Statins for extension of disability-free survival and primary prevention of cardiovascular events among older people: protocol for a randomised controlled trial in primary care (STAREE trial).

INTRODUCTION: The world is undergoing a demographic transition to an older population. Preventive healthcare has reduced the burden of chronic illness at younger ages but there is limited evidence that these advances can improve health at older ages. Statins are one class of drug with the potential to prevent or delay the onset of several causes of incapacity in older age, particularly major cardiovascular disease (CVD). This paper presents the protocol for the STAtins in Reducing Events in the Elderly (STAREE) trial, a randomised double-blind placebo-controlled trial examining the effects of

Immunomodulation for primary prevention of urinary tract infections in patients with spinal cord injury during primary rehabilitation: protocol for a randomized placebo-controlled pilot trial (UROVAXOM-pilot).

BACKGROUND: Urinary tract infections (UTIs) are common in individuals with neurogenic lower urinary tract dysfunction (NLUTD) following spinal cord injury (SCI). They are not only a great burden for affected individuals, but also cause considerable health costs. Furthermore, recurrent antibiotic treatments of UTIs contribute to the growing problem of bacterial resistance to antimicrobial compounds. Even though there is a multitude of different measures to prevent UTIs in individuals with NLUTD, no clear evidence exists for any of these. Oral immunomodulation with UTI-relevant Escherichia coli

Intra-Operative Ostial Irido-Zonulo-Hyaloido-Vitrectomy with Primary Posterior Capsulectomy for Prevention of Post-Operative Aqueous Misdirection in Combined Phaco-Trabeculectomy in Primary Angle Closure Glaucoma.

Purpose: The aim of this study is to investigate the ability of ostial irido-zonulo-hyaloido-vitrectomy (ostial IZHV) with primary posterior capsulectomy (PPC) in preventing aqueous misdirection (AM) in combined cataract and filtration surgery in a high-risk group of angle closure disease. Materials and Methods: Comparative case series-data collection of consecutive adult subjects (>18 years) with angle closure, undergoing phaco-trabeculectomy, when per-operatively anterior chamber depth (ACD) remained shallow, putting them at high risk for development of AM post-operatively. Study subjects we

[Effectiveness of a Multimodal Nursing Intervention for Primary Caregivers in Preventing Medical Adhesive-Related Skin Injury].

BACKGROUND: Although medical adhesive products are routinely used in clinical settings, research into both the medical-adhesive-associated skin outcomes of patients and the caregiving practices of patients and their primary caregivers remains limited. PURPOSE: The primary aim of this study was to evaluate the effectiveness of a multi-strategy nursing educational program in improving knowledge of medical adhesive-related skin injury (MARSI), health beliefs, self-efficacy, and skin care behaviors among primary caregivers of ostomy patients. In addition, peristomal transepidermal water loss (TEW

Multifocal primary hepatic epithelioid hemangioendothelioma: a case in which tumor biopsy was critical to prevent misdiagnosis.

Hepatic epithelioid hemangioendothelioma is rare, and because its imaging findings are nonspecific, pathological evaluation is essential for definitive diagnosis. In this 82‑year‑old woman, multiple hepatic masses were incidentally detected on contrast‑enhanced CT during postoperative surveillance for bladder cancer, and concurrent local recurrence made metastasis the leading suspicion. However, immunohistopathological examination of a liver biopsy excluded metastatic carcinoma and confirmed primary hepatic epithelioid hemangioendothelioma. She died of liver failure. This case underscores the

Effect of vaccination against HPV in the HPV-positive patients not covered by primary prevention on the disappearance of infection.

An undeniable risk factor for cervical cancer and intraepithelial neoplasia is persistent infection with HPV - types 16,18, 31, 45, 52 and others. Changes in sexual behavior may, in the coming decades, influence the epidemiology of HPV-related lesions. For those diseases, vaccination against HPV may be the only effective mean of prevention. Our study aims to show the results and changes in the smear for HPV DNA in patients after receiving a vaccination course with a 9-valent HPV vaccine in women with HPV infection. Out of 320 subjects with HPV- positive result 250 (78.1%) decided to be vaccina

[Primary isoniazid preventive therapy : A strategy still relevant in the era of test and treat ; literature review].

BACKGROUND: Tuberculosis remains a public health threat responsible as recently as 2018 for more than one million deaths. Chemoprophylaxis with isoniazid is one of the strategies implemented to control the disease. Although it is not yet widely prescribed, its utilization raises additional questions in the "test and treat" era of for anti-retroviral therapy. The objective of this study is to review the different randomized controlled trials of antitubercular Isoniazid Preventive Therapy (IPT). We have distinguished (a) "efficacy trials" (ET) comparing IPT to a placebo or the absence of chemopr

Health system barriers to implementation of TB preventive strategies in South African primary care facilities.

BACKGROUND: Isoniazid preventive therapy (IPT) is a key component of TB/HIV control, but few countries achieve high IPT coverage. METHODS: Using a behavioural COM-B design approach, the intervention consisted of a training on IPT guidelines and tuberculin skin testing (TST), identification of the optimal IPT implementation strategy by the health care workers (HCWs) of 3 primary care clinics, and a 2-month mentoring period. Using routine register data, TST and IPT uptake was determined 3 months before and 5 months after the intervention. Records were reviewed to identify factors associated wit

Effectiveness of offloading methods in preventing primary diabetic foot ulcers in adults with diabetes: a systematic review.

BACKGROUND: The incidence of foot ulceration related to diabetes is increasing. Many foot care professionals recommend offloading measures as part of management strategies for modulating excess pressure to prevent development of diabetic foot ulcers (DFUs). These measures may include padding, insoles/orthotic devices and footwear. There is a lack of evidence-based guidance on the effectiveness of the different offloading options for preventing primary ulceration in those with diabetes. OBJECTIVES: To identify, critically appraise and synthesize the best available evidence on methods of offloa

Analysis of Preventive Interventions for Malaria: Exploring Partial and Complete Protection and Total and Primary Intervention Effects.

Event dependence, the phenomenon in which future risk depends on past disease history, is not commonly accounted for in the statistical models used by malaria researchers. However, recently developed methods for the analysis of repeated events allow this to be done, while also accounting for heterogeneity in risk and nonsusceptible subgroups. Accounting for event dependence allows separation of the primary effect of an intervention from its total effect, which is composed of its primary effect on risk of disease and its secondary effect mediated by event dependence. To illustrate these methods

Preventing and managing aberrant drug-related behavior in primary care: systematic review of outcomes evidence.

Several strategies for preventing, identifying, and responding to aberrant opioid-related behaviors are recommended in pain management guidelines. This systematic review evaluated data supporting basic strategies for addressing aberrant opioid-related behaviors. Risk reduction strategies were identified via a review of available guidelines. Systematic literature searches of PubMed (May 1, 2007-January 18, 2013) identified articles with evidence relevant to nine basic strategies. Reference lists from relevant articles were reviewed for additional references of interest. Levels of evidence for a

Aspirin for prophylactic use in the primary prevention of cardiovascular disease and cancer: a systematic review and overview of reviews.

BACKGROUND: Prophylactic aspirin has been considered to be beneficial in reducing the risks of heart disease and cancer. However, potential benefits must be balanced against the possible harm from side effects, such as bleeding and gastrointestinal (GI) symptoms. It is particularly important to know the risk of side effects when aspirin is used as primary prevention--that is when used by people as yet free of, but at risk of developing, cardiovascular disease (CVD) or cancer. In this report we aim to identify and re-analyse randomised controlled trials (RCTs), systematic reviews and meta-analy…

The initial experience of safety and efficacy of argon plasma coagulation (APC) in the primary prevention of variceal bleeding.

BACKGROUND AND STUDY AIMS: The well-known complications of variceal bleeding together with the high mortality rate mandate effective prophylaxis. Because of the intolerance, failure of response and lack of compliance related to B blockers and because of the high incidence of variceal recurrence after endoscopic variceal ligation (EVL), other alternatives should be investigated. As APC provides coagulation at a shallow depth, it has been considered an ideal procedure to promote mucosal fibrosis for oesophageal varices. This study aims to investigate the safety and effectiveness of APC applicati

Primary omental torsion in an old woman: imaging techniques can prevent unnecessary surgical interventions.

Torsion and/or infarction of the greater omentum are rare but well-recognized clinical situations which present as an acute abdomen. The etiology is unknown and speculative. In most cases, the pathology is right sided and clinical presentation consists of an acute or subacute flank pain with mild peritonism usually evoking appendicitis or cholecystitis. Nevertheless, knowledge concerning these two problems can help the surgeon in proper diagnosis and treatment. Since the first report on primary torsion by Eitel in 1899, a few hundred more have been reported and some collective reviews publishe
