Molecular epidemiology of Coxiella burnetii from ruminants in Q fever outbreak, the Netherlands.

Q fever is a zoonosis caused by the bacterium Coxiella burnetii. One of the largest reported outbreaks of Q fever in humans occurred in the Netherlands starting in 2007; epidemiologic investigations identified small ruminants as the source. To determine the genetic background of C. burnetii in domestic ruminants responsible for the human Q fever outbreak, we genotyped 126 C. burnetii-positive samples from ruminants by using a 10-loci multilocus variable-number tandem-repeat analyses panel and compared them with internationally known genotypes. One unique genotype predominated in dairy goat her…

Scrotal cancer: incidence, survival and second primary tumours in the Netherlands since 1989.

BACKGROUND: Since the 1970s there have been few epidemiological studies of scrotal cancer. We report on the descriptive epidemiology of scrotal cancer in the Netherlands. METHODS: Data on all scrotal cancer patients were obtained from the Netherlands Cancer Registry (NCR) in the period 1989-2006 and age-standardised incidence rates were calculated also according to histology and stage. Relative survival was calculated and multiple primary tumours were studied. RESULTS: The overall incidence rate varied around 1.5 per 1,000,000 person-years, most frequently being squamous cell carcinoma (27%)…

[Rubella epidemic in the Netherlands, 2004/'05: awareness of congenital rubella syndrome required].

Rubella is a public health problem due to the teratogenic effects associated with primary rubella infection during pregnancy (congenital rubella syndrome). Following universal rubella vaccination of infants in the Netherlands, the incidence of rubella has declined dramatically. However, since September 2004, an outbreak has occurred among unvaccinated individuals, most of whom declined vaccination based on religious beliefs. In the period 1 September 2004-22 March 2005, 166 cases of rubella were reported, including 12 pregnant women. Monitoring for signs that the epidemic has spread to other p…

Molecular evidence for dissemination of unique Campylobacter jejuni clones in Curaçao, Netherlands Antilles.

Campylobacter jejuni isolates (n = 234) associated with gastroenteritis and the Guillain-Barré syndrome (GBS) in the island of Curaçao, Netherlands Antilles, and collected from March 1999 to March 2000 were investigated by a range of molecular typing techniques. Data obtained by pulsed-field gel electrophoresis (PFGE), amplified fragment length polymorphism (AFLP) analysis, multilocus sequence typing (MLST), automated ribotyping, and sequence analysis of the short variable region of the flagellin gene (flaA) were analyzed separately and in combination. Similar groupings were obtained by all me…

K-ras oncogene mutations in sporadic colorectal cancer in The Netherlands Cohort Study.

Activation of K-ras oncogene has been implicated in colorectal carcinogenesis, being mutated in 30-60% of the adenocarcinomas. In this study, 737 incident colorectal cancer (CRC) patients, originating from 120 852 men and women (55-69 years at baseline) participating in the Netherlands Cohort Study (NLCS), were studied in order to evaluate subgroups with respect to K-ras mutation status. Mutation analysis of the exon 1 fragment of the K-ras oncogene, spanning codons 8-29, was performed on archival colorectal adenocarcinoma samples of all patients using macrodissection, nested PCR and direct se…

Prevalence of self-reported respiratory disease symptoms among veterinarians in the Southern Netherlands.

In June 1991, 831 veterinarians registered in the Southern Netherlands were mailed a questionnaire to obtain details of work practice and health problems. One hundred fifty-two veterinarians were not eligible (retired or not working regularly), 497 practitioners returned a complete questionnaire (73% response rate). The purpose was to assess the prevalence of respiratory disease symptoms (RDS) and to compare the prevalence of RDS between occupational specialties and with a group of swine farmers. The professional specialty of each veterinarian was grouped as: small-animal practice (17%), large…

[Gallbladder extirpation in The Netherlands; frequency, surgical mortality and postoperative complications (1987-1989)].

In view of the fast-growing popularity of laparoscopic cholecystectomy, a retrospective analysis was made of 'traditional' cholecystectomy in the Netherlands in the period 1987-1989 (data provided by the Health Care Information Centre Foundation), as compared with the period 1979-1981 (data provided by the Medical Registration Foundation). As regards frequency, the number of cholecystectomies had decreased by 25.5% compared with the period 1979-1981. The number of cholecystectomies with exploration of the choledochus had decreased by 43.8%. The surgical mortality in the period 1987-1989 amount…

Concurrent psychiatric, somatic, and social vulnerabilities in patients at increased cardiovascular risk in the Netherlands.

OBJECTIVE: To evaluate clinical, psychiatric, demographic, and socioeconomic vulnerability in a Dutch primary care setting. METHODS: We included 34,392 patients aged 40-70 years who were registered for cardiovascular risk management in general practice before 2018. Latent class analysis was performed using 17 indicators across seven themes: somatic morbidity, adverse lifestyle, psychiatric, age, sex, ethnicity, and socioeconomic factors. We compared five-year risk of major adverse cardiovascular events using Fine-Gray competing risk models and assessed annual medical expenditures (2018-2022)

Feeding change: school meal support in the Netherlands and the perspectives of primary school professionals.

BACKGROUND: In 2023, the Dutch government launched a national School Meals Program for schools where at least 30% of children came from families with a low income. Reports on children arriving at school hungry led to initiation of the program, which allows schools considerable freedom in providing meal support. Despite little prior experience, many eligible schools adopted some form of support, marking a notable policy shift in the Netherlands. This study aims to explore Dutch primary school professionals’ perspectives on school meal support, focusing on the types implemented, considerations i…

Exploring cost trajectories of patients admitted to short-term residential care in the Netherlands.

OBJECTIVE: Short-term residential care (STRC) is a Dutch form of post-acute care intended to return older adults home to live independently, yet fewer than 55% of patients are discharged home. Because post-acute care costs are unevenly distributed, average cost trajectories may obscure clinically meaningful variation. This study examines variation in STRC cost trajectories and identifies patient characteristics associated with high-cost group membership. METHODS: We conducted a retrospective longitudinal observational study using national health claims data from Statistics Netherlands for pat…
