Update on diagnostic and treatment of uncomplicated and complicated malaria in adults and selected vulnerable populations.

Although malaria remains one of the most important infectious causes of morbidity and mortality world-wide with 40% of the global population at risk, significant progress has been made toward elimination, notably with the development and use of rapid diagnostic tests, insecticide-treated bed nets, indoor residual spraying, and artemisinin-based combination therapies (ACTs). P. falciparum infection remains the most common cause of severe infection and death, but non-P. P. falciparum infections, including the recently emerged 5(th) plasmodium species, P. knowlesi, are increasingly recognized as

Evidence for diversifying selection on erythrocyte-binding antigens of Plasmodium falciparum and P. vivax.

Malaria parasite antigens involved in erythrocyte invasion are primary vaccine candidates. The erythrocyte-binding antigen 175K (EBA-175) of Plasmodium falciparum binds to glycophorin A on the human erythrocyte surface via an N-terminal cysteine-rich region (termed region II) and is a target of antibody responses. A survey of polymorphism in a malaria-endemic population shows that nucleotide alleles in eba-175 region II occur at more intermediate frequencies than expected under neutrality, but polymorphisms in the homologous domains of two closely related genes, eba-140 (encoding a second eryt

Larval habitat preferences of Anopheles dirus and Anopheles maculatus in North Sumatra, Indonesia.

BACKGROUND: While human malaria transmission in Indonesia has declined, reported cases of zoonotic Plasmodium knowlesi are increasing. This study examined the larval habitats of malaria vectors in North Sumatra to assess the potential of larval source management as a control strategy for both human and zoonotic malaria vectors. METHODS: Multiple larval habitat surveys in the areas surrounding two dusuns with documented human and zoonotic (P. knowlesi) malaria cases in Langkat Regency, North Sumatra, were conducted over 2 years, encompassing both wet and dry seasons. Larval habitats were chara

Current Status and the Epidemiology of Malaria in the Middle East Region and Beyond.

Vector-borne parasitic infectious diseases are important causes of morbidity and mortality globally. Malaria is one of the most common vector-borne parasitic infection and is caused by five Plasmodium species, namely P. falciparum, P. vivax, P. ovale, P. malariae, and P. knowlesi. Epidemiologically, differences in the patterns of malaria cases, causative agent, disease severity, antimicrobial resistance, and mortality exist across diverse geographical regions. The world witnessed 229 million malaria cases which resulted in 409,000 deaths in 2019 alone. Although malaria cases are reported from

Comparative evaluation of methods of malaria parasite density determination in blood samples from patients & experimental animals.

Three methods for the quantitation of parasitaemia in malaria were compared with the standard method for ascertaining the accuracy in patients, Plasmodium berghei infected mice and P. knowlesi infected Rhesus monkeys. Technique I, where parasitaemia was calculated from the number of PRBCs in 10,000 RBCs in thin blood film and the total RBC count of the host, was used as the standard. Technique II, where parasitaemia was calculated based on the number of PRBCs per WBC and average total WBC count (8000/microliter), was least accurate. Technique IV, where parasitaemia was calculated from the numb

Observations on early and late post-sporozoite tissue stages in primate malaria. III. Further attempts to find early forms and to correlate hypnozoites with growing exo-erythrocytic schizonts and parasitaemic relapses in Plasmodium cynomolgi bastianellii infections.

Rhesus monkeys were heavily infected with sporozoites of Plasmodium cynomolgi bastianellii in an attempt to demonstrate the site of invasion of sporozoites into tissue cells and their growth there. Further attempts were made to correlate the appearance and loss of hypnozoites with parasitaemic relapses. Hypnozoites were demonstrated and once again shown to decrease in numbers over 229 days during which time the infection showed parasitaemic relapses. Liver biopsies taken at two-day intervals for 12 days showed that hypnozoites decreased in numbers over-all and growing schizonts were demonstrat

The position of mefloquine as a 21st century malaria chemoprophylaxis.

BACKGROUND: Malaria chemoprophylaxis prevents the occurrence of the symptoms of malaria. Travellers to high-risk Plasmodium falciparum endemic areas need an effective chemoprophylaxis. METHODS: A literature search to update the status of mefloquine as a malaria chemoprophylaxis. RESULTS: Except for clearly defined regions with multi-drug resistance, mefloquine is effective against the blood stages of all human malaria species, including the recently recognized fifth species, Plasmodium knowlesi. New data were found in the literature on the tolerability of mefloquine and the use of this medic

Prevalence and genetic diversity of simian malaria in wild macaque populations across Thailand: Implications for human health.

Over the past year, P. falciparum infections have declined in Thailand, yet nonhuman primate malaria infections have correspondingly increased, including Plasmodium knowlesi and P. cynomolgi. Nevertheless, little is known about simian malaria in its natural macaque hosts, Macaca mulatta and Macaca fascicularis. This study aims to address several research questions, including the prevalence and distribution of simian malaria in these two Thai wild macaque species, variations in infection between different macaque species and between M. fascicularis subspecies, and the genetic composition of the

In-depth comparative analysis of malaria parasite genomes reveals protein-coding genes linked to human disease in Plasmodium falciparum genome.

BACKGROUND: Plasmodium falciparum is the most virulent malaria parasite capable of parasitizing human erythrocytes. The identification of genes related to this capability can enhance our understanding of the molecular mechanisms underlying human malaria and lead to the development of new therapeutic strategies for malaria control. With the availability of several malaria parasite genome sequences, performing computational analysis is now a practical strategy to identify genes contributing to this disease. RESULTS: Here, we developed and used a virtual genome method to assign 33,314 genes from

Conformational variability in the D2 loop of Plasmodium Apical Membrane antigen 1.

Apical Membrane Antigen 1 (AMA1) plays a vital role in the invasion of the host erythrocyte by the malaria parasite, Plasmodium. It is thus an important target for vaccine and anti-malaria therapeutic strategies that block the invasion process. AMA1, present on the surface of the parasite, interacts with RON2, a component of the parasite's rhoptry neck (RON) protein complex, which is transferred to the erythrocyte membrane during invasion. The D2 loop of AMA1 plays an essential role in invasion as it partially covers the RON2-binding site and must therefore be displaced for invasion to proceed

How Many Is Enough? - Challenges of Multinucleated Cell Division in Malaria Parasites.

Regulating the number of progeny generated by replicative cell cycles is critical for any organism to best adapt to its environment. Classically, the decision whether to divide further is made after cell division is completed by cytokinesis and can be triggered by intrinsic or extrinsic factors. Contrarily, cell cycles of some species, such as the malaria-causing parasites, go through multinucleated cell stages. Hence, their number of progeny is determined prior to the completion of cell division. This should fundamentally affect how the process is regulated and raises questions about advantag

Use of non-human primate hepatocytes for in vitro study of the pre-erythrocytic stages of malaria parasites.

Methods were developed that allow invasion of sporozoites from simian malaria parasite species (Plasmodium cynomolgi, P. knowlesi, P. coatneyi, P. inui, P. gonderi, P. fragile) and development to schizont stages in rhesus and Saimiri monkey hepatocytes. The P. cynomolgi-rhesus monkey model was used to study inhibition of schizont development using monoclonal antibodies (MAbs) produced against the circumsporozoite (CS) protein of various strains and species of malaria parasites. Immunoelectron microscopy, using gold-labelled MAbs and cultured parasites, demonstrated that the CS protein persists

Challenges in the diagnosis and treatment of malaria in Polish workers returning from Africa: a case series and review of literature.

Malaria is a parasitic disease caused in humans by five species of Plasmodium: P. falciparum, P. vivax, P. malariae, P. ovale, and P. knowlesi and transmitted through a female mosquito bite. In 2020, there were 241 million cases of malaria worldwide including 627,000 deaths. Traveling to malaria endemic areas is a significant risk factor, therefore, it is very important to use non-specific and pharmacological prophylaxis. Malaria symptoms usually appear 10-14 days after infection and the disease may be suspected, based on patient examination and medical history, in patients with fever who have

Structure-based design of novel small-molecule inhibitors of Plasmodium falciparum.

Malaria is endemic in most developing countries, with nearly 500 million cases estimated to occur each year. The need to design a new generation of antimalarial drugs that can combat the most drug-resistant forms of the malarial parasite is well recognized. In this study, we wanted to develop inhibitors of key proteins that form the invasion machinery of the malarial parasite. A critical feature of host-cell invasion by apicomplexan parasites is the interaction between the carboxy terminal tail of myosin A (MyoA) and the myosin tail interacting protein (MTIP). Using the cocrystal structure of

Antigenic variation of bloodstage malaria parasites.

An antigen on the surface of erythrocytes infected with mature asexual malaria parasites has been shown to undergo antigenic variation in two malaria species. Plasmodium falciparum-infected erythrocytes from squirrel monkeys express a new antigen that is identified by reactivity with antibody from infected animals in an indirect immunofluorescence assay. Cloned P. knowlesi parasites in rhesus monkeys undergo antigenic variation of an erythrocyte surface antigen as defined by antibody-mediated cell agglutination (the SICA test) and indirect immunofluorescence. This variant antigen is a malarial

Properties of red cell ghost preparations susceptible to invasion by malaria parasites.

Experimental procedures are described that lead to the formation of sealed red cell ghosts capable of efficient invasion by malaria parasites. Both human and monkey cells have been studied with respect to invasion by Plasmodium falciparum and P. knowlesi respectively. Resealed ghosts containing about half the normal concentration of haemoglobin are prepared by osmotic lysis and resealing at a haematocrit of 70%. When examined in the scanning electron microscope, populations of these ghosts contain few echinocytes, but have an abundance of stomatocytic forms. When undiluted haemolysate is subst

Antigenic variation in African trypanosomiasis: a memorandum.

After reviewing the present knowledge on antigenic variation of the trypanosomes of the Trypanosoma (Trypanozoon) brucei species, this Memorandum discusses the relevance of this phenomenon to the possible development of new tools for trypanosomiasis control. As antigenic variation is related to protective immunity and immunopathology, it is of crucial importance for the feasibility of vaccine development and for treatment principles. It is also of interest as a model for understanding antigenic variation occurring during infections with Plasmodium knowlesi, Babesia, and others. Recent methods w

Evolution of the merozoite surface protein 7 (msp7) family in Plasmodium vivax and P. falciparum: A comparative approach.

Malaria parasites (genus Plasmodium) are a diverse group found in many species of vertebrate hosts. These parasites invade red blood cells in a complex process comprising several proteins, many encoded by multigene families, one of which is merozoite surface protein 7 (msp7). In the case of Plasmodium vivax, the most geographically widespread human-infecting species, differences in the number of paralogs within multigene families have been previously explained, at least in part, as potential adaptations to the human host. To explore this in msp7, we studied its orthologs in closely related non
