Testing the proficiency to distinguish locations with elevated plantar pressure within and between professional groups of foot therapists.

BACKGROUND: Identification of locations with elevated plantar pressures is important in daily foot care for patients with rheumatoid arthritis, metatarsalgia and diabetes. The purpose of the present study was to evaluate the proficiency of podiatrists, pedorthists and orthotists, to distinguish locations with elevated plantar pressure in patients with metatarsalgia. METHODS: Ten podiatrists, ten pedorthists and ten orthotists working in The Netherlands were asked to identify locations with excessively high plantar pressure in three patients with forefoot complaints. Therapists were instructed

Analysis of stiffness reduction in varying curvature ankle foot orthoses.

Ankle foot orthoses (AFO) are often used for patients who cannot generate a strong enough extension moment at the knee to allow functional gait. Orthotists often cut out portions of the AFO around the malleoli in order to improve comfort. There has been some question as to how this affects the stress distribution around the orthosis, the fatigue performance of the device, and the AFOs stiffness. To examine this, three orthoses were constructed with differing curvatures cut out of the malleolar regions. Photoelastic coatings were placed on the most stiff and least stiff orthoses, and the stress

A proposed prognostic formula for Perthes' disease.

UNLABELLED: We retrospectively reviewed 145 patients with unilateral Perthes' disease and compared a quantitative analysis of early radiographic signs with a predicted prognosis at long-term followup. The average age of the patients at followup was 18.7 years (range, 16.2-27.5 years). We used the age at onset and three radiographic factors as independent variables for multiple regression analysis. The final radiographic results were based on a modified Stulberg's classification as the dependent variable. Fifty patients (35%) had a good outcome, 33 patients (23%) had a fair outcome, and 62 pati

Elbow orthoses: a review of literature.

The objective of this study is to review the available literature on elbow orthoses in patients with various diagnoses to assess the scientific base of the prescription of elbow orthoses. A search of literature in Medline, Embase, Cochrane and Recal was performed using the keyword "elbow" combined with "orthosis related keywords". Abstracts were read to select the papers dealing primarily with monoarticular elbow orthoses. References of the selected papers were also examined. A total of 57 papers were read of which 18 met the selection criteria. Both the quality and quantity of the studies app

Surgical treatment with the technique of Galveston-Luque of spine deformities associated with the prune belly syndrome.

In prune belly syndrome (PBS), spine deformities, particularly scoliosis, are the most frequent accompanying orthopedic problem (18%). We give a detailed description of two cases with prune belly-associated spine deformities. A combined anterior-posterior fusion procedure with posterior correction and stabilization according to the Galveston-Luque technique was carried out in both patients. In one patient insufficiency of the pelvic fixation developed with loss of balance. Reoperation was performed. In the second patient correct alignment and balance was achieved, breakage of both metal rods o

A six-month followup of a randomized trial comparing the efficacy of a lateral-wedge insole with subtalar strapping and an in-shoe lateral-wedge insole in patients with varus deformity osteoarthritis of the knee.

OBJECTIVE: To assess the effect of a lateral-wedge insole with elastic strapping of the subtalar joint on the femorotibial angle in patients with varus deformity of the knee. METHODS: The efficacy of a wedged insole with subtalar straps and that of a traditional wedged insole shoe insert were compared. Sixty-six female outpatients with knee osteoarthritis (OA) were randomized (according to birth date) to be treated with either the strapped or the traditional inserted insole. Standing radiographs with unilateral insole use were used to analyze the femorotibial angles for each patient. In both

Gait in stabilized hemiplegic outpatients in rehabilitation.

PURPOSE: Hemiplegic patients generally expect an improvement of their gait even at 1 year after stroke. This open and prospective study was addressed to detect even subtle changes in gait characteristics in stabilized patients in rehabilitation treatment by using an opto-electronic system of analysis (ELITE). METHODS: Forty-two hemiplegic patients were entered on this study, and mean age was 63.2 years (SD 9.9). The gait measures were obtained four times for each patient; at time of baseline, at the 15th, 30th and 90th day from the beginning of rehabilitation treatment. Plantar flexion moment

[Patient with skeletal metastases. Mobile despite destroyed bones].

Bone metastases endanger both the mobility of the cancer patient and his ability to profit from medical/nursing care. Treatment aims to prevent fractures of the long bones and vertebrae and to avoid neurological defects caused by compression of the spinal cord. Treatment is indicated when the following criteria are met: for the long bones a defect measuring more than 2.5 cm in length or affecting more than 50% of the circumference; for the vertebrae destruction of more than 60%. Almost all patients require radiotherapy, with a minority also needing surgical treatment in the form of intralesion

External tibial torsion and the effectiveness of the solid ankle-foot orthoses.

The aims of this study were to determine the influence of external tibial torsion on the effectiveness of the ankle-foot orthoses (AFO) in children with lumbosacral myelomeningocele. Forty patients with normal tibial rotation and 18 patients with excessive external tibial torsion were evaluated with three-dimensional gait analysis at their comfortable walking speed. The group with normal tibial rotation showed significantly greater knee extension and lower mean extension moment compared with the group with external tibial torsion (p < 0.05). The posteriorly and laterally deviated ground-reacti

Advances in congenital craniofacial surgery.

Surgery for the correction of craniofacial anomalies has come a long way since its beginnings more than three decades ago. Throughout this period numerous developments have occurred in diagnosis and management, as well as a better understanding of the etiology of craniofacial anomalies. Significant technological advancements in imaging have allowed for more precise diagnosis and surgical planning. Quantitative analysis techniques have been developed allowing for more precise results analysis. Nonsurgical management techniques have improved, as has the understanding of the proper utilization of

Mid-trimester ultrasound diagnosis of isolated talipes equinovarus: accuracy and outcome for infants.

Seventeen fetuses were diagnosed with isolated congenital talipes equinovarus (CTEV) on mid-trimester ultrasound at the Royal Women's Hospital, Melbourne, between January, 1992 and December 1995. Sixteen of the 17 cases had an amniocentesis performed and all karyotypes were normal. The remaining case was phenotypically normal, except for a clubfoot. None of the pregnancies was complicated by any of the recognized intrauterine environmental causes of CTEV. Four of the babies were delivered prematurely and all survived the neonatal period. Six (35%) infants did not have CTEV at birth, although 2

The effect of use of a wrist orthosis during functional activities on surface electromyography of the wrist extensors in normal subjects.

Wrist orthoses are advocated for patients with lateral epicondylitis on the assumption that use of the orthosis decreases muscle activity of the wrist extensors during activities. The purpose of this study was to compare the amount of electrical activity, root mean square (RMS) calculated from surface EMG recorded over the wrist extensors, during activities when applying four conditions of wrist orthoses: dorsal; volar; semicircular; and no orthosis. Thirteen normal subjects (mean age 27.7 years) performed three lifting and two gripping tasks, repeated on three consecutive days under four orth

Chopart prosthesis and semirigid foot orthosis in traumatic forefoot amputation. Comparative gait analysis.

Gait was analyzed in seven otherwise healthy males at least 11 mo after they had recovered from a traumatic unilateral transmetatarsal amputation incurred during the course of their usual occupation. All seven were fitted with a semirigid foot orthosis. Four were also fitted with a Chopart prosthesis. Gait was evaluated with forceplate measurements of ground reaction force during free walking, by clinical observation of such ambulation on videotape, and by the subjective impression of the men as obtained by a questionnaire. In all men, with unmodified footwear, with the orthosis, and with the

Comparison of prehospital cervical immobilization devices using video and electromyography.

INTRODUCTION: Previous evaluations of prehospital devices intended for spinal immobilization have focused on the device's ability to restrict motion only. This study defines six relevant criteria for evaluation of cervical immobilization device (CID) performance. OBJECTIVES: To suggest relevant criteria for evaluation and use available technology to improve measurements for performance testing of prehospital-care devices. METHODS: Six parameters (motion restriction, access, ease of application, environmental performance, radiolucency, and storage size) were used to evaluate three types of CI

Fracture-dislocation of the shoulder in a child: case report.

This is a case of an 11-year old girl with a delayed diagnosis of a shoulder fracture-dislocation. Shoulder dislocations are rare, and proximal humerus fractures are uncommon. A fracture-dislocation in a child is, to our knowledge, as yet unrepresented in the English-language literature. The child's injury resolved without recurrence.

[Functional velopalatal insufficiency. Therapeutic possibilities].

The function of the soft palate is complex; it takes part in speech, swallowing, and masticatory mechanisms. Its pathology ranges from morphological deficits to functional ones; these are commonly due to a central nervous system damage. The clinical analysis is based upon examination and instrumental prove, and leads to an optimum choice between surgical and prosthetic solutions. The latter is the best one in terms of quick execution, low costs and future possibilities of improvement.

Results of newborn screening for CDH with and without sonography and correlation of risk factors.

The newborn screening results of 1,310 children examined by Ortolani's and Barlow's manual test, as well as 2,587 children investigated by sonography, have been compared. Of the sonographically pathological hips (technique of Graf), 52.2% had no sign of instability. This was especially true in dysplastic and decentering hip joints, but was found to a smaller degree even in dislocated hips. As a result, twice as many abduction pillows were ordered during the second period. Sonography also detected joints that were immature at birth, but became worse during development (8.9% of hips labeled type

[Anti-calcaneus carbon fiber orthosis in children with myelomeningocele].

An anti-calcaneus carbon fibre orthosis (ACCFO) is an appliance combining a plantar orthosis with an anterior counter-support at the tibia and two side supports, medial and lateral, leaving the posterior aspect of the leg and heel free. It is made of polyethylene and polycarbolactone resin reinforced by a covering of tubular carbon fibres and polymerized epoxy-resin. Fifty-five pairs have been used in 18 children aged from one years and five months to 12 years suffering from a paraplegia at L4-L5 or S1 level due to myelomeningocele. Two children were unable to walk before the use of the ACCFO
