Endoscopic therapy for esophageal hematoma with blue rubber bleb nevus syndrome.

A 57-year-old woman previously diagnosed with blue rubber bleb nevus syndrome (BRBNS) reported hematemesis. BRBNS is a rare vascular anomaly syndrome consisting of multifocal hemangiomas of the skin and gastrointestinal (GI) tract but her GI tract had never been examined. An upper gastrointestinal endoscopy revealed a large bleeding esophageal hematoma positioned between the thoracic esophagus and the gastric cardia. An endoscopic injection of polidocanol was used to stop the hematoma from bleeding. The hematoma was incised using the injection needle to reduce the pressure within it. Finally, …

Blue rubber bleb nevus syndrome: a case report and literature review.

Blue rubber bleb nevus syndrome (BRBNS) is a rare disease characterized by multiple venous malformations and hemangiomas in the skin and visceral organs. The lesions often involve the cutaneous and gastrointestinal systems. Other organs can also be involved, such as the central nervous system, liver, and muscles. The most common symptoms are gastrointestinal bleeding and secondary iron deficiency anemia. The syndrome may also present with severe complications such as rupture, intestinal torsion, and intussusception, and can even cause death. Cutaneous malformations are usually asymptomatic and…

Apocrine intraductal carcinoma in situ in nevus sebaceus: two case reports.

Ductal carcinoma in situ (DCIS) in the mammary glands consists of a proliferation of ductal luminal cells with preserved polarity but lacking the marked intercellular cohesion of hyperplastic cells. We report two cases of DCIS arising in the apocrine glandular structures of a nevus sebaceus (NS). The first patient was a 44-year-old woman with a ductal neoplasm composed of multilayered ductal elements with a monomorphous cellularity and a polar orientation of nuclei. The neoplastic cells were positive for Cm5.2 and CK7. The second patient was a 67-year-old woman with phacomatosis pigmentokerato…

So-called "malignant blue nevus": a clinicopathologic study of 23 patients.

BACKGROUND: Melanomas that arise in association with or that resemble blue nevi are extremely rare and have been termed "malignant blue nevi." The authors report on a single-institutional clinicopathologic study of "blue nevus-like melanomas" (BNLMs). METHODS: Twenty-six patients were identified with a "malignant blue nevus" over 29 years at the Sydney Melanoma Unit. Twenty-three patients were included in the current study after pathologic review. Clinical outcomes of those patients were compared with the outcomes in a matched control group of patients with melanoma (matched for age, sex, Bre…

Jaw cyst-Basal cell nevus-Bifid rib syndrome: a case report.

Jaw cyst-Basal cell nevus-Bifid rib syndrome or Gorlin-Goltz syndrome involves multiple organ system. The most common findings include multiple odontogenic keratocysts in the jaws and basal cell nevus on the skin that have an early age onset. These multiple odontogenic keratocysts warrant aggressive treatment at the earliest because of the damage and possible complications associated with them. Recurrence in these lesions is the most characteristic feature that has to be taken in consideration while explaining the prognosis to the patient. A case report of a child affected with Gorlin-Goltz sy…

Speckled lentiginous nevus syndrome: delineation of a new distinct neurocutaneous phenotype.

Speckled lentiginous nevus syndrome, a so far unrecognized cutaneous phenotype associated with neurological anomalies, is postulated on the basis of the following arguments. Phacomatosis pigmentokeratotica represents a twin spot phenomenon. One isolated half of this complex phenotype is observed rather often in the form of Schimmelpenning syndrome, whereas the other half of this twin nevus syndrome consists of a speckled lentiginous nevus associated with various neurological abnormalities incompatible with Schimmelpenning syndrome, such as hyperhidrosis, muscular weakness and dysesthesia. This…

Unilateral megalencephaly in linear nevus sebaceous syndrome: a neuroradiological case report.

We report a case of linear nevus sebaceous syndrome with seizure, mental retardation, and hemiparesis. Magnetic resonance imaging (MRI) clearly demonstrated associated brain malformations of unilateral megalencephaly with cortical dysplasia and white matter change ipsilateral to the sebaceous nevi of the face and neck. Although magnetic resonance angiography (MRA) demonstrated only distortion of the main cerebral arteries without any occlusive or dysplastic findings, single photon emission computed tomography (SPECT) using [123I]N-Isopropyl-p-iodoamphetamine (IMP) revealed hypoperfusion in the…

Pigmented spindle cell nevus: a clinicopathologic analysis of ninety-five cases.

BACKGROUND: Pigmented spindle cell nevus (PSCN) is often interpreted as a Spitz nevus or misdiagnosed as malignant melanoma. OBJECTIVE: The purpose of this study was to analyze the clinical and histologic features and to determine the biologic behavior of 95 cases of PSCN. METHODS: We reviewed clinical data, follow-up information, and microscopic features of all 95 cases of PSCN. RESULTS: PSCNs are dark brown to black, 3 to 6 mm in diameter, and occur most commonly on the extremities (75%) and back (16%) with a predilection for the legs. These lesions are more common in women in the third d…

Infantile Congenital Nevus With Atypical Proliferative Nodules and NTRK Gene Fusion.

Infantile congenital melanocytic nevus (CMN) is a rare type of benign skin lesion originating from neural crest cells. A proliferative nodule (PN) may develop within CMN, and in some cases, atypical proliferative nodules (APNs) with distinct histopathological features may arise, complicating clinical diagnosis and management. This report describes a case of a giant infantile CMN with APN and novel genetic findings, including TPR::NTRK1 gene fusion and MCL1 amplification. An 8-day-old male presented with a large, uniform, hairless black lesion on the plantar skin. Histological examination revea…

Syringocystadenoma Papilliferum and Nevus Sebaceous of Jadassohn: Disentangling Rare Simultaneous Pathologies.

Nevus sebaceous of Jadassohn (NSJ) is a cutaneous benign hamartoma that has been associated with the development of malignant cutaneous tumors such as basal cell carcinoma (BCC). Its coexistence with benign cutaneous tumors has also been reported, while differential diagnosis is considered challenging due to comparable characteristics. Syringocystadenoma papilliferum (SCAP) is a rare benign adnexal tumor and its coexistence with NSJ constitutes an unusual entity, which may lead to diagnostic uncertainty and warrants histopathological examination due to potential malignant transformation. We re…

Facial verrucous epidermal nevus with pruritus: Successfully treated with photodynamic therapy.

Verrucous epidermal nevus (VEN) is uncommon and is typically present at birth. The disorder is essentially cosmetic and is found more often in females which features were brown and skin-colored verrucous papules or plaques distributed linearly along Blaschko's lines. VEN on the face often adversely affect the patient's self-esteem. While complete excision of the lesion is curative, it may leave disfiguring scars. The treatment of VEN on the face is challenging. We report a case of 28-year-old woman who presented with a wart-like lesion on the left side of her face which was accompanied by prur…

Risk Factors for Incident Nevus-Associated vs De Novo Invasive Melanoma.

IMPORTANCE: No prospective epidemiologic studies have investigated genetic vs environmental factors on the risks of nevus-associated melanoma (NAM) and de novo melanoma. OBJECTIVE: To determine whether the risk factor profile differs for nevus-associated and de novo invasive melanoma, and whether the associations differ according to sex. DESIGN, SETTING, AND PARTICIPANTS: This population-based prospective cohort study (the QSkin Study) conducted in Queensland, Australia, included participants aged 40 to 69 years at baseline. Participants were recruited in 2011 and followed up until December …

Blue rubber bleb nevus syndrome: A case report and literature review.

Blue rubber bleb nevus syndrome (BRBNS) is a rare congenital clinical syndrome characterized by venous malformations in multiple organs, including the skin, gastrointestinal tract, liver, and lungs. In June 2022, Third Xiangya Hospital of Central South University admitted a rare case of BRBNS. The patient was hospitalized due to abdominal distension and a history of recurrent hematochezia. Abdominal imaging revealed vascular lesions in the liver and intestines, while colonoscopy demonstrated multiple blue vascular tumors. After symptomatic treatment and sirolimus therapy, the patient's symptom…

THIN MELANOMA ARISING IN NEVUS SPILUS: DERMATOSURGICAL APPROACH WITH FAVOURABLE OUTCOME.

Nevus spilus is a term in the literature used for a benign pigmented cutaneous lesion, occurring shortly after birth or in the early stages of infancy. The lesion itself is very distinguishable in most cases with numerous small papules or macules on a "café au lait" background pigmentation. It can be seen on different parts of the body with predominance in the areas of the extremities. Although benign, in some cases malignant transformation can occur. Malignant melanoma arising within a nevus spilus lesion is not so unlikely anymore. We report a 36-year-old female patient with a thin cutaneous…

Transformed Giant Congenital Melanocytic Nevus on 18 F-FDG PET/CT.

A 50-year-old man experienced cough and bloody sputum for 1 month. CT detected a mass in the right lung. Staging 18 F-FDG PET/CT revealed multiple hypermetabolic lesions in the lung, mediastinum, liver, and bones. Further physical examination revealed black patches in the skin covering most parts of the body, which presented at his birth and were growing very slowly, consistent with giant congenital melanocytic nevus. Pathology examination after biopsy of the lung demonstrated metastatic melanoma.
