Menatetrenone ameliorates osteopenia in disuse-affected limbs of vitamin D- and K-deficient stroke patients.

Significant reduction in bone mineral density (BMD) occurs in stroke patients on the hemiplegic and contralateral sides, correlating with the degree of paralysis and vitamin D and K deficiency due to malnutrition, and increasing the risk of hip fracture. We evaluated the efficacy of vitamin K2 (menatetrenone: menaquinone-4; MK-4) in maintaining BMD by comparing serum biochemical indices of bone metabolism between treated and untreated patients. In a random and prospective study, of 108 hemiplegic patients following stroke, 54 received 45 mg menatetrenone daily (MK-4 group, n = 54) for 12 month

Recognition of sickle cell anemia in skeletal remains of children.

The present study discusses in detail the osteological changes associated with sickle cell anemia in children and their importance in differential diagnosis. Posterior calcaneal and specific articular surface disruptive metacarpal lesions are diagnostic for sickle cell anemia. Calvarial thickening, tibial and femoral cortical bone thickening, and bowing are of more limited utility in differential diagnosis. Granular osteoporosis, pelvic demineralization and rib broadening are nonspecific. Localized calvarial "ballooning," previously not described, may have diagnostic significance. Bone marrow

Early mobilization of fractures of the metacarpals and phalanges.

Early mobilization of fractures of the metacarpals and phalanges is advocated. It should begin as soon as the stability of the fracture and the condition of the soft tissues permits. It has to be adapted to each case, and often consists of mobilization of the distal phalanges with the wrist and metacarpophalangeal articulations immobilized in a protective position. It is effective in preventing stiffness. Because it is a specific technique, it must be entrusted to specially trained physiotherapists, and requires regular and repeated clinical and radiologic follow up by the surgeon.

Cardiac, skeletal and ophthalmologic abnormalities in relatives of patients with the Marfan syndrome.

Nine patients with the Marfan syndrome and 40 of their first degree relatives were evaluated for the presence of cardiac, skeletal and ophthalmologic abnormalities. Aortic root dilatation and mitral valve prolapse were sought by echocardiography, and the metacarpal index was calculated from hand X-rays. Abnormalities of all the tests performed were present in all nine index cases, except for one normal eye exam. Mitral prolapse was present in thirteen relatives (33%) and aortic root dilatation in seven (18%). At least one cardiac abnormality was present in nineteen (47%) relatives. Aortic root

Open reduction and internal fixation for intraarticular fracture of metacarpal head.

Intraarticular fracture of the metacarpophalangeal (MP) joint presents complex problems related to the sophisticated functional aspects of the hand. Injury to the metacarpal head may have a severe effect on hand function but few studies have investigated the management of this condition. In this study, we applied open reduction and internal fixation for the displaced fracture of the metacarpal head and report the clinical and radiographic outcomes of our experience. Thirteen patients (12 men, 1 woman; mean age 21 years) were included in this study, and medical records and radiographs were revi

Intraoperative Bier's block as supplement to insufficient axillary block anesthesia in upper extremity surgery.

The authors reviewed 52 patients who underwent Bier's block, as supplementary anesthesia for insufficient axillary block in upper extremity surgical procedures. Prior to proceeding to the Bier's block, the mean value of pain using the visual optical analogue scale (VAS) was 7.0. In 48 of the patients supplementation with the Bier's block was sufficient (mean VAS score of 1.0) and all patients were comfortable throughout the procedure. In the remaining four patients supplementation with narcotics and sedatives via the other i.v. line was required. Intraoperative Bier's block provides a safe and

Computed X-ray densitometry measurement of mBMD & MCI in normal Thai and osteoporotic patients.

To establish the reference values of age-related change of metacarpal bone mineral density (mBMD) and metacarpal index (MCI) in screening for osteoporosis, both postero-anterior (PA) hands and lateral thoraco-lumbar radiography were done on 1,182 normal volunteers aged 17-83. From PA hands radiographs, mBMD and MCI were measured by computed X-ray densitometry (CXD) (Bonalyzer, Teijin Ltd., Tokyo). Exclusion of the surgical menopause condition and the causes of affected bone loss, the results show that mean mBMD and MCI in various age groups were significantly different (p-value < 0.005 for bot

Primary care of hand and wrist athletic injuries.

Athletic injures to the hand and wrist can range from simple sprains to severe fractures or soft-tissue disruptions that can permanently threaten the normal function of the extremity. This article deals with some of the more commonly noted sports-related injures to the hand and wrist to help the team physician make the correct diagnosis and establish the most effective treatment plan, so that the athlete may achieve maximum results and ultimately return to full participation in their sport.

Low metacarpal index predicts hip fracture: a prospective population study of 3,561 subjects with 15 years of follow-up.

BACKGROUND: Metacarpal index (MCI), measured from hand radiographs as the ratio between combined cortical thickness and bone diameter, has been suggested for assessment of bone mass and risk of osteoporotic fracture. We studied MCI for its ability to predict hip fractures. METHODS: Hand radiographs were taken and MCI determined in 3,561 subjects from a representative population sample of 8,000 Finns who were 30 years of age or over in 1978-80. Record linkage to the National Hospital Discharge Register identified 117 subjects who had been hospitalized for primary treatment of hip fracture by t

[Orthopedic treatment of fifth metacarpal neck fractures: prospective study].

Many methods of treatment have been proposed for the metacarpal neck fractures of the little finger, from early mobilisation to open or closed surgical techniques. A prospective studies of 20 cases treated by a non surgical technique is presented. The reduction was performed by manipulation under local anesthesia according to the Jahss technique and a modified Thomine brace was applied for four weeks. All patients were reviewed for follow-up at 30, 60, and 180 days. The fracture displacement was initially 34 degrees. It was reduced by manipulation to 12 degrees. All fractures healed with an av

Sensory processing in Parkinson's and Huntington's disease: investigations with 3D H(2)(15)O-PET.

There is conjoining experimental and clinical evidence supporting a fundamental role of the basal ganglia as a sensory analyser engaged in central somatosensory control. This study was aimed at investigating the functional anatomy of sensory processing in two clinical conditions characterized by basal ganglia dysfunction, i.e. Parkinson's and Huntington's disease. Based on previously recorded data of somatosensory evoked potentials, we expected deficient sensory-evoked activation in cortical areas that receive modulatory somatosensory input via the basal ganglia. Eight Parkinson's disease pati

Evaluation of "neocollateral" ligament formation by magnetic resonance imaging after total excision of the proximal interphalangeal collateral ligaments.

The presence of palpable thickening and maintenance of stability within weeks of total excision of the proximal interphalangeal (PIP) joint collateral ligaments is an established, although somewhat mysterious, phenomenon. To further investigate this postoperative thickening, magnetic resonance imaging (MRI) scans were obtained on 10 joints in 9 patients who had undergone total ligament excision for posttraumatic stiff PIP joints 3 months to 14 years earlier. Although the ultrastructure of these newly formed structures is yet unknown, their palpable consistency, MRI contour, distribution, and s

The use of historical and anthropometric data as risk factors for screening of low mBMD & MCI.

To evaluate the risk factors which affect bone loss in screening for osteoporosis, interview of anamnestic data (age, marriage status, pregnancies, menopausal age, intake of calcium, vegetables, protein and coffee, excessive use of alcohol and smoking, sedentary habits, family history), medical data, surgical data, followed by measurement of anthropometric variables [weight, height, antero-posterior (AP) thickness at xiphoid level], blood examination (calcium, inorganic phosphorus, alkaline phosphatase), both postero-anterior (PA) hands and lateral thoraco-lumbar radiography were done in 1,182
