Orthostatic hypotension in a case with multiple sclerosis.

A 37-year-old woman with a 5-year history of multiple sclerosis is reported. She began having recurrent syncope even in the sitting position; other neurological features included hiccup, faciooro-lingual flushing and clumsiness of the hands. She had alternating Horner's syndrome, mild hypoalgesia of the right face, exaggerated deep tendon reflexes of the upper extremities, decreased deep sensation and ataxia of the upper extremities, and incomplete transverse myelopathy with a T4 sensory level. Head-up tilt testing confirmed orthostatic hypotension with relative preservation of the heart rate …

Multiple sclerosis with hydromyelia demonstrated at autopsy.

Multiple sclerosis (MS) associated with intramedullary cysts has been recently documented radiographically in approximately a dozen cases, but histological confirmation in these cases has been lacking. These cysts have been noted to occur centrally in the spinal cord and have been previously diagnosed as syringomyelia. We report a patient with extensive hydromyelia, but no dissecting syringomyelic cavity, of the thoraco-lumbar spinal cord, which was co-existent with centrally located, non-necrotic, chronic demyelinative plaques. A few well demarcated non-necrotic plaques were seen in the cereb…

Role of viruses in the etiopathogenesis of multiple sclerosis.

Epidemiological studies performed to identify the possible cause of Multiple Sclerosis (MS) suggest that an environmental agent could be involved in its etiopathogenesis. For a long time it has been hypothesized that this agent was a virus, but until now no virus specific to MS has been consistently identified. Animal models indicate that the demyelination of the central nervous system can be induced by certain families of viruses, but the implication of this in the etiopathogenesis of MS has not been clearly demonstrated. Morbilliviruses were the most studied. Research on this subject arose f…

[Pathogenetic role of prolactinemia in multiple sclerosis].

RIA and rosette formation were employed to measure the content of prolactin and lymphocyte populations in the blood of 26 men and 36 women suffering from multiple sclerosis (MS). Hyperprolactinemia was discovered to be attended by a decline of the T lymphocyte count, whereas hypoprolactinemia was marked by the impairment of the ratio of active to total "rosettes". In addition, 16 MS women suffering from persistent galactorrhea-amenorrhea and 40 parturients and lactating women with MS were examined with a special purpose in view. A relationship was demonstrated between exacerbation and developm…

[Multiple sclerosis with early manifestation in children].

From the starting point of cases reported in literature, five of the authors' own cases are reported in which multiple sclerosis had its origins not later than in the fourteenth year of the patient's life. There is no basic difference from adult cases in respect of neurological symptoms, course, and cerebrospinal-fluid condition, but unmistakably, well remitting episodic courses were in the foreground, multilocular failures preponderated, and cerebral symptoms predominated over spinal symptoms. As in adult cases, females were more commonly affected. There were inflammatory changes in the cereb…

Hemiballismus revealing multiple sclerosis in an infant.

The authors report a case of a 15-month-old infant who presented a left hemiballismus associated with left Marcus-Gunn pupil, and retrobulbar optic neuritis, which disappeared spontaneously in 7 days. Six weeks later, the infant experienced a mild paraparesis with a palsy of the left sixth cranial nerve, which disappeared following steroid therapy. Evoked visual potentials elicited abnormal latencies; cerebrospinal fluid (CSF) showed a rise in white cells and oligoclonal bands. According to the usual classification, this case corresponds to definite multiple sclerosis. The fact that hemiballis…

Trigeminal neuralgia in a patient with multiple sclerosis.

The incidence of trigeminal neuralgia in patients with multiple sclerosis is low; however, the association of the two can present a difficult diagnostic problem to the unsuspecting practitioner. Certain characteristics of trigeminal neuralgia in patients with undiagnosed multiple sclerosis can lead to a diagnosis of the disease.

[Dyslexia of digits and numbers in multiple sclerosis].

In a 32-year-old man with remitting-progressive multiple sclerosis a right-side hemiparesis and alexia of digits of numbers developed during an exacerbation. While reading aloud the patient changed digits, left them out or transposed. No changes were observed in reading of letters, words and no agraphia was present. Alexia persisted for about one month. The location of alexia in the occipital lobe and its association pathways with the left parietal lobe are discussed.

[A case of multiple sclerosis with pure alexia].

We report here a case of multiple sclerosis (MS) showing pure alexia. The patient was a 28 year-old, right-handed male student of medicine. He had been healthy prior to college, but when he tried to read the newspaper one morning in January of 1983, he suddenly became aware that he could not. He could speak fluently and had no disturbances of auditory comprehension. No signs of abnormality were noted in his writing ability. Thereafter, the patient occasionally experienced difficulty in reading, together with right homonymous hemianopsia. These symptoms, however, usually vanished following slee…

A case-control study of multiple sclerosis.

We conducted a study of 145 persons with multiple sclerosis who had been identified in a 1970 survey and 145 friend controls, to investigate whether the development of MS was associated with exposure to uncommon viruses or an older age at infection with 1 or more common viruses. The most striking finding was a strong positive association for history of infectious mononucleosis (IM), suggesting older age at exposure to Epstein-Barr virus, the most common etiologic agent of IM. We also found significant positive associations for number of different domiciles before adulthood and for visits outsi…

Parity as a factor in incontinence in multiple sclerosis.

Twelve consecutively selected patients with multiple sclerosis and incontinence had electrophysiologic studies performed of the pudendal and perineal innervations of the anal and urinary sphincter. Single-fiber electromyogram density measurements were obtained in the external anal sphincter. Fecal incontinence was found to be unexpectedly frequent. The results suggest that incontinence in patients with multiple sclerosis is often due to the interaction of several factors, including central lesions, lesions of the conus medullaris and, also, coincidental pelvic nerve lesions associated with chi…

Paradoxical effects of temperature in multiple sclerosis.

Six cases of multiple sclerosis are reported in which cold produced a temporary exacerbation of symptoms and signs of the disease. Also, in one case investigated in detail by psychophysical methods, heating produced a paradoxical deterioration in vision and simultaneous improvement in sensory and motor function. The effect of temperature in multiple sclerosis is discussed and a physiological explanation for the paradoxical response to heating is suggested.

[Intellectual and mood disorders in multiple sclerosis].

Reports of frequency and interpretation of intellectual and mood disorders differ in multiple sclerosis (MS). Forty-one patients with MS defined according to MacAlpine's criteria were evaluated by psychometric tests (WAIS) and neuropsychologic examinations (study of language, gnosic and praxic activities, dynamic gestural organization, memory and learning) together with, in 24 of them, the AMDP psychopathologic rating scale. Intellectual disorders were noted in 65 p. 100 of patients. Although more frequent in severe and chronic forms they were nevertheless of early onset since more than a half…

Multiple sclerosis in a nationwide series of twins.

A nation wide Finnish Twin Cohort was linked with sickness insurance and hospital discharge registers on the basis of the unique identification number assigned to each Finnish citizen. The study series consisted of 4,063 monozygotic (MZ) and 9,001 dizygotic (DZ) same-sexed twin pairs born before 1958 and alive in 1981. Altogether, 22 subjects representing 11 MZ pairs and 10 DZ pairs showed clinically definite multiple sclerosis (MS). In one MZ pair both members had the disease. The frequency of MS among DZ twins corresponded to the figure in the Finnish population, but the prevalence was highe…

Treatment of spasticity with tizanidine in multiple sclerosis.

Spasticity is a frequent and often disabling symptom in MS patients. Current drugs used as antispastic agents include Dantrolene Sodium, Baclofen and Diazepam. Tizanidine (5-chloro-4-(2imidazolin-2 yl amino)-2,1,3-benzothialdiazole) is a new antispasticity agent that has purported central action. A double blind placebo controlled trial was performed to study the efficacy of this drug in MS patients. Sixty-six patients entered an eight week therapeutic trial and fifty-nine completed the trial. Patients were assessed at 0, 2, 3 and 8 weeks of therapy for clinical effects. Electrophysiologic test

Prevalence of multiple sclerosis in British Columbia.

A province wide prevalence study on multiple sclerosis (MS) was conducted in British Columbia (B.C.). The prevalence date was July 1, 1982. The major portion of this study was a review of all the files of neurologists practicing in B.C. as this was judged to be the most accurate source for identifying MS patients. 239,412 neurologists' files were hand searched by one researcher using modified Schumacher criteria for classification. Other sources used during the study for identifying MS patients were the MS Clinic, general practitioners, ophthalmologists, urologists, specialized facilities such…

Presenting signs and symptoms in multiple sclerosis.

The disability outcome related to the presenting signs and symptoms was studied retrospectively in 193 patients with definite multiple sclerosis (MS). Patients whose first sign was motor weakness were found to be more severely disabled, at all stages throughout the course of the disease, than patients with other presenting signs or symptoms. Patients whose disease manifested initially with sensory disturbances or paraesthesiae proved to be less seriously disabled than other patients at all stages of the disease. A similar tendency was found in patients presenting with optic neuritis (ON) for t…
