The relationship of the angle of immobilisation of the knee to the force applied to the extensor mechanism when partially weight-bearing. A gait-analysis study in normal volunteers.

We describe the influence of the angle of immobilisation during partial weight-bearing on the forces across the extensor mechanism of the knee. Gait analysis was performed on eight healthy male subjects with the right knee in an orthotic brace locked at 0 degree, 10 degrees, 20 degrees and 30 degrees, with the brace unlocked and also without a brace. The ground reaction force, the angle of the knee and the net external flexion movement about the knee were measured and the extensor mechanism force was calculated. The results showed a direct non-linear relationship between the angle of knee flex

Musculoskeletal ultrasound--a state of the art review in rheumatology. Part 1: Current controversies and issues in the development of musculoskeletal ultrasound in rheumatology.

As we begin the 21st century, musculoskeletal ultrasound (MSUS) is routinely used by an increasing number of rheumatologists throughout Europe and there is a growing interest in the application of MSUS in rheumatological practice in the UK. MSUS allows high-resolution, real-time imaging of articular and periarticular structures and has the advantages of being non-radioactive, inexpensive, portable, highly acceptable to patients and repeatable. There are a number of critical issues that need to be addressed in order to develop the role of MSUS within rheumatology. These include issues of equipm

An introduction to electromyography: an invited review.

Electromyography is a complex diagnostic test that is useful in diagnosing many neuromuscular and musculoskeletal conditions. Physiatrists and neurologists become familiar with this test during their training, while other physicians have minimal or no exposure, outside of perhaps some basic physiology lectures. A review of the anatomy, physiology, and physics upon which electrodiagnostic testing is based and of the various techniques used during a study provides the necessary foundation for understanding and using study results. A discussion of the conditions for which electrodiagnostic testin

Mutations in extracellular matrix molecules.

Genetic studies of humans and mice continue to highlight the nonredundant mechanical role of components in complexes that anchor cells to extracellular matrices. At the same time, recent data provide exciting insights into nonredundant, critical roles of transcription factors in regulating differentiation and function of matrix-producing cells.

Orthopaedic pitfalls.

When presented with an orthopaedic problem, rational management cannot occur without a reasonable diagnosis. Such a diagnosis can only be made by listening to the patient's history, and by examining him with a clear knowledge of important and relevant anatomy. Such relevant anatomy has been outlined in order to make sense of the symptom complexes in: --arm pain --knee pain --back pain without sciatica --back pain with sciatica The behaviour of collagen, which takes three weeks to heal and at least three weeks more to strengthen, is the basis of rational treatment of ligamentous injuries. A case of the Klippel-Trenaunay-Parkes Weber syndrome.

An extreme case of Klippel-Trenaunay-Parkes Weber syndrome is presented. The patient had extensive cutaneous naevus involving the left side of the body and consisting of naevus flammeus, hemangioma cavernosum, and naevus verrucosus. The left extremities were longer and there were multiple arteriovenous connections between a and v subclavia and a and v radialis. Secondary symptoms were sciatica, varicosity and osteoporosis.

First-degree atrioventricular block in 14-year-old child due to Lyme disease.

Lyme disease is an infectious disease caused by bacteria of the Borrelia burgdorferi sensu lato (Bbsl) complex and is characterized by predominant lesions of the skin, cardiovascular system, nervous system and musculoskeletal system. We have described a clinical case of first-degree atrioventricular block in a 14-year-old boy caused by Lyme borreliosis. The disease started with the manifestations of cardiovascular system involvement. The patient and his parents did not recall observing a tick bite or manifestation of erythema migrans (EM). The boy was prescribed doxycycline. Three weeks after …

Motor-evoked pain on a background of widespread muscle strain: a novel model simulating everyday activities.

OBJECTIVES: Musculoskeletal pain is among the most prevalent types of pain. However, experimental models that induce acute musculoskeletal pain by everyday activities - on top of sustained daily muscular strain - in a controlled and moderate manner are still lacking. Such models are essential for investigating underlying mechanisms of musculoskeletal pain and evaluating treatment approaches. The present study proposes a novel experimental model to address this gap. METHODS: A total of 43 pain-free participants completed two motor (lifting objects) and two sensory (hot water immersion, noxious

Work-related musculoskeletal disorders amongst undergraduate nursing students at the University of Johannesburg.

BACKGROUND: Work-related musculoskeletal disorders (WRMSDs) present as pain or discomfort in the musculoskeletal system that individuals experience from work-related activities. Substantial research evidence exists on qualified nurses with WRMSDs, but there is a distinct lack of research regarding nursing students and their work environment in South Africa. AIM: The primary aim of this study was to establish a baseline prevalence of musculoskeletal disorders (MSDs) amongst undergraduate nursing students. The secondary aim was to identify the role of certain occupational and biopsychosocial fa…

Analysis of Required Investigations of Work-Related Musculoskeletal Disorders in Spain.

Musculoskeletal disorders (MSD) are the most common source of occupational health problems in Western countries. In Spain, musculoskeletal disorders (MSDs) can be reported either as accidents or occupational diseases. When reported as an occupational disease, a full diagnosis is performed, as the compensation system needs the approval of the social security authority and a mandatory investigation has to be performed. Although many methods are available for investigating the causes of occupational accidents, occupational diseases have not been analysed with the same depth, and there is a lack o

From bowel inflammation to the bone and joints: musculoskeletal examination in inflammatory bowel disease (IBD).

BACKGROUND: One of the most important complications in inflammatory Bowel Disease (IBD) are musculoskeletal manifestations that are reported … nts were examined by an expert rheumatologist and their musculoskeletal symptoms were assessed. The musculoskeletal system was evaluated by Modified Schober test, Thoracic expansion (TE), Occiput to wall distance (OWD), and Patrick's or FABER test. Peripheral joints were also examined in all four extremities. Then patients were referred for pelvic and lumbosacral x-ray. …
