Fluoxetine inhibits K(+) transport pathways (K(+) efflux, Na(+)-K(+)-2Cl(-) cotransport, and Na(+) pump) underlying volume regulation in corneal endothelial cells.

We have studied regulatory volume responses of cultured bovine corneal endothelial cells (CBCEC) using light scattering. We assessed the contributions of fluoxetine (Prozac) and bumetanide-sensitive membrane ion transport pathways to such responses by determining K(+) efflux and influx. Cells swollen by a 20% hypo-osmotic solution underwent a regulatory volume decrease (RVD) response, which after 6 min restored relative cell volume by 98%. Fluoxetine inhibited RVD recovery; 20 microM by 26%, and 50 microM totally. Fluoxetine had a triphasic effect on K(+) efflux; from 20 to 100 microM it inhib

Involvement of Na(+)-K(+)-2Cl(-) cotransporters in hypertonicity-induced rise in intracellular calcium concentration.

A hypertonic saline containing propylene glycol facilitates calcium (Ca(2+)) influx through voltage-dependent Ca(2+) channels. The present study performed experiments to elucidate the mechanism by which Na(+)-K(+)-2Cl(-) cotransporters participate in the rise in the intracellular calcium concentration ([Ca(2+)]i) under the hypertonic condition. Both furosemide and ethacryonic acid significantly decreased the [Ca(2+)]i raised by hypertonicity. Similarly, Na(+)-, K(+)-, or Cl(-)-free saline also reduced it. Both norepinephrine and dopamine significantly enhanced the rise in [Ca(2+)]i. In conclus

An unusual cause of thyrotoxic periodic paralysis: triiodothyronine-containing weight reducing agents.

Thyrotoxic periodic paralysis resulting from exogenous thyroid hormone administration (thyrotoxicosis factitia) has been rarely reported. We … blood acid-base status, suggesting acute potassium shifting into cells. Approximately, 174 mmol of potassium chloride was administrated to restore his muscle strength within 12 hours, but rebound hyperkalemia (6.1 mmol/L) developed upon recovery. Despite normal serum free triiodothyronine and thyroxine levels, the suppressed concentration of thyroid stimulating hor …

Targeting ion channel networks in diabetic kidney disease: from molecular crosstalk to precision therapeutics and clinical innovation.

Diabetic kidney disease (DKD), a major microvascular complication of diabetes, is closely associated with functional imbalances in ion channels regulating sodium (Na+), calcium (Ca2+), potassium (K+), and chloride (Cl-). This review systematically examines the roles of ion channels in glomerular filtration barrier dysfunction, tubular reabsorption, and fibrotic processes in DKD, with emphasis on the pathological relevance of sodium-glucose cotransporter 2 (SGLT2), epithelial sodium channels (ENaC), transient receptor potential (TRP) channels, chloride channels, aquaporins (AQPs), and PIEZO cha

Membrane potentials in Rana temporaria muscle fibres in strongly hypertonic solutions.

Conventional microelectrode methods were used to measure variations in resting membrane potentials, E(m), of intact amphibian skeletal muscle fibres over a wide range of increased extracellular tonicities produced by inclusion of varying extracellular concentrations of sucrose. Moderate increases in extracellular tonicity to up to 2.6x normal (2.6tau) under Cl(-) free conditions produced negative shifts in E(m) that followed expectations for the K(+) Nernst equation (E(K)) applied to a perfect osmometer containing a conserved intracellular K(+) content despite any accompanying cell volume chan

DNA twist at high alkali ion concentrations: evidence against C-form DNA in solution.

DNA is highly negatively charged, making its structure strongly dependent on the ionic environment. DNA twist-a central DNA property-varies with ion concentration and identity. Prior studies have focused on salt concentrations below 1 M, and it is unclear whether twist trends persist at higher concentrations. It has been proposed that at high salt, DNA transitions from its canonical B-form to C-form, originally observed by fiber diffraction. Here, we use single-molecule magnetic tweezers to measure DNA twist in high concentrations of LiCl, NaCl, KCl, and CsCl. For all salts, twist initially in

Allergic contact dermatitis to polyethylene glycol and nitrofurazone.

We report a case of worsening dermatitis after the application of an antibiotic ointment (Furacin) containing furazone and polyethylene glycol. Patch tests to nickel sulfate, potassium dischromate, chloride cobalt, Furacin, nitrofurazone 1% petrolatum, polyethylene glycol mix 4% petrolatum, polyethylene glycol (PEG) 300 4% petrolatum, and PEG 400 as is (ai) were positive. The use of topical agents containing nitrofurazone or polyethylene glycol on damaged skin may predispose to contact allergy. We discuss the sensitizing properties of nitrofurazone and polyethylene glycol.

Local medical treatment of interstitial pregnancy after in-vitro fertilization and embryo transfer (IVF-ET): two case reports.

Two cases of interstitial pregnancy (one singleton, one heterotopic twins) were confirmed 31 and 34 days after in-vitro fertilization. Serum human chorionic gonadotrophin concentrations were 35,000 and 86,000 mlU/ml, respectively. They were treated conservatively with transvaginal ultrasonically guided intra-ovular injection of either methotrexate in the singleton pregnancy, or potassium chloride into the ectopic sac of the heterotopic twins. No complications were observed. The intra-uterine pregnancy continued and the patient delivered a 3350 g healthy baby at 39 weeks.

Magnesium deficiency following jejunoileal bypass operations for obesity.

Skeletal muscle biopsies, blood samples, and 24-hour urines, before and after magnesium infusions, were obtained from 12 patients who had undergone jejuno-ileal bypass surgery several years earlier, selected for probable magnesium deficiency on the basis of repeated hypomagnesemia. The patients retained significant amounts of the infused magnesium, and exhibited elevation of low skeletal muscle magnesium and potassium, with concomitant decreases of muscle sodium and chloride. These changes were accompanied by increased urinary calcium and sodium and decreased urinary phosphorus excretion.

THE ACID-BASE COMPOSITION OF GASTRIC SECRETIONS.

The chief inorganic factors in secretions obtained from isolated pouches constructed in the fundus and in the pyloric antrum of the cat's stomach were found to be chloride ion and fixed base. In a series of samples obtained from the fundic pouch, chloride ion was approximately stationary at 165 cc. 0.1 N per 100 cc. During digestion of food in the stomach, secretions from the pouch contained fixed base in amounts varying considerably from an average of 47 cc. 0.1 N per 100 cc. Material allowed to remain in the pouch after the completion of food digestion in the stomach showed an increasing con

Experimental patch testing with chromium-coated materials.

BACKGROUND: Chromium coatings on metal alloys can be decorative, and prevent corrosion and metal ion release. We recently showed that handling of a chromium-containing disc resulted in chromium deposition on the skin. OBJECTIVES: To examine patch test reactivity to chromium-coated discs. METHODS: We included 15 patients: 10 chromium-allergic patients, and 5 patients without chromium allergy. All were patch tested with potassium dichromate, cobalt chloride, nickel sulfate, and nine different metallic discs. The chromium-allergic patients were also patch tested with serial dilutions of potassi

Neonatal myocardial oxygen consumption during ventricular fibrillation, hypothermia, and potassium arrest.

BACKGROUND: Many investigators have examined oxygen consumption in adult heats under conditions that simulate those encountered during cardiac operations and those that approximate basal metabolism. Few studies, however, have addressed this issue in neonatal myocardium. METHODS: Hearts from 3- to 9-day-old piglets were studied in a blood-perfused isolated heart preparation in working, empty beating, fibrillating, potassium chloride-arrested (at 37 degree C and 15 degree C), and hypothermic (15 degree C) states. RESULTS: Oxygen consumption (expressed in milliliters of O2 per 100 g of ventricu…

Electrocardiographic QT prolongation and sudden death in renal hypokalemic alkalosis.

A 3-year-old boy was found to have a mixed tubulopathy with hypokalemia (1.9 mmol/l), alkalosis (blood pH 7.51, plasma carbon dioxide pressure 46 mm Hg, plasma bicarbonate 35.7 mmol/l) and hypophosphatemia (1.21 mmol/l). An electrocardiogram disclosed a prolonged heart rate corrected QT interval of 490 ms. The boy was put on potassium chloride, phosphate and nonsteroidal anti-inflammatory agents. With this treatment plasma phosphate normalized and plasma potassium increased up to 3.0-3.3 mmol/l. Three years later the child, who did not have history of gastroesophageal reflux or epileptic convu…

Effects of indapamide versus hydrochlorothiazide on plasma lipids and lipoproteins in hypertensive patients: a direct comparison.

To test the claim that indapamide causes fewer adverse metabolic effects than hydrochlorothiazide (HCTZ), six months of treatment with HCTZ 25 mg/day was compared with six months of treatment with indapamide 2.5 mg in a randomized double-blind study of 44 patients with mild to moderate hypertension. Both drugs significantly reduced blood pressure, with no significant differences in the reduction of potassium and chloride; neither drug was associated with a significant change in plasma total cholesterol, high density lipoprotein (HDL), apolipoprotein A1, apolipoprotein B or the ratio of total c

Sudden deafness in a man with thyrotoxic hypokalemic periodic paralysis.

We treated a 32-year-old man with hearing loss in the right ear followed by severe muscle weakness of the extremities. When admitted to hospital by ambulance, the serum potassium level was 1.5 mEq/L. The muscle weakness was improved following administration of potassium chloride but the hearing impairment continued. Hearing level of the right ear was extremely impaired at the level of 60-105 dB, while hearing in the left ear was fairly normal. Spontaneous nystagmus was also present. The triiodothyronine level was 290 ng/dl, thyroxine 14.4 micrograms/dl, thyroid stimulating hormone (TSH) 0.84 m…

[Salt wasting syndrome caused by congenital, insufficient synthesis or aldosterone function--etiology, diagnosis and management].

Salt wasting syndrome is caused by a congenital or acquired synthesis disorder or by the aldosterone function disorder. It manifests itself by ionic disorders where the sodium and chlorine level decrease with the simultaneous potassium retention. Synthesised aldosterone is in the glomerular zone of the adrenal cortex. Symptoms of dyselectrolitemia are not distinctive, they develop within a few first days of life. The suction aversion, apathy, lack of growth or progressing, body mass loss is being noticed. The most often cause of salt wasting syndrome is the congenital cortical adrenal hyperpla

Delayed Diagnosis in a Male With Congenital Chloride Losing Diarrhea.

Congenital chloride diarrhea is a secretory type of diarrhea, inherited in as autosomal recessive. Our case involves a 12-month-old male who initially presented in infancy and was treated with an exclusive elemental formula diet. At 12 months of age, he presented with significant hypokalemia, hypochloremia, and metabolic alkalosis. The diagnosis was established with stool electrolytes demonstrating a stool chloride of 145 mmol/L. He initially was treated with sodium and potassium supplementation and a proton pump inhibitor. Genetic testing revealed a large 4.3-kb deletion encompassing exons 15

"Recurrent Papillary Necrosis and Nephrocalcinosis Induced by Nonsteroidal Anti-Inflammatory Drugs for Gouty Arthritis Associated with Congenital Chloride-Losing Diarrhea: A Major Risk for Kidney Loss".

Congenital chloride-losing diarrhea (CCLD) is a rare genetic disorder due to autosomal recessive mutation in the SLC26A3 gene on chromosome 7. It is characterized with chronic watery diarrhea with high fecal chloride (Cl: >90 mmol/L), low potassium (K), and metabolic alkalosis with low urinary Cl and K. The overall long-term prognosis is favorable with optimal life-long salt and K supplementation. In this case report, we describe a man with progressive renal failure and small kidneys that showed nephrocalcinosis and papillary necrosis. His disease was diagnosed since birth and was confirmed by
