a well-described lesion, much confusion exists in its pathogenesis especially of the associations of this lesion with other lesions. Approximately, one-third of patients with ABC present simultaneously with another bone pathology. The aim of this paper is to describe a rare case of simultaneously occurring ABC of the maxilla and mandible with the mandibular lesion occurring predominantly as a giant cell granuloma. The pap…

Computerized tomographic evaluation of symphyseal donor sites used in the reconstruction of the posterior maxilla: a case report of 2 patients.

Autogenous bone grafts are commonly used for reconstruction of the deficient maxillary alveolar ridge. Mandibular ramus, symphysis, extraction sites, and retromolar sites are the most common sources for harvesting autogenous bone grafts from intraoral regions. Several authors have used computerized tomography (CT) to evaluate the dimensional change of bone graft, but there are limited reports evaluating donor sites for autogenous bone. In this report, 2 patients were treated with autogenous bone graft from the mandibular symphysis. Postoperative CT scans were used to evaluate the radiographic …

[Nasal septal abscess and palatine process of the maxilla abscess complicating acute rhinosinusitis in a 12-year old boy].

We report a case of nasal septal abscess and palatine process of the maxilla abscess secondary to acute rhinosinusitis in an 12-year-old boy. Rare complication of acute sinusitis is the nasal septum abscess; even rarer is the abscess of the palatine process of the maxilla, which our patient presented. Nasal septum abscess is an reservoir of suppurative secretion between cartilage or bone of the septum and their periostium or perichondrium. Nasal septum abscess is most often bilateral, causing nasal cavities obstruction. Other symptoms are: nasal pain, fever, headache, nasal tenderness, bad gen…

Evaluation of the precision obtained with a fixed surgical template in the placement of implants for rehabilitation of the completely edentulous maxilla: a clinical report.

The fabrication of a complete maxillary implant-supported prosthesis in a patient with slight resorption of the alveolar ridge and a high lip line presents a professional challenge. The implants must be placed with high precision to achieve good esthetics, phonetics, and function. A fixed surgical template using microimplants has been developed for this purpose. The objective of this investigation was to compare a fixed surgical template (FST) to a conventional movable surgical template (MST) for the precise placement of implants in the slightly resorbed edentulous maxilla. Three patients (28 …

Interpositional bone grafting and Le Fort I osteotomy for reconstruction of the atrophic edentulous maxilla. A two-stage technique.

This study presents the results from ten consecutive patients who, because of insufficient bone volume for conventional implant placement in the maxilla, were treated with an interpositional bone graft and Le Fort I osteotomy. The endosteal implants were placed six months after the osteotomy. A total of 60 screw-shaped titanium implants (Brånemark) were placed, of which three failed to integrate during the six-month healing period. No further implants were lost during the follow-up period, ranging from 15 to 39 months after placement of the implants. All patients received fixed bridges and all…

Recurrent Odontogenic Keratocyst Mimicking Persistent Periapical Lesion in the Anterior Maxilla: A Case Report With 15-year follow up.

Odontogenic keratocysts (OKCs) are aggressive odontogenic cysts with a high recurrence rate, often presenting diagnostic challenges due to their ability to mimic common periapical pathologies. Although more prevalent in the mandible, maxillary OKCs, especially those resembling periapical lesions, are rare and prone to misdiagnosis. This report details the case of a patient who presented with persistent swelling and pus discharge from the anterior maxilla, initially misdiagnosed as a chronic apical abscess following root canal treatment. Despite multiple endodontic interventions, symptoms recur…
