Association between spinal morning stiffness and lumbar disc degeneration: the Rotterdam Study.

OBJECTIVE: To explore the associations between spinal morning stiffness and lumbar disc degeneration (LDD). DESIGN: Data from a cross-sectional general population-based study (Rotterdam Study-I) were used. Intervertebral disc spaces and osteophytes of people aged ≥55 years were scored on lumbar lateral radiographs (L1-2 through L5-S1 was scored). Logistic regression analysis was used to explore associations between spinal morning stiffness and two definitions of LDD (i.e., 'narrowing' and 'osteophytes'). Spinal morning stiffness combined with low back pain and its association with LDD was als…

Quantifying features from X-ray images to assess early stage knee osteoarthritis.

Knee osteoarthritis (KOA) is a progressive degenerative joint disease and a leading cause of disability worldwide. Manual diagnosis of KOA f … design customized algorithms that automatically detect and quantify joint space narrowing (JSN) and osteophytes from the extracted edges. The proposed model quantitatively assesses JSN and finds the number of intercondylar osteophytes, contributing to severity classification. The system achieves accuracies of 88% for JSN detection, 80% for osteophyte identif …

The use of a new high-speed shielded curved drill is associated with improved intraoperative and clinical outcomes after cervical corpectomy and fusion procedures: a retrospective case series.

BACKGROUND: Anterior cervical corpectomy and fusion (ACCF) is an effective technique to address multi-level degenerative cervical myelopathy. However, as the number of surgical levels increases, the outcomes worsen with respect to complication rates, range of motion and length of surgery. This study aimed to determine the clinical outcome of ACCF procedures performed using a new distally curved and shielded drilling device. METHODS: A retrospective study was conducted on 43 ACCF procedures in which the device was used for osteophyte removal. Patient files were reviewed to assess the early cli…

Effects of Resection of Posterior Condyles of Femur on Extension Gap of Knee Joint in Total Knee Arthroplasty.

BACKGROUND: When evaluating the effects of the preparation of the flexion gap on the extension gap in total knee arthroplasty (TKA), the effects of posterior condylar resection and osteophyte removal on the extension gap should be differentiated. Although the amount of osteophytes differs between patients, posterior condylar resection is a procedure that is routinely implemented in TKA. The aim of this study was to assess the effects of the resection of the posterior condyle of the femur on the extension gap in posterior-stabilized (PS) TKA. METHODS: We enrolled 40 knees that underwent PS TKA…

Superior dislocation of the patella: a pathognomonic finding and review of literature.

A 59-year-old woman with a painful right knee that became locked in extension after a trivial trauma was seen at the emergency room. This was caused by unloaded hyperextension in bed. She was diagnosed with a superior dislocation of the patella. A closed reduction was performed, but a recurrent episode was seen within a week. An arthroscopy was performed, in which the causative osteophytes were removed. In the 12-month follow-up after treatment, no recurrence was seen. A superior dislocation of the patella is caused by patellofemoral osteophytes that interlock. This can cause a degenerative kn…

Locked Superior Patellar Dislocation.

Superior dislocation of the patella is a rare cause of knee locking, with fewer than 20 cases reported in the English literature. The vast majority of traumatic or even atraumatic knee locking cases are due to such varied conditions as meniscal tears, osteochondral lesions, osteoarthritis, and ligamentous tears. A careful examination with proper radiographic imaging must be undertaken to properly evaluate patients who present with a locked knee, as they may have interdigitating patellofemoral osteophytes. An unusual case of a locked knee secondary to interlocking osteophytes between the femora…

The Valgus Stress Radiograph Does Not Determine the Full Extent of Correction of Deformity Prior to Medial Unicompartmental Knee Arthroplasty.

Routine preoperative stress radiographs have been advocated, in part, to determine "full correctability" of deformities before proceeding with unicompartmental knee arthroplasty (UKA) despite limited data supporting their utility. Fifty consecutive patients undergoing medial UKA with robotic navigation were studied. In 20° of flexion, significantly greater correctability was achieved after removal of osteophytes by an additional 1.8°, with a mean corrected alignment of 2.5° varus. Seventy-four percent of knees were not correctable to neutral alignment or more. In conclusion, preoperative stres…
