Clinical evaluation of the NovaplexTM Dermatophyte qPCR assay for the detection of dermatophyte fungi and Candida albicans in skin and nail material.

Cutaneous fungal infections are usually caused by dermatophytes, but also yeasts and other non-dermatophyte molds. The microbiological diagnosis of these mycoses traditionally relies on direct microscopic examination (DME) and culture. However, these procedures hold important limitations. Recently, some marketed qPCR methods have emerged for the identification of both dermatophyte and non-dermatophyte fungi. Herein, we aim to perform a head-to-head evaluation of the clinical performance of the NovaplexTM Dermatophyte RUO qPCR assay (Seegene) in comparison to the EurobioPlex Dermatophytes CE-IV…

Characterization and phylogenetic analysis of a Cunninghamella bertholletiae isolate from a bottlenose dolphin (Tursiops truncatus).

BACKGROUND: Cunninghamella is a genus of the order Mucorales which includes saprophytic species, rarely causing mycoses. The most frequently reported in human mycoses is the thermophilic species Cunninghamella bertholletiae. However, this species does not appear to cause mucormycosis in animals, so there is scarce information about C. bertholletiae isolates from animals. AIMS: In this paper we describe the phenotypic and genotypic characterization, and the phylogenetic analysis, of an isolate of C. bertholletiae involved in a central nervous system mucormycosis in a dolphin. METHODS: The iso…

A narrative review of dermatologic protocols for primary care medical service trips in Latin America and the Caribbean.

BACKGROUND: Skin disorders are prevalent on primary care medical service trips in Latin America and the Caribbean and commonly include scabies, superficial mycoses, and pyoderma. There have been no previous attempts to describe protocols that international volunteer clinicians use in managing these patients. The purpose of this study was to collect North American clinical protocols used by sending organizations in their volunteer operations in Latin America and the Caribbean, summarize the most common pharmacologic and nonpharmacologic management strategies, and compare these to published inte…

Characterization of Metarhizium viride Mycosis in Veiled Chameleons (Chamaeleo calyptratus), Panther Chameleons (Furcifer pardalis), and Inland Bearded Dragons (Pogona vitticeps).

Metarhizium viride has been associated with fatal systemic mycoses in chameleons, but subsequent data on mycoses caused by this fungus in reptiles are lacking. The aim of this investigation was therefore to obtain information on the presence of M. viride in reptiles kept as pets in captivity and its association with clinical signs and pathological findings as well as improvement of diagnostic procedures. Beside 18S ribosomal DNA (rDNA) (small subunit [SSU]) and internal transcribed spacer region 1 (ITS-1), a fragment of the large subunit (LSU) of 28S rDNA, including domain 1 (D1) and D2, was s…

Screening of some essential oils against Trichosporon species.

White Piedra is a superficial mycoses characterized by nodules on the hair shaft, caused by the basidiomycetous yeast Trichosporon species. In this study 25 essential oils were extracted and screened against two Trichosporon species i.e. Trichosporon asahii and Trichosporon cutaneum. Both these fungi procured from MTCC Chandigarh were maintained on yeast malt agar plates and tubes at 25 degrees C. Two screening methods viz., agar well diffusion assay and minimum inhibitory concentration were adopted for the study. The results showed that the maximum anti-yeast activity against T. asahii and T.…

[Fungi and yeasts isolated in mycological studies in skin and nail infections in The Netherlands, 1992-1993].

OBJECTIVE: To describe fungi and yeasts isolated from skin and nail infections in the Netherlands. DESIGN: Retrospective. SETTING: Centraalbureau voor Schimmelcultures (CBS), Baarn, the Netherlands. METHOD: Results of mycological investigation of skin and nail samples in the period 1992-1993 were analysed. After a clinical diagnosis of mycosis, performed by dermatologists and general practitioners, material was sent to the CBS for mycological research. RESULTS: The clinical diagnosis of onychomycosis was rather accurate, especially if made performed by dermatologists. Mycoses of the skin w…

Epidemiological study of sporotrichosis and histoplasmosis in captive Latin American wild mammals, São Paulo, Brazil.

Sporotrichosis and histoplasmosis are deep mycosis with a high incidence in human beings in Brazil. In domestic animals histoplasmosis has been described only in dogs, but the occurrence of sporotrichosis among domestic animals in Brazil has been described in dogs, cats, mules and asses. There is also a case of this disease reported in a chimpanzee (Pan troglodites). The purpose of this research was to perform an epidomiological study of these mycoses using delayed hypersensitivity tests (histoplasmin and sporotrichin) in Latin American wild mammals. This research was assayed using 96 healthy …

Blastomycosis presenting as solitary nodule: a rare presentation.

Blastomycosis is a chronic granulomatous and suppurative mycosis, caused by Blastomyces dermatitidis, which in the great majority of cases presents as a primary pulmonary disease. Primary cutaneous blastomycosis is very rare. We present a 57-year-old female patient with a solitary, slowly progressive nodule over upper lip of 2½ months duration. Initially, differential diagnosis of cutaneous leishmaniasis, pyoderma and deep mycoses were entertained. Slit smear preparation was suspicious of deep mycotic infection which was subsequently confirmed by biopsy and culture.

Invasive fungal disease in university hospital: a PCR-based study of autopsy cases.

Invasive fungal disease (IFD) has high mortality rate, especially in the growing population of immunocompromised patients. In spite of introduction of novel diagnostic approaches, the intravital recognition of IFD is challenging. Autopsy studies remain a key tool for assessment of epidemiology of visceral mycoses. We aimed to determine species distribution and trends of IFD over the last 10 years in unselected autopsy series from a large university hospital. Forty-five cases of visceral mycoses, confirmed by histopathology and panfungal PCR, were found in 587 consecutive autopsies. Major under…

Adverse events associated with itraconazole in 189 patients on chronic therapy.

Itraconazole was administered at doses of 50-400 mg/day to 189 patients with a variety of systemic mycoses for a median of five months. Adverse reactions possibly due to itraconazole were seen in 74 patients (39%). Mild gastrointestinal reactions were most common; other reactions including hypertriglyceridaemia, hypokalaemia and liver enzyme elevations occurred less frequently. No fatal reactions have been noted and toxicity has rarely led to a discontinuation of therapy. Chronic therapy with itraconazole appears well tolerated by the majority of patients.

A case of mycetoma-like chromoblastomycosis in Qatar.

Chromoblastomycosis is one of the neglected tropical mycoses associated with chronic infection of the skin and subcutaneous tissues. We report a case of 49-year-old patient originally from India presented with a mycetoma-like lesion in his right foot which was slowly progressing over three-year period. The diagnosis of chromoblastomycosis was confirmed following surgical excision and identification of the unique histological pathology of muriform bodies. The patient was subsequently treated with a prolonged course of oral itraconazole with a favorable outcome. The clinical presentations, asses…

Airway and parenchymal manifestations of pulmonary aspergillosis.

Pulmonary aspergillosis encompasses a heterogeneous group of mycoses that result from either colonisation or pathogenic damage of lung tissue by Aspergillus fungi. These clinical entities range from relatively benign saprophytic hypersensitivity associated with fungal inhabitation to life threatening invasive disease. The diagnosis of pulmonary disorders related to Aspergillus is on the increase and it is more important than ever those both general and respiratory physicians have a good understanding of these disorders. This paper reviews the contemporary understanding of the clinical, radiogr…

How I treat cryptococcosis in organ transplant recipients.

Cryptococcosis is a significant opportunistic mycoses in organ transplant recipients. Topical developments in the field in the past few years have highlighted important issues and at the same time raised new questions regarding the management of this yeast. These include, for example, management of pretransplant cryptococcosis during transplant candidacy and timing of transplant in these instances; potential for donor transmission of cryptococcosis in light of recent fatal transmissions; and prevention and treatment of Cryptococcus-associated immune reconstitution syndrome. Discussed herein ar…

Dermatophytosis: the management of fungal infections.

Dermatophytosis is an infection of the hair, skin, or nails caused by a dermatophyte, which is most commonly of the Trichophyton genus and less commonly of the Microsporum or Epidermophyton genera. Tinea capitis, tinea pedis, and onychomycosis are common dermatologic diseases that may result from such an infection. The treatment of fungal infections caused by a dermatophyte has been successful when treated with oral or topical antifungal agents. Terbinafine, itraconazole, and fluconazole are oral antimycotics that are effective in the treatment of superficial mycoses, although, depending on th…

[Infectious complications of lymphedema].

Erysipelas and lymphangitis are frequent complications of lymphedemas (20 to 30%). The most important risk factor for erysipelas is lymphedema since this is a protein rich edema that contributes to the risk of infection. In case of lymphedema the treatment is the usual consensus treatment for erysipelas. A prophylactic treatment with penicillin is requested as soon as the first recurrence. This prophylactic treatment includes skin care, particularly treatment of injuries and intertrigos. Hyperplastic skin leads to maceration and then mycoses. Physiotherapy does not increase the risk for infect…

Successful Treatment of Mucormycosis and Aspergillus sp. Rhinosinusitis in an Immunocompromised Patient.

Fungal rhinosinusitis is a serious infection related to immunosuppressive conditions. It is caused by common opportunistic mycoses, such as Mucor, Aspergillus sp. and Candida sp. Treatment is difficult and most of the patients with an invasive form, die. We report a case of a 13 year old boy who developed Mucor and Aspergillus sp rhinosinusitis while receiving chemotherapy for acute lymphoblastic leukemia. Even though the patient was intensely immunossupressed, early diagnoses associated with surgical debridement and effective doses of amphotericin B allowed a favorable outcome and hospital di…
