Editorial - The NHS urgent and emergency care crisis: how much worse could it get?

NHS urgent and emergency care is under intolerable strain. This strain is increasingly causing harm to patients. Timely and high-quality patient care is often not being delivered due to overcrowding driven by workforce and capacity constraints. This drives low staff morale perpetuating burn out and high absence levels which currently dominate. Whilst COVID19 has accentuated and arguably expedited the crisis; the spiral of decline in urgent and emergency care has been decade long and unless urgent action is taken, we may not yet have reached its nadir.

Group early intervention eye movement desensitization and reprocessing therapy as a video-conference psychotherapy with frontline/emergency workers in response to the COVID-19 pandemic in the treatment of post-traumatic stress disorder and moral injury-An RCT study.

OBJECTIVE: Frontline mental health, emergency, law enforcement, and social workers have faced unprecedented psychological distress in responding to the COVID-19 pandemic. The purpose of the RCT (Randomized Controls Trial) study was to investigate the effectiveness of a Group EMDR (Eye Movement Desensitization and Reprocessing) therapy (Group Traumatic Episode Protocol-GTEP) in the treatment of Post-Traumatic Stress Disorder (PTSD) and Moral Injury. The treatment focus is an early intervention, group trauma treatment, delivered remotely as video-conference psychotherapy (VCP). This early interv

Autonomy, agency and value: an intrinsic approach to the value of autonomy does not escape the expressivist objection - a reply to Long.

Long's argument that an intrinsic approach to the value of autonomy could allow an autonomy-based approach to assisted dying to avoid the expressivist objection fails because autonomy is not the only morally valuable goal, so moral values other than autonomy also require consideration in ethical decision-making about requests for assisted dying. Accepting requests for assisted dying because respecting patients' autonomy is considered more valuable than patients' lives still expresses value judgements about their lives. This can only be avoided by considering autonomy to be the only, or supreme

Understanding How Religiousness Shapes Perceptions of Compulsive Sexual Behavior.

In the present study, we test the model of moral incongruence by examining whether moral disapproval of pornography mediates the relationship between organizational religious activity and self-reported CSB and whether the frequency of viewing pornography moderates the relationship between moral disapproval and self-reported CSB in two samples: a general population sample and a sample of members of the Church of Jesus Christ of Latter-day Saints ("Mormons"). Analyses revealed that, among both samples, frequency of pornography viewing moderated the indirect effect of organizational religious act

Religious Beliefs and Refusal of Assisted Reproductive Technologies: A Qualitative Case Study.

This exploratory and descriptive qualitative case study examined how religious beliefs shaped decision-making and emotional experiences in a couple facing infertility who re-fused assisted reproductive technologies (ART), with partic-ular attention to moral distress concerning embryo status and the psychosocial implications of treatment refusal. Psy-chological follow-up in the context of reproductive counseling involved a heterosexual couple and a potential gestational carrier. Data were obtained from semi-structured interviews and projective graphic techniques within the House-Tree-Person fra

To dispense or not to dispense? Ethical case decision-making in pharmacy practice.

In daily practice, pharmacists are regularly confronted with moral problems in which deciding what to do is not always a straightforward decision. In this contribution we show how the use of a specific method for moral deliberation can (in retrospect or prospective) aid moral judgements. We use the case of dispensing isotretinoin to demonstrate one ethical reflection method, namely the Utrecht Method.

Seizures, Cultural Beliefs, and Ethical Tensions.

This case narrative explores the ethical challenges that arise when culturally rooted surrogate decision-making conflicts with standard biomedical care. A 20-year-old Somali man presented with suspected autoimmune encephalitis requiring urgent immunosuppressive treatment to prevent permanent brain injury. Lacking decisional capacity due to encephalopathy, his mother served as his surrogate.Guided by religious and cultural beliefs, his family attributed his condition to demonic possession and declined medical treatment in favor of a spiritual healing ritual. This decision generated profound mor

People conform to social norms when gambling with lives or money.

Many consider moral decisions to follow an internal "moral compass", resistant to social pressures. Here we examine how social influence shapes moral decisions under risk, and how it operates in different decision contexts. We employed an adapted Asian Disease Paradigm where participants chose between certain losses/gains and probabilistic losses/gains in a series of moral (lives) or financial (money) decisions. We assessed participants' own risk preferences before and after exposing them to social norms that are generally risk-averse or risk-seeking. Our results showed that participants robus

Repairing with a warm heart: How medical practitioners cultivate affective relationships with clients.

Healthcare providers are often advised to manage emotions to adhere to professional feeling rules and avoid deep affection for clients, but there are situations where this would not apply. We offer an atypical case of a cleft lip and palate (CLP) treatment program where affectionate medical provider-patient relationships are encouraged. Participant observation in a CLP department, in-depth interviews with the medical team, and public online content from the official WeChat account of the department showed that a meaningful bond was formed through a contextual concordance of moral consideration

Violent Video Game Exposure and Cyberbullying in Early Adolescents: A Latent Moderated Mediation Model.

Violent video game exposure (VVGE) is a significant predictor of adolescent cyberbullying perpetration. However, little is known about the mediating and moderated mechanisms between them. This study examined the mediating role of moral disengagement between VVGE and cyberbullying perpetration as well as the moderating role of callous-unemotional (CU) traits on those associations. A total of 2,523 Chinese adolescents (Mage = 13.22, SD = 1.60, 48.4 percent girls) participated this study. Structural equation modeling showed that VVGE was significantly related to cyberbullying perpetration and mor

Induced delivery of anencephalic fetuses: a response to James L. Walsh and Moira M. McQueen.

James Walsh and Moira McQueen accurately conclude that the early delivery of anencephalic fetuses is morally acceptable, but the reasoning they use to reach that conclusion is flawed. First, the principle of double effect does not require a weighing of good and evil, but rather seeks a sufficient reason for tolerating the physical evil indirectly intended. Second, the principle of double effect requires a clear distinction between physical and moral causality. Third, the Catholic moral tradition will not admit direct and intended killing of the innocent. Rather, early delivery of anencephalic

'Taking the green pill': An interpretative phenomenological analysis of the lived experiences of climate distress.

INTRODUCTION: Climate distress (CD) is an emerging psychological response to the climate crisis, encompassing anxiety, grief, shame, and helplessness. While empirical research has begun to explore its prevalence and emotional impacts, little is known about the lived experience of CD. This study qualitatively explored how CD is experienced, maintained, and managed. METHODS: Semi-structured interviews were conducted with 10 participants experiencing CD, recruited via social media. Data were analysed using interpretative phenomenological analysis (IPA), allowing for an in-depth exploration of pa

Professional Socialization and Competency-Based Role Development in Clinical Ethics.

This essay argues that the professional formation of clinical ethicists should be neither reduced to technical competency nor equated with possession of authoritative moral answers. In conditions of reasonable moral pluralism, the distinct professional identity of the clinical ethicist is better grounded in a disciplined practice: rigorous ethical analysis, transparent reason-giving, role clarity, respect for affected persons, attention to power and exclusion, and a willingness to revise judgments. I define professional moral humility as recognition of the fallibility and situated character of

Siblings caring for siblings with Intellectual Disabilities: Naming and negotiating emotional tensions.

Extended longevity among adults with Intellectual Disabilities (ID) and increasing rates of diagnosis of Autism Spectrum Disorders (ASD) mean that parents are unlikely to remain primary carers throughout the lifecourse of adults with ID and ASD. In the context of decreased funding for disability services and policy moves toward de-congregated living, non-disabled (ND) siblings of people with ID/ASD are increasingly likely to be drawn into support and care roles for their siblings. Drawing on literature on moral emotions and the ethics of care, and on narratives collected from 25 ND siblings in

Thoracoabdominal normothermic regional perfusion: Is it ethical?

Thoracoabdominal normothermic regional perfusion (TA-NRP), a new method of controlled donation after circulatory death, seems to provide more and better organs for patients on organ transplant waiting lists compared to standard controlled donation after circulatory death. Despite its benefits, the ethical permissibility of TA-NRP is currently a highly debated issue. The recent statement published by the American College of Physicians (ACP) highlights the reasons for these debates. Critics' main concern is that TA-NRP violates the Dead Donor Rule. This paper presents an ethical analysis of the

When bad things happen to good nursing departments: how to stay hopeful in tough times.

How does a crisis impact a hospital? And how should nursing executives manage the crisis so that productivity and morale remain high? The purpose of this article is to trace an organizational crisis from its inception to its resolution. Every crisis has predictable components that if recognized will enable an administrator to diminish interpersonal hostility and conflict. Key strategies for resolving a crisis include sending objective messages to staff concerning the magnitude of the crisis, enlisting staff support in resolving problems contributing to the crisis, and projecting an optimistic …

Heartbeats, Burdens, and Biofixtures.

This paper addresses a dichotomy in the attitudes of some clinicians and bioethicists regarding whether there is a moral difference between deactivating a cardiac pacemaker in a highly dependent patient at the end of life, as opposed to standard cases of withdrawal of treatment. Although many clinicians hold that there is a difference, some bioethicists maintain that the two sorts of cases are morally equivalent. The author explores one potential morally significant point of difference between pacemakers and certain other life-sustaining treatments: specifically, that the former are biofixture

The pregnant brain dead and the fetus: must we always try to wrest life from death?

This article deals with the ever more timely and often vexing topic of maintaining a brain-dead mother as an incubator for her developing offspring. It explores the issue by: (1) reviewing the history of the problem and the "state of the art" today, (2) examining the moral problem of using brain-dead persons as incubators for potential or actual others, (3) searching for moral differences between maternal death early or late in pregnancy, and (4) presenting a possible resolution in such tragic cases. It concludes that (1) a moral necessity to deliver viable infants from brain-dead mothers exis
