Compromised Conscience: A Scoping Review of Moral Injury Among Firefighters, Paramedics, and Police Officers.

BACKGROUND: Public Safety Personnel (e.g., firefighters, paramedics, and police officers) are routinely exposed to human suffering and need to make quick, morally challenging decisions. Such decisions can affect their psychological wellbeing. Participating in or observing an event or situation that conflicts with personal values can potentially lead to the development of moral injury. Common stressors associated with moral injury include betrayal, inability to prevent death or harm, and ethical dilemmas. Potentially psychologically traumatic event exposures and post-traumatic stress disorder c

Occupational Therapy Within Integrated Community Care Systems: A Systematic Review and Narrative Synthesis of Policy and System-Level Evidence.

BACKGROUND/OBJECTIVES: Population aging and rising long-term-care demand require community-based systems that support function and participation. Occupational therapy (OT) is provided across community-care settings, yet its policy- and system-level position remains inconsistently defined. This review examined how OT is positioned, financed, accessed, coordinated, staffed, and implemented within integrated community care. METHODS: PubMed, CINAHL, Scopus, ProQuest Central, and APA PsycINFO were searched for English-language peer-reviewed literature published from 1 January 2010 to 29 July 2026.

Mapping the process of implementing Health System Performance Assessment into political decision-making: expert survey among 22 European countries.

BACKGROUND: Health System Performance Assessment (HSPA) is an essential tool for evaluating and improving health system efficiency, transparency, and accountability. While existing research has focused mainly on HSPA frameworks, indicators and methodological issues, less attention has been paid to how HSPA results are reported, reviewed and linked to policy-making. OBJECTIVES: This study maps the implementation of HSPA across European countries and examines its reported relevance for policy-making from the perspective of national HSPA representatives. METHODS: The study is based on a structu

Exploring influences on intravenous-to-oral antibiotic switching: a qualitative interview study across healthcare professions.

OBJECTIVES: Intravenous antibiotics are often continued longer than necessary leading to prolonged admission and increased infection risk. Existing research has reported barriers to IV-to-oral switch (IVOS) predominantly from a physician's perspective, leaving gaps in understanding how other healthcare professionals may influence IVOS decisions. This study aimed to explore influencers to IVOS among different healthcare professions. METHODS: Semi-structured interviews were conducted with doctors, nurses, pharmacists, and pharmacy technicians at an acute UK NHS hospital trust, selected through

Management of pain for people with dementia living in nursing homes: stakeholders' perspectives on a prototype of a complex intervention.

BACKGROUND: Healthcare personnel often report uncertainty about how to identify and alleviate pain in people with dementia. Up to 80% of nursing home residents with dementia are estimated to experience chronic pain. There is a lack of structured pain management interventions that focus on integrating and building upon the individual resources of people with dementia in the nursing home context. Although a prototype of such an intervention has been developed, further knowledge is needed regarding its functioning in clinical practice, relevance, and capacity to meet clinical needs. Stakeholders

Perceived Competence in Nutrition Care Among Hospital Nurses and Its Associated Factors: A Cross-Sectional Study.

Background/Objectives: Hospital malnutrition is a prevalent and clinically significant issue associated with prolonged hospital stays, increased complications, and mortality. Although nurses play a key role in monitoring nutritional status and delivering nutrition care, many lack confidence and do not perceive nutrition care as part of their primary responsibilities. This study assessed nurses' self-perceived nutrition care competence, identified associated factors, and examined their objectively scored nutrition knowledge alongside self-reported nutrition care competence. Methods: A cross-sec

The Independent Associations of Professional Autonomy, the Work Environment, and Role Ambiguity with Job Satisfaction Among Korean Nurses in Physician-Support Roles: A Cross-Sectional Study.

Background/Objectives: Korean hospitals have deployed nurses in physician-support roles since 1997, and more so since the physicians' collective action in 2024, yet the role lacked a clear legal definition until the Nursing Act and its enforcement rule provided one. This study asked which of professional autonomy, the work environment, and role ambiguity remains independently associated with job satisfaction when the three are considered together. Methods: A total of 130 nurses in these roles at two Korean university hospitals completed a cross-sectional survey in July-August 2025, before that

[Optimizing follow-up of pregnancies of unknown location: a simplified clinical algorithm for ectopic pregnancy risk].

OBJECTIVES: Management of pregnancies of unknown location (PUL) relies on close monitoring with serial human chorionic gonadotropin (hCG) measurements and repeated ultrasound examinations to exclude ectopic pregnancy (EP). Although safe, this strategy leads to multiple consultations and significant organizational and economic burden. This paper aims to develop and preliminarily validate a simplified clinical algorithm to stratify the risk of EP in women presenting with PUL, in order to adapt follow-up frequency. METHODS: The prospective monocentric OPTIGLI study was conducted at Nantes Univer

Perspectives of Healthcare Providers and Managers on Older Adults' Transition to a Long-Term Care Facility in the Côte-Nord Region of Quebec.

Background/Objectives: Older adults' transitions to long-term care facilities (LTCFs) are complex. In Quebec, Canada, recent policies promoted LTCFs as living environments. The Life Story questionnaire was suggested as one tool to support this shift. In addition, rural regions may face specific barriers to successful transitions to LTCFs. The Côte-Nord region, with a vast territory and a low population density, offers a distinctly rural setting. The objective of this study was therefore to describe healthcare providers' and managers' perceptions of the use of the Life Story questionnaire durin
