Understanding violence as a health determinant among ethnic communities in Colombia's Pacific region: a qualitative analysis.

This study sought to understand the manifestations of violence affecting Indigenous and Afro-descendant communities in the Nariño Pacific region, Colombia, through the framework of the social determinants of health. A qualitative research design was implemented with 60 purposively selected participants, ensuring variation by ethnicity (49 Afro-descendant and 11 Indigenous), gender (34 women and 26 men), and role (institutional and community). All participants were adults affected by the armed conflict and registered in the National Victims Registry (RUV). Data were collected through in-depth i

Reexamining the use of race in medical algorithms: the maternal health calculator debate.

The concept of race is prevalent in medical, nursing, and public health literature. Clinicians often incorporate race into diagnostics, prognostic tools, and treatment guidelines. An example is the recently heavily debated use of race and ethnicity in the Vaginal Birth After Cesarean (VBAC) calculator. In this case, the critics argued that the use of race in this calculator implied that race confers immutable characteristics that affect the ability of women to give birth vaginally after a c-section. This debate is co-occurring as research continues to highlight the racial disparities in health

The risk associated with organophosphorus nerve agents: from their discovery to their unavoidable threat, current medical countermeasures and perspectives.

The first organophosphorus nerve agent was discovered accidently during the development of pesticides, shortly after the first use of chemical weapons (chlorine, phosgene) on the battlefield during World War I. Despite the Chemical Weapons Convention banning these substances, they have still been employed in wars, terrorist attacks or political assassinations. Characterised by their high lethality, they target the nervous system by inhibiting the acetylcholinesterase (AChE) enzyme, preventing neurotransmission, which, if not treated rapidly, inevitably leads to serious injury or the death of t

(Not) wanting to choose: outside agencies at work in assisted reproductive technology.

Human choice and interventions that could seem to threaten the course of 'nature' or 'chance' are at the heart of controversies over assisted reproductive technology across Western countries. These debates focus predominately on so-called 'selective reproductive technology'. While today, the technique of in-vitro fertilization (IVF) raises few political and bioethical debates in France and other Western countries, concerns remain that human intervention might replace 'natural' processes, threatening human procreation. These polemics focus on situations that require a decision, notably embryo s

The consequences of chronic cannabis smoking in vulnerable adolescents.

Cannabis, like the mythic shape-shifter, presents in various guises, morphing with the perspective and context of the observer. Arguments about cannabis are confused by a myriad of debates-medical, social, ethical and political-as if a single conceptual umbrella can capture the variety and granularity of marijuana-related issues. This paper responds to marijuana use as it is commonly practised by youth in Australia. It has little to say about synthetic cannabinoids, specific medicinal cannabinoids, or medicinal properties of marijuana. We address those adolescents genetically and environmental

Globalization of Craniofacial Plastic Surgery: Foreign Mission Programs for Cleft Lip and Palate.

International Humanitarian Interchanges are a bona fide component of surgery and medicine. Additionally, these programs also provide substantial benefit both to the doers and the recipients. The foreign mission program is potentially a weapon of foreign policy which is underutilized and underestimated.Physician job dissatisfaction is increasing. However, the happiness and satisfaction of the participants in the short-term multidisciplinary trips, repeated, well-organized and respectful, with rather complete integration of the surgical system of the sister countries ("Plan B"), approaches 100%.T

Deadly public policy: what the future could hold for the HIV epidemic among injection drug users in Vancouver.

The scope and scale of the HIV outbreak that occurred among injection drug users in Vancouver in the late 1990s was unprecedented and resulted in some 2,000 new HIV infections, with incidence rates reaching 18 per 100 person-years. This outbreak, localized mainly in one neighbourhood, cost the Canadian health care system more than 1 billion dollars to diagnose, care and treat. A number of factors combined to stabilize HIV incidence: 1) HIV prevalence became saturated among those at highest risk; 2) several public health policies focused on drug users were implemented, including increased and a

Delta 9-tetrahydrocannabinol induces dopamine release in the human striatum.

The influence of cannabis on mental health receives growing scientific and political attention. An increasing demand for treatment of cannabis dependence has refueled the discussion about the addictive potential of cannabis. A key feature of all addictive drugs is the ability to increase synaptic dopamine levels in the striatum, a mechanism involved in their rewarding and motivating effects. However, it is currently unknown if cannabis can stimulate striatal dopamine neurotransmission in humans. Here we show that Delta 9-tetrahydrocannabinol (THC), the main psychoactive component in cannabis,

Lifestyle strategies for weight control: experience from the Finnish Diabetes Prevention Study.

Currently, in many European countries more than half the adult population is overweight; it hass become 'abnormal' to be of 'normal weight'. The risk of type 2 diabetes, CVD, hypertension and certain forms of cancer increase with increasing weight. Biological evolution has produced body-fat-regulating mechanisms that are more powerful in protecting against weight loss than against weight gain. The current environment offers constant availability of affordable palatable energy-rich foods, with no need to consume the energy through physical activity. The 'obesogenic' environment is to some exten

Polio eradication: mobilizing and managing the human resources.

Between 1988 and 2004, the Global Polio Eradication Initiative grew to become the largest international health effort in history, operating in every country of the world. An estimated 10 million health workers and volunteers have been engaged in implementing the necessary polio supplementary immunization activities (SIAs) on a recurring basis, and at least 35,000 well-trained workers have been conducting polio surveillance. A combination of task simplification, technological innovations and adaptation of strategies to fit local circumstances has allowed the Initiative to use a wide range of wo

'They take advantage of, and force themselves on us': a qualitative analysis of Haitian women's perceptions of sexual exploitation and abuse by MINUSTAH.

The United Nations Stabilization Mission in Haiti (MINUSTAH), established in 2004, was tasked with maintaining peace and stability in the aftermath of political upheaval in Haiti. However, it faced numerous allegations of peacekeeper-perpetrated sexual exploitation and abuse (PP-SEA) during its deployment, drawing significant international attention and criticism. This qualitative study conducted eight focus group discussions with Haitian women who have been affected by or had knowledge of PP-SEA. Through rigorous thematic analysis, this research uncovers narratives that illuminate how Haitian

Diamonds and viruses: Reconstructing the epidemiological origins of HIV-1 in Angola's mining frontier.

In 1988, the remote mining town of Dundo, in Lunda-Norte, Angola, displayed an anomalously high HIV-1 prevalence of 14.2%, despite the absence of recognised epidemic drivers. Archival and epidemiological evidence suggests that colonial medical interventions, rather than sexual transmission alone, were central to the early amplification of the virus. Under the Companhia de Diamantes de Angola (Diamang), a vast corporate health system promoted extensive use of injections and blood transfusions, relying on reusable equipment and limited sterilization. Quantitative data on transfusion ratios and i

Gender-Affirming Bilateral Orchiectomy for the Urogynecologist.

INTRODUCTION AND HYPOTHESIS: Access to gender-affirming care has improved for transgender individuals, yet significant barriers to surgical interventions persist. Bilateral orchiectomy is a straightforward, same-day procedure that plays a crucial role in gender affirming care. This article and accompanying video outline the indications for this procedure and provide a step-by-step demonstration of the surgical technique. METHODS: We describe the surgical steps of gender-affirming bilateral orchiectomy, highlighting its similarities to vaginal gynecologic procedures. We also discuss indication

Scaling-up symptom-agnostic, community-wide screening toward global tuberculosis elimination: opportunities, challenges, and lessons from history.

There has been little change in global tuberculosis (TB) incidence in the 21st century. Although case notification has increased, millions of people with TB each year remain unreached. Recently there has been increased recognition that many people with undiagnosed, potentially infectious TB do not experience or report TB symptoms. Symptom-agnostic screening (e.g., by chest X-ray) can effectively identify such forms of TB. Although this activity is increasing globally and is beneficial to individuals screened, current levels fall far short of what is needed to impact transmission and population

Adapting the informed push model to the last mile of the contraceptive supply chain in South Kivu in the Democratic Republic of Congo.

In the Democratic Republic of Congo (DRC), contraceptive security is one of the largest barriers to improving access to family planning. This article presents findings from a pilot study that adapted an informed push distribution model to the last mile of the contraceptive supply chain, between the health zone pharmacy and health facilities, in the eastern region of DRC. The intervention consisted of three changes in the supply chain: organization of more efficient transportation routes, in-depth involvement of the community in supply chain management and bundling of contraceptives with other

[Controversies between mental health and disability standards in Mexico].

There are controversies between the practice of psychiatry, current international standards and mental disorders or conditions with disabilities, framed in social care models. The objective of this work is to provide evidence and analyze the main gaps in mental health such as: the invisibility of some people with disabilities for the design of policies, legislation, or public programs; the predominance of the medical model, in which the substitution of decision-making in informed consent prevails, which violates the rights of legal personality, equality, freedom, security and respect for perso

"Consumed by creed": Obsessive-compulsive symptoms underpin ideological obsession and support for political violence.

Radicalization is a process by which individuals are introduced to an ideological belief system that encourages political, religious, or social change through the use of violence. Here we formulate an obsessive-compulsive disorder (OCD) model of radicalization that links obsessive passion (OP; one of the best predictors of radical intentions) to a larger body of clinical research. The model's central tenet is that individual differences in OCD symptom severity could shape radical intentions via their influence on OP. Across four ideological samples in the United States (Environmental activists

Neoliberalism, Climate Change, and Displaced and Homeless Populations: Exploring Interactions Through Case Studies.

There is a growing attention to neoliberal policies and practices as they relate to climate change and housing within academic literature. However, the combined effects of neoliberal political and economic decisions on the interaction between climate change and displaced and homeless populations have not been substantially explored. In this paper, we identify and focus on three key re-emerging themes prevalent within neoliberal discourses: economic considerations, individualization, and short-termism. To examine the intersecting influence of climate change and these themes on vulnerable popula

[Canary in the Coal Mine: Reflections on the Current Global Wave of Nursing Strikes].

A worldwide wave of nurse strikes has erupted. Labor strikes represent a last resort measure and are a powerful means for nurses to fight back against adverse working conditions such as heavy workloads, unfair pay, and unsafe staffing conditions that not only affect patient care outcomes but also economic growth and social political stability. Despite the pandemic placing most nurses under extremely stressful conditions for an extended period of time, nurses remained on duty to cope with the changing circumstances while continuing to provide high-quality care. Overwhelmed nurses worldwide are

Historical Highlights on Electroconvulsive Therapy Practice in Iraq: A Brief Narrative.

Electroconvulsive therapy (ECT) is an established treatment option for severe psychiatric disorders. Electroconvulsive therapy practice began in Iraq in 1946, primarily to treat patients with schizophrenia. Since then, it has developed and expanded to treat mood disorders and major depression. Initially, both modified and unmodified ECT was administered to patients in Ibn Rushd and Al Rashad mental hospitals in Baghdad. The ongoing political turmoil from 3 wars, economic sanctions, and internal conflicts halted the development of mental health services across the country; ECT provision was not
