Psychiatric Disorders Among Patients Seeking Treatment for Co-Occurring Chronic Pain and Opioid Use Disorder.

OBJECTIVE: Psychiatric comorbidities complicate treatment of patients with chronic pain and opioid use disorder, but the prevalence of specific comorbid psychiatric disorders in this population has not been systematically investigated. METHODS: 170 consecutive participants entering a treatment research program for co-occurring chronic pain and opioid use disorder between March 2009 and July 2013 were evaluated with the Structured Clinical Interview for DSM-IV-TR Axis I Disorders (SCID-I/P) and the Diagnostic Interview for DSM-IV Personality Disorders (DIPD-IV). RESULTS: The prevalence of any

[Pharmacotherapy of a 67-year old female with borderline personality disorder].

Pharmacotherapy in older adults with personality disorders is very complicated. On the one hand, this is caused by interference of the personality disorder in the therapeutic relationship. On the other hand, age specific factors, such as polypharmacy and changing pharmacokinetics and -dynamics play an important complicating role. In this article the difficulties of pharmacotherapy in older adults with personality disorders are illustrated by the description of a case of a 67-year old female with a borderline personality disorder. She has an extensive history of many therapies, which have not b

The dependent personality and interpersonal problems.

At the core of the dependent personality disorder lies an excessive reliance on other people for support, direction, and nurturance. However, because of their interpersonal style, many patients high in dependency have difficulties establishing and maintaining close interpersonal relationships. The present study examined interpersonal problems related to the dependent personality disorder. The relationships between dependency and interpersonal problems were examined in 43 adult psychiatric inpatients and 66 nonpatient adults. All subjects were assessed for social functioning in terms of social

Should neurologists treat common psychiatric comorbidities in patients with epilepsy?

Psychiatric comorbidities are common and highly impactful among people with epilepsy, yet they are underrecognized and undertreated due to challenges including shortage of specialty mental health providers and lack of neurologist education to enable management by neurologists. The purpose of this special issue is to address these gaps by providing a practical resource for neurologists to safely manage comorbidities via pharmacotherapy for common comorbidities. In this introductory article, we summarize key categories of psychiatric problems in epilepsy and a broad overview of management strate

Perceptions of parental bonding in patients with various personality disorders, lifetime depressive disorders, and healthy controls.

The aims of the present study was (a) to examine the relationship between parental bonding and various personality disorders, when the effect of a lifetime depressive disorder was statistically controlled for, and (b) to examine the relationship between parental bonding and a lifetime depressive disorder when the effects of various personality disorders were accounted for. The study included 135 psychiatric outpatients and 41 healthy controls. The patients were diagnosed according to DSM-III-R axis I and axis II. Results indicated that obsessive-compulsive personality disorder was associated w

Impact of co-occurring psychiatric disorders on retention in a methadone maintenance program: an 18-month follow-up study.

We assess the influence of co-occurring psychiatric disorders on retention in 189 opioid dependent patients in a methadone maintenance treatment (MMT) and determine the incidence of psychiatric co-morbidity during an 18-month follow-up period. About 68.5 % were retained in the MMT. Neither co-occurring mental disorders (chi-square = 0.303, df = 1, p = 0.622) nor methadone doses [85 (88.9) vs. 79.2 (85) mg/day, p = 0.672] were related to retention. In the follow-up period 19 new diagnoses were made, mainly major depression and antisocial and borderline personality disorders. Co-occurring psychi

Association of vitiligo with hospitalization for mental health disorders in US adults.

BACKGROUND: Vitiligo has a complex bidirectional relationship with mental health (MH) disturbances. However, little is known about the relationship between vitiligo and MH emergencies. OBJECTIVE: To examine the associations of vitiligo and MH hospitalizations in the United States. METHODS: Data from the 2002 to 2012 National Inpatient Sample were analysed, including a ~20% sample of all US hospitalizations (n = 87 053 155 children and adults). Prevalence of hospitalization for MH disorders, their length of stay (LOS) and cost of care were determined for those with vitiligo compared to those

Domestic Mass Shooters: The Association With Unmedicated and Untreated Psychiatric Illness.

BACKGROUND: Given the relative lack of psychiatric information and data on the perpetrators of US mass shootings, the aim of our study was to understand who these "mass shooters" were and whether they had a psychiatric illness. If so, were they competently diagnosed, and if so, were they treated with appropriate medication for their diagnoses before the violence? METHODS: Because a prospective study of diagnosis and treatment could not, for obvious reasons, be carried out, we designed a retrospective, observational study of mass shooters, defined as those who killed 4 or more people with fire

[Mental diseases and sexual health in old age and their reciprocal influence].

BACKGROUND: The proportion of aging persons in the population is growing, accompanied by an increase in mental and neurological disorders. The sexuality of older people is often overlooked or stigmatized, despite playing a central role in well-being. Mental illnesses can substantially impair sexual health, while conversely, sexual dysfunctions are considered risk factors for mental distress. OBJECTIVE: This study examined the reciprocal relationship between mental health and sexuality in older age, considering biopsychosocial aspects. METHODS: The investigation is based on a selective litera

[Anxiety disorders--syndrome, personality and relationships. Results of multi-methodology diagnosis].

Patients attending the Polyclinic of the Psychiatry Department of the Ludwig Maximilian University in Munich for ambulatory examination and counseling because of panic anxiety states with agoraphobia, were investigated using methods of descriptive-phenomenological diagnostic evaluation of syndromes and personality (structured clinical interviews on the basis of DSM III Criteria) and with methods of empirical married couple and family diagnostic evaluation (Giessen-Paar-Test, Family-Assessment-Measure). On the basis of the findings established on three different observational planes--syndrome,

The prevention of recurrent suicidal acts.

1 There have been few controlled prospective investigations into the prevention of suicidal behaviour and by and large they have failed to demonstrate the efficacy of social work, psychotherapy or psychiatric treatment. 2 A group of 58 high-risk patients with multiple episodes of suicidal behaviour was treated with mianserin 30 mg at night or placebo in a six month double-blind trial of the efficacy of an antidepressant in reducing suicidal behaviour. 3 Patients were screened for depression, schizophrenia and organic disease. Patients were diagnosed as suffering from personality disorders acco

The Maudsley long-term follow-up of child and adolescent depression. 1. Psychiatric outcomes in adulthood.

BACKGROUND: Strong links exist between juvenile and adult depression but comorbid conduct disorder in childhood may mitigate this continuity. AIMS: To test the impact of comorbid conduct disorder on psychiatric adult outcomes. METHOD: A group of 149 subjects assessed at the Maudsley Hospital in the period 1970-1983 and meeting DSM-IV criteria for major depressive disorder with (n=53) or without (n=96) conduct disorder were interviewed 20 years later. Data were collected on the lifetime history of psychiatric disorders. RESULTS: Adult depressive recurrence was high for major depression (62.4

Patients with Hidradenitis Suppurativa Have a High Psychiatric Disease Burden: A Finnish Nationwide Registry Study.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease of hair follicles that is associated with various comorbidities. To further clarify the associations between HS and psychiatric disorders, we conducted a nationwide retrospective study that included 4,381 patients with HS, and 39,554 with psoriasis and 43,248 with melanocytic nevi as controls. Patient data were obtained from the statutory Finnish Care Register for Health Care. The incidence of HS was 3.0/100,000 persons/year. At least one psychiatric diagnosis was found in 24.1% of the patients with HS compared with 19.1% of

Hear My Voice: One Woman's Experience of Seclusion: A Case Study in a Secure Forensic Hospital.

Seclusion use can cause harm and exacerbate existing trauma for the person secluded. Literature reporting on the woman's experience is sparse. However, it is understood that the experience of seclusion is negative, harmful, associated with feelings of fear and perceived as a punishment for behaviour. Women are under-represented in forensic hospital settings; therefore, their voices and experiences are rarely heard. This study aimed to explore the experience of seclusion for a woman admitted to a secure forensic hospital using a single case study approach. This study draws on findings from a pr

Involuntary admission: the case of anorexia nervosa.

Involuntary treatment of psychiatric disorders has always been controversial; this is especially true for eating disorders. Patients with anorexia nervosa of life threatening severity frequently refuse psychiatric hospitalization. Ambivalence toward treatment is characteristic of eating disorders and patients are often admitted to inpatient programs under pressure from family and doctors. In this article, we report research on the positive or negative impact of involuntary admission in the treatment of eating disorders, its application and effectiveness as well as the adverse consequences of c

Cannibalism in paranoid schizophrenia: a case report.

In ancient times, cannibalism was deemed acceptable for reasons of survival when faced with extreme need or for religious or magic reasons. In today's society, these cases are extremely rare and are generally associated with severe mental illness. We present a case of cannibalism in an individual with schizophrenia. The patient who lacked insight into his illness admitted to committing acts of cannibalism and offered in-depth explanations of the procedures used to prepare the bodies. He made statements on television and to the press and was given the nickname of "the people-eater", an identity
