Neuropathological and ultrastructural features of amebic encephalitis caused by Sappinia diploidea.

Here we present the neuropathological, ultrastructural, and radiological features of Sappinia diploidea, a newly recognized human pathogen. The patient was a 38-year-old man with visual disturbances, headache, and a seizure. Brain images showed a solitary mass in the posterior left temporal lobe. The mass was composed of necrotizing hemorrhagic inflammation that contained free-living amebae. Immunofluorescence microscopy showed that the organism was not a species of ameba previously known to cause encephalitis. Trophozoites had a highly distinctive double nucleus, and transmission electron mic

Subtype analysis of neuropathologically diagnosed patients in a Japanese geriatric hospital.

Dementia is a social problem in Japan, as it is in other countries. Recently, there has been an increase in the number of patients with Alzheimer-type dementia (ATD) in the population. We analyzed 239 cases of patients autopsied at Fukushimura Hospital over a 10-year period. Clinicopathologically, 66% of these cases (158 cases) presented with dementia symptoms. The predominant form of illness was ATD. We found dementia with Lewy bodies (DLB) to be as frequent as vascular dementia (VD). Although the numbers were small, limbic neurofibrillary tangle dementia (LNTD) and Pick's disease (PiD) follo

Neurodevelopmental disorders as a cause of seizures: neuropathologic, genetic, and mechanistic considerations.

This review will consider patterns of developmental neuropathologic abnormalities-malformations of cortical development (MCD)--encountered in infants (often with infantile spasms), children, and adults with intractable epilepsy. Treatment of epilepsy associated with some MCD, such as focal cortical dysplasia and tubers of tuberous sclerosis, may include cortical resection performed to remove the "dysplastic" region of cortex. In extreme situations (eg, hemimegalencephaly), hemispherectomy may be carried out on selected patients. Neuropathologic (including immunohistochemical) findings within t

[Neuropathologic investigation of area tempestas sclerosis in epileptic sensitive rats].

AIM AND METHODS: The relation between AT sclerosis (loss of neurons and proliferation of astrocytes) and long-lasting epileptic susceptibility was investigated by thionine staining, GFAP immunohistochemistry and observing the behavior of rats, after scorpion venom (SV) or normal saline (NS) administrated for three week. RESULTS: Compared with NS+ NS group, both the loss of neurons and proliferation of astrocytes were very marked in KA+ NS group (epileptic susceptible rats) (P < 0.05), but those changes were not visible in KA+ NS group (epileptic nonsusceptible rats). CONCLUSIONS: It suggeste

The prevalence of the neuropathological lesions of Alzheimer's disease is independent of race and gender.

Senile plaques (SP) and neurofibrillary tangles (NFT) are the lesions characteristic of Alzheimer's disease (AD). In this study, we examined variation in the proportion of individuals who had these lesions by race, age, and gender in a series of 138 autopsies conducted at the Office of the Chief Medical Examiner of the State of Maryland between 1990 and 1998. Cases were selected on the bases of age between 40 to 79 years and non-natural manner of death, and included 73% males, 61% subjects < 65 years of age, and 42% African Americans. Observations were conducted on histologic sections of the h

Cholesterol and neuropathologic markers of AD: a population-based autopsy study.

OBJECTIVE: To examine the association of plasma cholesterol (total and high-density [HDL] and low-density lipoprotein) levels with neuritic plaques (NP) and neurofibrillary tangles (NFT) in a population-based autopsy series of 218 Japanese American men followed as a part of the Honolulu-Asia Aging Study. METHODS: Cholesterol levels were measured in late life (average age at death 84.6 years) in all subjects (n = 218) and in midlife (20 years before late life) in a subsample (n = 89); for the analyses, levels were categorized into quintiles, with the lowest quintile serving as the reference. Neuropathological abnormalities in schizophrenia: evidence from magnetization transfer imaging.

Post-mortem and structural brain imaging studies in schizophrenia have reported macroscopic changes such as global and regional cortical volume reductions, but it has been more difficult to characterize the histopathological changes that underlie these abnormalities. Magnetization transfer imaging (MTI), a novel MRI technique, more sensitive to subtle or early neuropathological changes than conventional MRI, provides a quantitative measure of macromolecular structural integrity represented by the magnetization transfer ratio (MTR). In this study, we used MTI to examine 25 patients with schizop

Proximal myotonic myopathy: clinical, neuropathologic, and molecular genetic features.

The primary genetic abnormality in myotonic dystrophy (DM) is an expansion of the CTG trinucleotide repeat on chromosome 19q. Recently, patients with similar clinical features, but without this genetic alteration, have been designated as proximal myotonic myopathy (PROMM). We describe two additional cases of PROMM, both of whom presented with clinical features suggestive of myotonic dystrophy. The patients had electromyographic (EMG) evidence of myotonia, normal cardiac evaluation, and no cataracts. Genetic analysis of peripheral blood leukocytes revealed no expansion of the trinucleotide repe

A neuropsychological-neuropathological case study of variant Creutzfeldt-Jakob disease.

We report the first neuropsychological-neuropathological case study of a patient with variant Creutzfeldt-Jakob disease (vCJD) who was seen at the early stages of the disease, and whose cognitive functioning was monitored in the following months until his death. At presentation, his neuropsychological profile included impaired ability to retain new episodic information, deficits on tests of retrieval from semantic memory, and impairments on tests of memory for public knowledge, such as famous personalities. Tests of executive function were also performed poorly. Picture recognition memory and

MELAS with the mitochondrial DNA 3243 point mutation: a neuropathological study.

We performed a neuropathological examination of the central nervous system from seven autopsied patients with mitochondrial myopathy, encephalopathy, lactic acidosis and stroke-like episodes (MELAS). Five of the seven cases were confirmed to have the mitochondrial DNA (mtDNA) 3243 point mutation. In addition to the changes reported previously, diffuse atrophy of the cerebral and cerebellar cortices, diffuse gliosis of cerebral and cerebellar white matter, and cactus formation of Purkinje cells were observed. Electron microscopy revealed accumulation of mitochondria in the cactus formations. Th

Pathological and neuropathological findings in two males with fragile-X syndrome.

The present paper addresses post mortem pathological and neuropathological findings in two males with fragile-X syndrome, aged 67 and 87 years. Both subjects died from sudden, unexpected cardiovascular causes, and both showed abnormalities of the mitral valve, ventricular hypertrophy and cardiomegaly. Both cases demonstrated macrocephaly characteristic of the classical Martin-Bell phenotype in FRAXA. There was increased brain weight in both cases: macroscopically, both cerebral and cerebellar hemispheres appeared normal, but dilated lateral ventricles were seen; and microscopic examination of

Retrospective study of Creutzfeldt-Jakob disease in Belgium: neuropathological findings.

Creutzfeldt-Jakob disease (CJD) is a spongiform encephalopathy that affects about 1 in 10(6) inhabitants in most countries. Recently, a new variant of CJD has been linked to the epidemic of bovine spongiform encephalopathy. Therefore, vigilance concerning the disease's incidence has been increased. We conducted a comprehensive, nation-wide and retrospective study. In 79 Belgian autopsies, we found the characteristic triad of spongiosis, neuronal loss and reactive gliosis. The occipital cortex was most affected, while the cerebellum was mostly spared. Immunohistochemistry was performed using hy

[A sporadic case of Creutzfeldt-Jakob disease with peculiar neuropathological lesions].

The authors report a case of Creutzfeldt-Jakob disease in a man aged 60 years from a family without a history of similar disease. The disease extended over 11 months. In the clinical picture initially equilibrium disturbances and dementia with psychotic symptoms predominated, EEG pattern was not typical of CJD. Neuropathological examination revealed extensive spongiform lesions in the cortex of all cerebral lobes, in striatum and substantia nigra, moreover a considerable number of kuru plaques was found in cerebellar cortex. The authors consider that the case meets the criteria accepted for th

Neuropathological and aetiological studies of sporadic non-suppurative meningoencephalomyelitis of cattle.

Sporadically occurring non-suppurative encephalitis appears to be a frequent condition of Swiss cattle. Fifty-one such cases diagnosed over a period of 10 years were examined retrospectively to investigate whether they constituted one or more distinct diseases, and to search for aetiological agents. Three cases were characterised by periventricular granulomatous encephalitis, and most probably represented a different disease, but the remaining 48 cases had disseminated non-suppurative encephalitis with widespread neuronal changes. Neuronal degeneration was very marked in the hippocampus of 10

Tick-borne encephalitis in dogs: neuropathological findings and distribution of antigen.

Eight dogs originating from different regions of Austria [all of them known as tick-borne encephalitis (TBE) areas] with severe neurological signs were either euthanatized or died spontaneously. Tick-borne encephalitis virus (TBEV) antigen was detected in the brains of five of these dogs by immunohistology, but not in the others. All of the dogs, however, had identical neuropathological changes. There were moderate lymphohistiocytic meningitis, widespread neuronal necroses, karyorrhexis of glial cells, numerous neuronophagic nodules, and extensive microgliosis. In the cerebellum, loss of Purki

Neuropathological findings in new variant CJD and experimental transmission of BSE.

The diagnosis of new variant Creutzfeldt-Jakob disease is dependent on the neuropathological examination of brain tissue following brain biopsy or autopsy. The characteristic neuropathological features are multiple 'florid' plaques in the cerebral and cerebellar cortex, spongiform change most marked in the basal ganglia, severe thalamic gliosis and marked accumulation of the disease-associated prion protein in diffuse or pericellular deposits in the cerebrum and cerebellum. These features allow distinction from cases of sporadic, familial and iatrogenic Creutzfeldt-Jakob disease in neuropathol

Neuropathological and neuroradiological correlates of paranoid symptoms in organic mental disease.

This paper reviews paranoid symptoms in older patients with organic mental disease. We have taken a dual approach to this topic, examining patients with dementia in whom paranoid symptoms are present and also assessing the presence of organic brain changes in patients diagnosed as having late-onset schizophrenia, paraphrenia or delusional disorder. (For the sake of continuity and not wishing to pre-empt any discussion of the nosological categorisation of late-onset psychoses, we refer to late-onset persecutory state as paraphrenia.) Firstly, there is a description of the various paranoid sympt

Laminar cortical necrosis in MELAS syndrome: MR and neuropathological observations.

Laminar cortical necrosis has been described in many conditions of cellular energy depletion, such as hypoxia and hypoglycemia. In MELAS, a genetic defect in mitochondrial protein synthesis leads to impairment of oxidative phosphorylation, with subsequent insufficient energy production within the cell. Neurons are more vulnerable to energy depletion than glial cells and vascular elements, and among the layers of the cerebral cortex the lower laminae are more vulnerable than the superficial layers. We studied a child with severe MELAS syndrome two months before death with MR and compared the im
