Magnetization transfer imaging is an MRI technique that provides quantitative information about in vivo tissue integrity, including myelin and axonal content, and is expressed as the magnetization transfer ratio (MTR). The optic neuritis lesion can model the MS lesion in vivo and permits use of non-invasive markers of optic nerve myelination (visual evoked potential [VEP] latency) and retinal neuroaxonal loss (optical coherence tomography [OCT]) to provide further information about the in vivo substrates of optic nerve MTR. Twenty-five patients with optic neuritis were studied using an optic n…

PURPOSE: To explore the multifocal visual evoked potential (mVEP) as a technique for tracking local optic nerve damage after unilateral optic neuritis (ON).
METHODS: Humphrey visual fields and mVEP recordings were obtained from three patients within 7 days of an episode of ON. Patients were retested during the recovery phase, approximately 4 to 7 weeks later. The multi-input procedure of Sutter was used to obtain 60 local VEP responses (the mVEP) to a scaled checkerboard pattern. The mVEPs were recorded separately for monocular stimulation of both eyes.
RESULTS: Initially, all three patients…

The transient pattern electroretinogram (PERG) was recorded from 16 patients with acute optic neuritis and from 13 patients with acute non-arteritic anterior ischemic optic neuropathy (AION). All patients were tested within 35 days from the the onset of visual symptoms and all had significant central visual field abnormalities in their affected eyes as quantified by automated perimetry. Analysis of the PERGs showed that the amplitude of the N95 peak was abnormally reduced for each eye affected with AION while it remained normal in optic neuritis. No significant alteration in P50 amplitude was …

COVID-19 has affected millions of individuals worldwide, yet the neuro-ophthalmic consequences among survivors remain incompletely characterized. In this case report, we describe a case of unilateral optic neuritis identified in the emergency department using ultrasound in a previously healthy young woman shortly after confirmed SARS-CoV-2 infection. This case highlights the importance of recognizing neuro-ophthalmic sequelae of COVID-19, outlines the diagnostic evaluation, and demonstrates clinical improvement with corticosteroid therapy.

Optic neuritis (ON) has over 60 distinct causes, only one of which is multiple sclerosis (MS). This poses a challenge for its prompt and accurate diagnosis. Patients frequently present with pain that worsens on eye movements, followed by reduced colour vision and acuity, progressing over a couple of weeks. The recovery phase typically follows a similar patte…

A man in his 30s presented with sudden vision loss in the right eye and diminished vision in the left eye accompanied by headache. Clinical findings included bilateral disc oedema, reduced visual acuity in both eyes (right eye>left eye), dense premacular haemorrhage in the right eye and impaired colour vision. Blood tests were positive for serum myelin oligodendrocyte glycoprotein (MOG) antibodies, confirming MOG-associated optic neuritis (ON). Early intravenous methylprednisolone administration led to partial recovery; however, the right eye required pars plana vitrectomy for visual improveme…

The association between allergic diseases and optic neuritis (ON) remains insufficiently characterized. This nationwide cohort study investigated the relationship between major allergic diseases (atopic dermatitis, asthma, and allergic rhinitis) and the risk of ON development. We performed a population-based cohort study utilizing data from the Korean National Health Insurance Service database. The study included 4,052,827 individuals aged 20 years or older who participated in health screenings in 2009. All participants were monitored until December 2018 or until ON diagnosis. Cox proportional…

Ethambutol is a first-line chemotherapeutic agent, which is commonly used in combination with other drugs for the treatment of tuberculosis. Ethambutol-induced optic neuritis is a serious and rare side effect that is either dose or duration-related and causes progressive painless vision loss, and cecocentral scotomas in the visual field. A rare case of ethambutol-induced optic neuritis was reported in a 52-year-old female who was taking anti-tubercular treatment for pulmonary tuberculosis for five months. She presented with painless diminished vision in both eyes. The patient was diagnosed wit…

BACKGROUND AND OBJECTIVES: Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy (GFAP-A) has been reported as a spectrum of autoimmune, inflammatory central nervous system disorders. Linear perivascular radial gadolinium enhancement patterns on brain magnetic resonance imaging (MRI) are a hallmark of these disorders. GFAP-A is associated with cerebrospinal fluid (CSF) GFAP antibody (GFAP-Ab), while the association with serum GFAP-Ab is less clear. This study aimed to observe the clinical characteristic and MRI changes of GFAP-Ab-positive optic neuritis (ON).
METHODS: We performed …

Optic neuritis is an inflammatory demyelinating disorder that primarily affects the optic nerve and is often associated with multiple sclerosis. While it is rare for optic neuritis to be accompanied by autoimmune encephalitis, it can occur in some cases. A 65-year-old woman with bipolar disorder presented with a progressively altered mentality. Magnetic resonance imaging of the brain showed no definite abnormal findings. Electroencephalography revealed nonconvulsive status epilepticus. Cerebrospinal fluid study and autoimmune and paraneoplastic encephalitis antibodies were negative. The patien…

The coexistence of optic neuritis and Guillain-Barré syndrome is a rare combination of neurological diseases. The trigger of an autoimmune inflammatory process is often a respiratory mycoplasma infection. Ignorance of such combination can lead to diagnostic and therapy mistakes. This article describes the case of a rare combination of overlapping optic neuritis and Guillain-Barré syndrome, associated with Mycoplasma pneumoniae and provides the short literature review. Further studies are required to identify common pathogenetic mechanisms of combined inflammatory lesions of the optic nerves an…
