Exploring the clinical significance of anti-acetylcholine receptor antibody titers, changes, and change rates in Myasthenia Gravis.

INTRODUCTION/AIMS: Myasthenia Gravis (MG) is a common neuromuscular junction disorder that is primarily mediated by anti-acetylcholine receptor antibodies (AChR-Ab). However, using AChR-Ab titers to predict MG severity and improvement remains controversial. This study aims to explore the relationship between AChR-Ab titers and AChR-Ab rate of change (RR-AChR-Ab, %) and MG scores. METHODS: We used a prospective study approach, and included 62 patients with generalized MG (GMG) who were positive for AChR-Ab. We measured AChR-Ab titers, MGFA-QMGS, and MG-ADL scores at baseline (before treatment)…

A Clinical Case of Thymoma Presenting With Pure Erythroid Aplasia.

Thymic tumors are a rare condition that affects both genders equally, typically presenting between the ages of 40 and 70. Although often asymptomatic, they can give rise to paraneoplastic syndromes, with myasthenia gravis being the most common. The presentation of pure red cell aplasia, however, remains an uncommon manifestation. Thymectomy is the primary treatment, but it may not always be sufficient for managing associated syndromes, and immunosuppressive therapy may be necessary. This case involves a 50-year-old, otherwise healthy man, who presented to the emergency department with progress…

Congenital Myasthenia Gravis Presenting as Refractory Seizures and Respiratory Failure: A Case Report.

Congenital myasthenia syndrome (CMS) is an inherited disorder that involves muscle weakness and fatigue. It can present at birth or late childhood and has variable presentations and severity. It consists of a heterogeneous group of disorders characterized by defective neuromuscular junction (NMJ) transmission. Muscle weakness is common in patients with CMS, but other clinical presentations depend on the genetic defect. CMS is associated with mutations of genes at NMJ, involving the acetylcholine receptors (AChR) subunits. Here, we present the case of a 14-month-old child who presented with ref…

Recent advances in the therapeutic insights of thiazole scaffolds as acetylcholinesterase inhibitors.

Suppression of the acetylcholinesterase (AChE) enzyme is a prevalent strategy for curing diverse mental disorders, including Alzheimer's disease (AD) and the chronic autoimmune disease Myasthenia gravis. Acetylcholinesterase inhibitors promote cholinergic transmission via blocking AChE, which is implicated in the degradation and deficiency of acetylcholine. Various studies proved that the lack of cholinergic neurons in the central nervous system is the substantial reason for the behavioral abnormalities and cognitive retogradation that distinguish mental diseases such as dementia and AD. Moreo…

Successful treatment of generalized myasthenia gravis with telitacicept: a Chinese case series and literature review.

BACKGROUND: Despite existing treatments of generalized myasthenia gravis (gMG), there remains a need for more effective therapies with fewer side effects. Telitacicept, targeting B lymphocyte stimulator (BLyS) and a proliferation-inducing ligand (APRIL), emerges as a potential novel therapy for gMG. CASE PRESENTATION: In our study, four patients with gMG to standard treatments underwent an 8-week course of telitacicept monotherapy. Post-treatment, all patients exhibited satisfactory improvements. The Myasthenia Gravis Foundation of America Quantitative Myasthenia Gravis (MGFA-QMG) scores, 15-…

Concentric Needle Jitter Measurements in Voluntarily Activated Deltoid Muscle in Healthy Control Subjects.

INTRODUCTION/AIMS: The deltoid muscle demonstrates sensitivity in detecting abnormal decrement on repetitive nerve stimulation in cases of myasthenia gravis and congenital myasthenic syndromes. However, single-fiber electromyography (concentric needle jitter) normative data is currently lacking, which this study aims to address. METHODS: Jitter, measured by the mean consecutive difference (MCD), was assessed in the voluntarily activated deltoid muscle of 32 healthy adults (16 men and 16 women) with no known or suspected neuromuscular disease. For each subject, the mean MCD of 20 pairs was cal…

A novel nanoprobe for multimodal instantaneous detection of tiopronin and tetracycline hydrochloride.

Tiopronin (TP) is a common reagent used in clinical therapy and can cause problems such as myasthenia gravis and gastrointestinal disturbance if not used in the correct dose. Tetracycline (TC) is a class of antibiotic drugs whose long-term accumulation in the body poses a serious threat. Currently, these two substances are mostly detected in a single mode, susceptible to environmental and operational factors. In addition, no probes have been found that can detect the two substances at the same time. Therefore, a novel probe for the multi-modal instantaneous detection of TP and TC was developed…
