[Anesthetic management of four pediatric patients with CCAM for pulmonary lobectomy].

We report perioperative management of four pediatric patients with congenital cystic adenomatoid malformation (CCAM) for pulmonary lobectomy under general anesthesia combined with thoracic epidural anesthesia. The patients were two newborn infants and two infants. A 23-day-old girl whose cyst contained air and a one-day-old girl whose cyst content was drained percutaneously before induction, showed uneventful courses during perioperative period. In a one-yr-old boy with a cyst filled with purulent material, the endotracheal tube became nearly occluded with copious purulent material during oper

Update on lasers in pediatric dermatology: how primary care providers can help patients and families navigate appropriate treatment options and timelines.

PURPOSE OF REVIEW: The use of lasers in pediatric dermatology is well established, but recent literature has expanded the evidence for specific timelines of treatment. Additionally, new devices and combinations with medical therapy have improved outcomes and treatment options for various conditions. RECENT FINDINGS: Pulsed dye laser remains the first-line laser for vascular lesions. Recent guidelines support early initiation of laser treatment in port-wine birthmarks to optimize outcomes. For hemangiomas, laser treatment can offer a meaningful addition to oral propranolol therapy. Lasers with

Regional anesthesia for sickle cell disease vaso-occlusive crisis: A single-center case series.

Pain management is challenging for patients with sickle cell disease (SCD) who present in vaso-occlusive crisis (VOC). Opioid therapy is highly effective, nevertheless undesirable side effects can hinder their effectiveness. Regional anesthesia with deposition of perineural anesthetic offers nociceptive blockade, local vasodilatation, and reduces the inflammatory response. Among pediatric patients, continuous peripheral nerve block (CPNB) for perioperative adjunctive analgesia is safe. Herein, we describe the trajectory of a cohort of pediatric SCD patients with opioid-refractory upper-extremi

Awake flexible bronchoscopy in children: A case series, feasibility and precautions.

INTRODUCTION: Flexible bronchoscopy under anesthesia is a mainstay diagnostic tool for evaluating respiratory disorders in pediatric patients. While flexible bronchoscopy is generally regarded as a safe procedure with low risk for major complications, it does entail additional risks associated with the use of general anesthesia. The use of diagnostic awake flexible bronchoscopy in children is not well documented in current literature. OBJECTIVES: The objective of this case series is to investigate the feasibility and potential utility of awake flexible bronchoscopy in pediatric patients and t

Surgical Risk, Operative Time, and Anesthesia Time Associated With Combining Tracheostomy and Gastrostomy Tube Placement Under a Single Anesthetic.

INTRODUCTION: Pediatric patients may need both tracheostomy and gastrostomy tube (G-tube) placement to satisfy both oxygen and nutritional requirements for sustaining life. It is unclear if combining both procedures under one anesthetic is associated with reductions in total operative time or surgical risk, compared to performing the two procedures separately. METHODS: Our study used the 2016-2021 National Surgical Quality Improvement Program-Pediatric Participant Use Files. Patients age 0-2 years were included if they underwent elective tracheostomy or G-tube placement and no concomitant pro

Which frequency is better for pediatric shock wave lithotripsy? Intermediate or low: a prospective randomized study.

PURPOSE: Shock wave lithotripsy (SWL) is the first option in the treatment of pediatric kidney stones; however, optimal frequency is still uncertain. The aim of this study was to compare low frequency [60 shocks per minute (SWs/min)] and intermediate frequency [90 SWs/min] in terms of lithotripsy success, complications, cardiac arrhythmia, anesthesia time, secondary procedures, and efficiency quotient (EQ) in children. METHODS: Seventy-eight consecutive children who received SWL for radiopaque renal stones between July 2016 and January 2020 were randomly divided into two groups: Group 60 (SWL

Anesthetic management of patients with an anterior mediastinal mass: continuing professional development.

PURPOSE: Many cases have been reported of hemodynamic and airway collapse induced by general anesthesia in patients with an anterior mediastinal mass. We examined the literature for predictors of perioperative risk, guidelines for preoperative investigations, and strategies for management of the patient with a mediastinal mass. PRINCIPAL FINDINGS: In patients with an anterior mediastinal mass, symptoms may range from none to severe and may include orthopnea, stridor, cyanosis, jugular vein distension, or superior vena cava syndrome. In limited case series, incidences of serious complications

Leptomeningeal Vascularity in Gadopiclenol-Enhanced Pediatric Brain MRI.

BACKGROUND AND PURPOSE: Gadolinium-based contrast agents (GBCAs) are essential in pediatric neuroimaging, enabling visualization of pathology that disrupts the blood-brain barrier. Gadopiclenol is a new macrocyclic GBCA with higher T1 relaxivity, permitting a 50% dose reduction while maintaining diagnostic quality. However, it is unknown whether this higher relaxivity alters background extra-axial vascular enhancement. We hypothesized that gadopiclenol, despite its lower dose, would increase background enhancement on postcontrast pediatric brain MRI compared with gadoterate meglumine (Gd-DOTA)

Precision in Planning: Airway Battles in Pediatric Temporomandibular Joint Ankylosis.

BACKGROUND: Pediatric temporomandibular joint (TMJ) ankylosis produces severe trismus, mandibular hypoplasia, and retrognathia, creating an anticipated difficult airway with minimal apnea tolerance. Awake fiberoptic intubation is often impractical in children; therefore, airway strategies must prioritize continuous oxygenation and preservation of spontaneous ventilation. METHODS: A consecutive case series of seven pediatric patients with TMJ ankylosis undergoing maxillofacial surgery was analyzed. All underwent inhalational induction with 8% sevoflurane in oxygen while maintaining spontaneous

Transnasopharyngeal positive pressure ventilation during balloon dilation of severe subglottic stenosis in a low-weight infant: a case report.

BACKGROUND: Acquired subglottic stenosis (ASGS) is a significant cause of pediatric airway obstruction and presents various anesthetic challenges due to the complexities of altered airway anatomy. While balloon dilation serves as a first-line minimally invasive intervention, shared airway management between surgeons and anesthesiologists requires meticulous coordination. This case report demonstrates the successful implementation of nasopharyngeal airway-assisted positive pressure ventilation during balloon dilation in a critically ill infant with grade III ASGS. This approach proposes a new v

A comparative evaluation of intranasal α2-adrenoceptor agonists and intranasal midazolam as premedication in pediatric sedation: A meta-analysis of randomized controlled trials.

BACKGROUND: Midazolam and α2-adrenoceptor agonists have been widely used off-label as intranasal sedatives for children. The present meta-analysis aimed to evaluate the effects of two interventions in pediatric sedation. METHODS: PubMed, Embase, and Cochrane Library were searched from inception to April 2022. All randomized controlled trials used intranasal α2-adrenoceptor agonists and midazolam as sedatives in children were enrolled. Parental separation, anesthesia induction or facemask acceptance, sedation level, different hemodynamic parameters and adverse events were considered as outcome

[Ultrasound-guided paravertebral block for pyloromyotomy in 3 neonates with congenital hypertrophic pyloric stenosis].

BACKGROUND AND OBJECTIVES: Hypertrophic pyloric stenosis is a relatively common affection of gastrointestinal tract in childhood that results in symptoms, such as projectile vomiting and metabolic disorders that imply a high risk of aspiration during anesthetic induction. In this way, the carrying out of a technique with general anesthesia and intravenous rapid sequence induction, preoxygenation and cricoid pressure are recommended. After the correction of systemic metabolic alkalosis and pH normalization, cerebrospinal fluid can keep a state of metabolic alkalosis. This circumstance, in addit

Pediatric nasolacrimal duct obstruction.

PURPOSE OF REVIEW: Review the current management for pediatric nasolacrimal duct obstruction and congenital dacryocele. RECENT FINDINGS: Early probing in the office, and probing beyond 1 year of age in a facility with general anesthesia are equally effective. Congenital nasolacrimal duct obstruction is associated with anisometropic amblyopia. Infants with unilateral dacryocele are at risk for developing a dacryocele on the unaffected side. SUMMARY: The decision to probe early in the office or continue medical management and probe beyond a year of age in a facility with a general anesthetic i
