Decremental atriofascicular accessory pathway with bidirectional conduction: delineation of atrial and ventricular insertion by radiofrequency current application.

A 17-year-old girl with a corrected complex congenital heart disease and recurrent episodes of supraventricular tachycardia was referred for catheter ablation. Electrophysiologic studies revealed the presence of an accessory pathway (AP) with bidirectional conduction and decremental properties. We demonstrated a course parallel to the node-His AV conduction system. Transient abolition of the bidirectional conduction through the AP was obtained by radiofrequency application to the ventricular insertion located in the distal right bundle branch and to the atrial insertion, located in the mid-ant

Yaw direction neurons in the cat inferior olive.

1. Single units that responded to yaw rotation were recorded extracellularly in the caudal inferior olive (IO) of barbiturate-anesthetized cats. Of 276 neurons, 55 responded reliably to yaw, and extensive quantitative data were recorded from 25. 2. No yaw-sensitive IO neuron responded to somatosensory or auditory stimuli but two responded, though unreliably, to flash. 3. Yaw-sensitive IO cells fired at low (1-4 spikes/s), irregular rates during one direction of rotation. Though cells responded reliably during yaw, firing rates varied considerably from cycle to cycle. Rotation speed and acceler

[Prescription of hearing aids - examples for limits and misindication (author's transl)].

For example there are described forms of hearing disorders in which the prescription of a hearing aid is inadvisible, ineffective or seeming unexpected good. However, an explanation for this resulted only from making use of all possibilities of audiometric diagnostic. This is illustrated in patients with endocranial tumors, with diffuse neural hearing disorder, and with psychogenic or psychogenic superimposed hearing impairment. The term of "inner ear hearing disorder", therefore, even in the prescription of hearing aids should not comprise all those impairments of hearing in which the values

Cardiac ganglionitis associated with sudden unexpected death.

In a postmortem study of the hearts of two young women who died suddenly and unexpectedly, we found a remarkably similar and distinctive ganglionitis, predominantly in the region of the sinus node. Both women had ventricular fibrillation at the time of collapse. Vesicular neuritis and older neural degeneration were present in other regions of the heart. Except for focal fibromuscular dysplasia of the sinus node artery and atrioventricular node artery of one heart, there was no other significant anatomic abnormality in either heart. The functional significance of this cardiac ganglionitis is un

Selective recording of physiologically evoked neural activity in a mixed autonomic nerve using a minimally invasive array.

Real-time closed-loop control of neuromodulation devices requires long-term monitoring of neural activity in the peripheral nervous system. Although many signal extraction methods exist, few are both clinically viable and designed for extracting small signals from fragile peripheral visceral nerves. Here, we report that our minimally invasive recording and analysis technology extracts low to negative signal to noise ratio (SNR) neural activity from a visceral nerve with a high degree of specificity for fiber type and class. Complex activity was recorded from the rat pelvic nerve that was physi

A neural network-assisted 3D theoretical thermoelastic solution for laminated liquid crystal elastomer plate used in restoring cardiac mechanical function.

Some atrial contractile assist devices applied on the heart surface can be regarded as a laminated Liquid crystal elastomer (LCE) plate under steady temperature loads and a contact mechanical force. An exact solution for the deformation of the laminated LCE plate under combined thermal and mechanical loads is derived by solving the three-dimensional (3D) equilibrium equations including heat conduction and thermoelastic theory. The validity of mathematical formula and computer programming is proved by convergence and comparison examples with finite element method (FEM). In order to simplify the

The Vagus Nerve Somatosensory-evoked Potential in Neural Disorders: Systematic Review and Illustrative Vignettes.

Objective. To review the scientific publications reporting vagal nerve somatosensory-evoked potential (VSEP) findings from individuals with brain disorders, and present novel physiological explanations on the VSEP origin. Methods. We did a systematic review on the papers reporting VSEP findings from individuals with brain disorders and their controls. We evaluated papers published from 2003 to date indexed in PubMed, Web of Science, and Scielo databases. We extracted the following information: number of patients and controls, type of neural disorder, age, gender, stimulating/recording and grou

Spatiotemporal characteristics of neural activity in tibial nerves with carbon nanotube yarn electrodes.

BACKGROUND: Reliable interfacing with peripheral nervous system is essential to extract neural signals. Current implantable peripheral nerve electrodes cannot provide long-term reliable interfaces due to their mechanical mismatch with host nerves. Carbon nanotube (CNT) yarns possess excellent mechanical flexibility and electrical conductivity. It is of great necessity to investigate the selectivity of implantable CNT yarn electrodes. NEW METHOD: Neural interfaces were fabricated with CNT yarn electrodes insulated with Parylene-C. Acute recordings were carried out on tibial nerves of rats, and

Vagus Nerve Modulation Using Focused Pulsed Ultrasound: Potential Applications and Preliminary Observations in a Rat.

The use of focused ultrasonic waves to modulate neural structures has gained recent interest due to its potential in treating neurological disorders non-invasively. While several papers have focused on the use of ultrasound neuromodulation on peripheral nerves, none of these studies have been performed on the vagus nerve. We present preliminary observations on the effects of focused pulsed ultrasound (FPUS) on the conduction of the left cervical vagus nerve of a Long Evans rat. Ultrasound energy was applied at a frequency of 1.1 MHz, and at spatial-peak, temporal average intensities that range

Poland-Moebius syndrome: a case report.

BACKGROUND: The primary site of pathology in Moebius syndrome is still unknown, although several studies have variably localized the lesion in the extraocular muscles, cranial nerves, or central nervous system. CASE: A 24-year-old man with Poland-Moebius syndrome and acquired progressive bilateral paralytic lower eyelid ectropion is described. OBSERVATIONS: In this patient, magnetic resonance imaging studies revealed a barely detectable pontine hypoplasia and normal recti muscles. Nerve conduction studies of the facial nerves showed a severe demyelinating or dysmyelinating type of neuropathy

Relationships Between Nerve Ultrasound Findings and Clinical, Genetic, Laboratory and Electrophysiological Findings in Patients With Chronic Idiopathic Axonal Polyneuropathy.

INTRODUCTION/AIMS: Chronic idiopathic axonal polyneuropathy (CIAP) is an increasingly recognized, frequent and often disabling disease with unknown etiology. We explored the utility of nerve ultrasound in the investigation of patients with predominantly sensory CIAP, by comparing ultrasound with clinical, genetic, laboratory and electrophysiological findings. METHODS: Thirty-nine patients with a predominantly sensory CIAP were included. The averaged measurements of the ultrasound cross-sectional areas (CSAs) of median and ulnar nerves at mid-forearm and mid-humerus (Mean UL CSAs) for each pat

Clinical and neurophysiological features of neuropathic pain in hereditary transthyretin amyloidosis associated polyneuropathy.

BACKGROUND: Hereditary transthyretin amyloidosis with polyneuropathy (ATTRv-PN) is often associated with neuropathic pain (NP), involving developing mechanisms across different nerve fibres. This study aimed to explore the relationship between NP intensity and clinical/neurophysiological measures in symptomatic ATTR V30M (p.V50M)-PN patients. METHODS: We included 106 symptomatic patients (46 males; mean age 47.5 ± 13.2 years). NP severity was classified using three pain-related items from the Norfolk QOL-DN, generating three groups: no pain, mild pain, and moderate-to-severe pain. Clinical an

Meta-analysis on effects of lymphatic drainage techniques in the management of carpal tunnel syndrome.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common neuropathy caused by median nerve compression, leading to pain, numbness, and functional impairment. While surgical decompression remains the definitive treatment for severe cases, non-surgical approaches are often utilized for symptom management. Lymphatic drainage techniques, including manual lymphatic drainage (MLD) and Kinesio taping, have been proposed as potential therapies for CTS by reducing edema and nerve compression. However, their efficacy remains uncertain. This study aimed to evaluate the effects of lymphatic drainage technique

The Utility of the Tinel Sign in the Diagnosis of High Tarsal Tunnel Syndrome.

BACKGROUND: Tarsal tunnel syndrome (TTS) can be divided into distal and proximal components. The latter is referred to as high TTS (HTTS) in which the tibial nerve is compressed above the location of the laciniate ligament. Although diagnosis of TTS has been shown to be enhanced by elicitation of a positive Tinel sign, no research into the utility of this provocation test has been conducted in the diagnosis of HTTS. This study aims to investigate the usefulness of the Tinel sign in diagnosing HTTS. METHODS: Seventy patients with electrophysiologic confirmation of HTTS were evaluated for the p

Development of a near infrared region based non-invasive therapy device for diabetic peripheral neuropathy.

Diabetic Peripheral Neuropathy (DPN) is a nerve damage that is treated with painkillers and steroids which have the drawback of interference with other medications and the dangers of side effects. Novelty of the proposed work is to develop a Near Infrared Region (NIR) based non-invasive therapy device called 'DPNrelief-1.0V'developed with a 890 nm wavelength diodes. DPNrelief-1.0V delivers a total dosage of 6.174 J/cm2 with heat absorption by tissue of 61.74 Joules at 30 minutes. The device was tested by carrying out a pilot study with 8 patients where 4 were treatment group and control group.

Guillain-Barre syndrome of acute motor axonal neuropathy (AMAN) type associated with herpes zoster: a case report.

Guillain Barre syndrome (GBS) following Varicella zoster is a rare presentation and has only been reported in a few cases around the world. Of the reported cases, the type of GBS is not specified in the majority, and where specified is of the acute inflammatory demyelinating polyradiculoneuropathy (AIDP) type. We report a case of acute motor axonal neuropathy (AMAN) type GBS following herpes zoster in a 27-year-old male who presented with bilateral lower limb weakness and left sided lower motor neuron type facial nerve palsy a week after herpes zoster infection.

Heterogenous electrophysiological features in early stage of hereditary transthyretin amyloidosis neuropathy.

INTRODUCTION: Hereditary transthyretin-mediated amyloidosis (ATTRv, v for variant) is a progressive disease caused by mutations in the TTR gene, leading to sensory-motor, axonal and length-dependent neuropathy. However, some patients may show variable electrophysiological pattern. The aim of this study was to evaluate the electrophysiological features of TTR amyloid neuropathy at the time of the first nerve conduction study (NCS) to assess whether there were distinguishing features useful for early diagnosis. METHODS: We retrospectively revised the first electrophysiological findings of ATTRv
