Acute pancreatitis is a common condition, only occasionally leading to necrosis of the pancreas. In instances where abscess formation takes place, the predominant microbial profile involves both aerobic and anaerobic enteric species. We present the case of a patient with clostridial emphysematous pancreatitis who developed pneumoperitoneum without associated visceral perforation.

An 1180-g infant with esophageal atresia and tracheoesophageal fistula developed life-threatening respiratory distress syndrome. Conventional mechanical ventilation resulted in gastric perforation and pneumoperitoneum. High-frequency ventilation stabilized the infant, permitting distal occlusion of the esophagus with a Silastic band. Fistula ligation was subsequently performed under more optimal physiologic conditions.

Since the complications herewith presented as a consequence of surgical renal biopsy are really exceptional and references found in the existing literature rare, we have considered that the contribution of these two cases which developed a pneumomediastinum-pneumoperitoneum and pneumocele prespectively during the immediate post-surgical period would be of interest. Evolution was favourable in both cases, with spontaneous resolution of the process.

PURPOSE: Spontaneous colon perforations are usually encountered as necrotizing enterocolitis in the neonatal period, but occur rarely in infants and children without pathological conditions. This study was conducted to describe its clinical implication beyond the neonatal period.

An extramucosal pylorotomy was performed in infants with hypertrophic pyloric stenosis made possible by laparoscopic progress. After performing six cases by laparoscopy, the authors describe what precautionary measures must be taken for the pneumoperitoneum, the instruments utilized, and the advantages of this technique. It appears very likely that laparoscopic pyloromyotomies will become a widespread practice in the future.

Pneumatosis cystoides intestinalis is a rare condition characterized by the presence of gas-filled cysts in the bowel wall. A case is reported which was associated with peptic ulcer and in which there was involvement of the ileum and pre-pyloric region of the stomach. Stomach involvement is a rare occurrence. Chronic pneumoperitoneum was also present. The mechanism of its occurrence is discussed.

Six neonates receiving assisted ventilation developed loculated posterior mediastinal gas collections. Relation to other mediastinal structures localizes these gas collections to the infraazygous region, and the midline location distinguishes infraazygous pneumomediastinum from air within the pulmonary ligaments. Infraazygous pneumomediastinum does not communicate freely with the anterior mediastinum, but often dissects cephalad and is frequently associated with pneumoperitoneum.

The small volume of the infant abdomen limits the application of laparoscopic procedures. We successfully repaired a duodenal atresia in a 2-kg female infant using a standard diamond-shaped anastomosis and intracorporeal suturing and knot-tying techniques. Anesthesia and positive pressure ventilation assured adequate gas exchange during pneumoperitoneum during the procedure.

Plain abdominal radiographs are rarely diagnostic when the patient presents with acute abdominal pain. Emergency physicians, therefore, should be aware of the appropriate indications and limitations of abdominal films in this setting and should be skilled in their interpretation to exclude the rare cases of pneumoperitoneum, pneumobilia, hepatic-portovenous gas, small and large bowel obstruction, toxic megacolon, volvulus and intramural gas.

A 28-year-old, healthy pregnant patient developed bilateral pneumothorax, subcutaneous emphysema, pneumomediastinum, pneumoretroperitoneum and pneumoperitoneum following endotracheal intubation and manual ventilation during general anesthesia for breech extraction. It is likely that positive-pressure ventilation was the cause for this very rare combination of complications. Early recognition and treatment may prevent such a catastrophe.
