Chemotherapeutic approaches to nematodes: current knowledge and outlook.

The situation in the chemotherapy of nematode infections. in both human and veterinary medicine, is currently satisfactory. There are different drugs available: the benzimidazoles, levamisole, pyrantel, and the different macrocyclic lactones (Tables 1. 2, 3, 4). In human medicine, mebendazole and albendazole are mainly used against the intestinal nematodes. while ivermectin and diethylcarbamazine are microfilaricidal drugs used in prophylaxis against onchocercosis and filarial infections. The situation in veterinary medicine is, however, governed by the great resistance problem in some animal …

Efficacies of albendazole sulfoxide and albendazole sulfone against In vitro-cultivated Echinococcus multilocularis metacestodes.

The metacestode stage of Echinococcus multilocularis is the causative agent of alveolar echinococcosis (AE), a parasitic disease affecting the liver, with occasional metastasis into other organs. Benzimidazole carbamate derivatives such as mebendazole and albendazole are currently used for chemotherapeutic treatment of AE. Albendazole is poorly resorbed and is metabolically converted to its main metabolite albendazole sulfoxide, which is believed to be the active component, and further to albendazole sulfone. Chemotherapy with albendazole has been shown to have a parasitostatic rather than a p…

Human intestinal capillariasis (Capillaria philippinensis) in Taiwan.

A human case of intestinal capillariasis was first recognized in the Philippines in the mid-1960s. The parasitosis is a life threatening dis … most simple and convenient method of diagnosing capillariasis is stool examination. Treatment with mebendazole or albendazole for 20-30 days is known to be effective. All 7 cases we observed were cured through adequate treatment with anthelmintics and general management. Guidelines for treatment of cystic and alveolar echinococcosis in humans. WHO Informal Working Group on Echinococcosis.

Summarized in this article are recent experiences in the treatment of human cystic echinococcosis (CE) and alveolar echinococcosis (AE) of the liver caused by the metacestode stages of Echinococcus granulosus and E. multilocularis, respectively. For CE, surgery remains the first choice for treatment with the potential to remove totally the parasite and completely cure the patient. However, chemotherapy with benzimidazole compounds (albendazole or mebendazole) and the recently developed PAIR procedure (puncture-aspiration-injection-re-aspiration) with concomitant chemotherapy offer further opti…

The treatment was carried out with different drugs: mebendazole in two cases, thiabendazole in one and "pyrvinium pamoate" in another. Our conclusion is that there exist in the Canavese the climatic, environmental and social-economical conditions which can favour Strongyloidiasis.

Predisposition of individuals and families in Mexico to heavy infection with Ascaris lumbricoides and Trichuris trichiura.

Stool specimens from 90% of the population of 2 marginal communities in Coatzacoalcos, Mexico, were examined by the Kato-Katz technique for the estimation of eggs per gram of faeces (epg). Average epg values for Ascaris lumbricoides and Trichuris trichiura were calculated for each family in the community, based on the arithmetic mean epg of family members for each parasite. 39 families were classified as either lightly or heavily infected with A. lumbricoides and infected members were treated with mebendazole. A. lumbricoides expelled following treatment were collected from the children 2-10 y…

Evaluation of the Efficacy of Anthelmintic Drugs Against Trichinella spiralis Larvae.

Background: Albendazole, mebendazole, and ivermectin are effective against adult Trichinella spiralis but show limited efficacy against encapsulated muscle stage larvae. This limitation highlights the need for improved experimental approaches to evaluate anthelmintic activity at this stage and to identify alternative therapeutic candidates. Methods: Seven antiparasitic drugs, albendazole (ABZ), miltefosine (MLT), ivermectin (IVM), tribendimidine (TBD), praziquantel (PZQ), artesunate (ART), and mefloquine (MEQ), were evaluated for in vitro activity against T. spiralis muscle larvae. Lipid profile of Trichinella papuae muscle-stage larvae.

Outbreaks of trichinellosis caused by Trichinella papuae have been reported in South-East Asia. Mebendazole and thiabendazole are the treatments of choice for trichinellosis; however, both drugs result in significant side effects and are less effective for muscle-stage larvae (L1). An alternative therapeutic agent is needed to improve treatment. Information on lipid composition and metabolic pathways may bridge gaps in our knowledge and lead to new antiparasitics. The T. papuae L1 lipidome was analysed using a mass spectrometry-based approach, and 403 lipid components were identified. Eight li…

Prevalence of intestinal parasitic infections among school children in capital areas of the Democratic Republic of São Tomé and Príncipe, West Africa.

BACKGROUND: Although the Democratic Republic of Sao Tome and Principe (DRSTP) has undertaken school children-based deworming programs against intestinal parasitic infections (IPIs) using a single dose of mebendazole annually since 2005, it remains unclear as to the outcome to date. The present study intends to investigate the recent IPIs status among school children living in capital areas of the DRSTP. METHODS: A total of 252 school children (121 boys and 131 girls) of grades 4 and 5 from 4 primary schools located in the capital areas participated in the present study and their fresh fecal s…

An insect-tapeworm model as a proxy for anthelminthic effects in the mammalian host.

Invertebrate models provide several important advantages over their vertebrate counterparts including fewer legislative stipulations and faster, more cost-effective experimental procedures. Furthermore, various similarities between insect and mammalian systems have been highlighted. To obtain maximum use of invertebrate models in pharmacology, their fidelity as analogues of vertebrate systems requires verification. We utilised a flour beetle (Tenebrio molitor)-tapeworm (Hymenolepis diminuta) model to evaluate the efficacy of known anthelmintic compounds, praziquantel, mebendazole and levamisol…

A controlled evaluation of two school-based anthelminthic chemotherapy regimens on intensity of intestinal helminth infections.

BACKGROUND: School-based deworming programmes have been promoted as a cost-effective strategy for control of nematode infection in developing countries. While numerous efficacy studies have been conducted, there is little information on actual programme effectiveness in areas of intense transmission. METHODS: A randomized trial of a school-based deworming programme was conducted in 12 primary schools on Pemba Island, Zanzibar. Four schools each were randomized to control, twice a year deworming with single dose mebendazole or three times a year deworming. Baseline and 12-month follow-up data …

Dual targeting of EGFR signaling and microtubule dynamics enhances replication-associated cellular stress and suppresses tumor growth in gefitinib-resistant non-small cell lung cancer.

Acquired resistance to epidermal growth factor receptor (EGFR)-targeted therapy remains a major obstacle in the treatment of EGFR-mutant non-small cell lung cancer (NSCLC). This study investigated whether repurposing mebendazole (MBZ), a microtubule-disrupting agent, could restore sensitivity to gefitinib in gefitinib-resistant HCC827 GR5 cells. Gefitinib monotherapy showed minimal cytotoxicity, reducing cell viability only marginally (92.7 ± 1.5%), whereas MBZ significantly decreased viability to 79.7 ± 0.6% at 0.1 µM and 29.7 ± 0.6% at 0.5 µM (p < 0.001). Combination treatment (gefitinib 1 µ…

Preclinical toxicity and pharmacokinetics of a new orally bioavailable flubendazole formulation and the impact for clinical trials and risk/benefit to patients.

BACKGROUND: Flubendazole, originally developed to treat infections with intestinal nematodes, has been shown to be efficacious in animal models of filarial infections. For treatment of filarial nematodes, systemic exposure is needed. For this purpose, an orally bioavailable amorphous solid dispersion (ASD) formulation of flubendazole was developed. As this formulation results in improved systemic absorption, the pharmacokinetic and toxicological profile of the flubendazole ASD formulation have been assessed to ensure human safety before clinical trials could be initiated.
