PURPOSE: To review the neuro-ophthalmological and radiological findings of acute methyl alcohol intoxication.
METHOD: 8 acute methyl alcohol intoxication cases were evaluated.
RESULTS: All patients were male and their ages varied between 21 and 55. At the initial examination, 6 to 12 days after methanol intake, visual acuity ranged from no light perception to counting fingers at 2 meters with no color perception. Total necrosis of the penis is a very serious complication of circumcision with an electrical scalpel. Reconstruction of the penis was achieved by mobilization of the corpora cavernosa and detachment from the ischiopubic rami, which was then placed in a scrotal tunnel to avoid recurrent adhesion to ischiopubic rami and to ensure skin cover of the penis. After a follow-up of 6 months, this child urinated with a good stream and presented spontaneous erections.

Concanavalin A (Con A) can induce an immune-mediated hepatitis. Since direct evidence of immune mechanism for this hepatitis is lacking, we employed adoptive transfer to study the mechanism of Con A-induced hepatitis. Intravenous administration of Con A (20 mg/kg) to Balb/c mice was accompanied by elevations of serum alanine aminotransferase (ALT) levels and midzonal necrosis with lymphocyte infiltration in the liver. A segmental necrosis of the ascending colon sometimes affecting the terminal ileum was observed 13 times in 12 end-stage renal disease patients over a 5400 patient-years observation period. In all but three cases the patient was operated within 24 h of onset of the abdominal pain. Three patients had a bowel perforation; nine had a limited intestinal necrosis. Avascular necrosis of the carpal scaphoid is a common disorder after a trauma, i.e. a fracture or mild and repetitive injury. Sometime it can be associated with a systemic disease or chronic steroid intake. Rarely avascular necrosis is found in the absence of a known etiology and so termed "idiopathic". Skin necrosis is a well-recognized although rare complication of continuous heparin therapy. We report the case of a 66-year-old diabetic woman with end-stage renal failure who received intermittent intravenous heparin during hemodialysis. She developed severe necrotic cutaneous ulcers over both legs, with typical histological findings. A very unusual appearance of fat necrosis in the breast is described. The low density/hypo-echoic rim and high density/hyperechoic central part are shown on mammography and ultrasound, respectively. The mass had the appearance of a 'mycetoma' in a cavity in the lung. The radiological diagnosis of fat necrosis was confirmed by histopathology.

A case is reported of necrosis of the columella nasi due to prolonged intubation in a 21-year-old road traffic accident patient. We report a case of radiation necrosis occurring 47 years after resection and radiotherapy for a juvenile pilocystic cerebellar astrocytoma, the longest yet reported. The patient presented with progressive lower cranial nerve dysfunction, and eventually died from cardiopulmonary arrest secondary to aspiration. The presentation, diagnosis, pathological features, and management of radiation necrosis are discussed.

It has been proposed that nontraumatic ischemic necrosis of bone (INB) is a result of lipid associated intraosseous thrombosis. A histological study of 15 patients with INB confirmed the presence of intravascular lipid and thrombosis in the vessels of the femoral head. A similar analysis of 11 patients with primary osteoarthritis (OA) showed similar changes at lower levels. These changes were not observed in seven control femoral heads. The possibility that both INB and OA result from intraosseous thrombosis is discussed.

We report a patient with AIDS who developed rapidly progressive, acute outer retinal necrosis. Cytomegalovirus etiology was proposed as a possibility in view of the histopathologic identification of the virus in the fellow concomitantly affected eye. Treatment with ganciclovir resulted in disappearance of the posterior pole lesions and inactivation of the peripheral lesions. We suggest that ganciclovir be used as soon as this condition is suspected in order to optimize the outcome.

Secondary subarticular avascular necrosis was observed grossly in 11.7% of femoral heads removed surgically because of osteoarthritis (OA). In most cases the necrosis was superficial, however in 15% of the 82 cases of secondary necrosis the necrosis was deep and wedge shaped similar in appearance to the lesions seen in primary subarticular avascular necrosis. Most of the lesions were seen in the early stage of OA and it is suggested that the lesion may interfere with the subsequent reparative phenomenon.

The acute retinal necrosis (ARN) syndrome is an increasingly occurring entity characterized by the triad of acute confluent peripheral retinitis with papillitis and anterior-chamber uveitis.
