Evaluation and Pharmacologic Management of Paroxysmal Sympathetic Hyperactivity in Traumatic Brain Injury.

OBJECTIVE: Paroxysmal sympathetic hyperactivity (PSH) can occur in up to 10% of severe traumatic brain injury (TBI) patients and is associated with poorer outcomes. A consensus regarding management is lacking. We provide a practical guide on the multi-faceted clinical management of PSH, including pharmacological, procedural and non-pharmacological interventions. In addition to utilizing a standardized assessment tool, the use of medications to manage sympathetic and musculoskeletal manifestations (including pain) is highlighted. Recent studies investigating new approaches to clinical managemen

[Gasserian ganglion block for trigeminal neuralgia under dexmedetomidine sedation].

We report a case of successful procedural sedation using dexmedetomidine (DEX) in a 68-year-old woman undergoing left gasserian ganglion block for intractable trigeminal neuralgia. DEX was chosen to provide an effective sedation and clear communication about the injection of drugs or thermocoagulation. After 15 minutes of DEX administration at 0.8 microg x kg(-1) x hr(-1), nerve block needle insertion was bearable. The patient could feel a weakening of the sensation of her left maxilla and mandible after receiving a 0.4 ml mepivacaine test dose. High-frequency thermocoagulation elicited tolera

Emerging Treatment Options for Fibroids.

Leiomyomas (fibroids) are common, usually benign, monoclonal tumors that arise from the uterine myometrium. Clinical presentation is variable; some patients are asymptomatic, whereas others experience heavy menstrual bleeding, pain, bulk symptoms, and/or alterations in fertility. Previously, treatment options for fibroids were largely surgical. However, over the last decade, options have grown to include many medical and procedural options that allow for uterine and fertility preservation. Clinicians must become familiar with these options to adequately counsel patients desiring treatment of f

The American Society of Pain and Neuroscience (ASPN) Guidelines for Advanced Practice Providers in Interventional Spine and Pain Management Practice.

BackgroundChronic pain management has advanced with minimally invasive interventions that reduce surgical trauma and recovery time. Specialized interventional spine clinics rely on multidisciplinary approaches to improve outcomes. Advanced Practice Providers (APPs), including nurse practitioners, physician assistants, and clinical nurse specialists, play increasingly important roles in these settings. However, standardized guidance on APP integration, training, and scope of practice remains limited. This guideline, developed by an expert panel, is the first to address APP roles in intervention

Evaluation of needle to nerve interaction for neurological complications in ultrasound-guided interscalene brachial plexus blocks.

BACKGROUND: The use of regional anesthesia techniques carries a well-known risk of perioperative neurological complications. The interscalene approach to brachial plexus block (ISB) is associated with the largest risk of neurological complications of any peripheral nerve block. The reported incidence of nerve injury after ISB varies, with estimates ranging from 0.5 % to 3 % of cases. In this prospective study, we sought to correlate the needle-to-nerve interaction during ISB procedures with the development of post-operative neurological outcomes. METHODS: A novel grading system was developed

Methods and Timing of the Assessment of Post-Operative Pain in Minimally Invasive Hernia Surgery: A Scoping Review Comparing Robotic With Laparoscopic Repair.

BACKGROUND: Minimally invasive techniques, including laparoscopic and robotic-assisted surgeries, have revolutionized hernia repair by reducing recovery times and postoperative pain (1). Existing studies have explored general outcomes of robotic surgery, including shorter hospital stays and fewer complications (2). However, assessing and comparing postoperative pain in this context remains challenging due to inconsistencies in measurement methods, timing, and reporting practices across studies. METHODS: A scoping review was conducted on studies published from January 2014 to August 2024. Stud

Chronic Pain Syndromes: Complex Regional Pain Syndrome.

Complex regional pain syndrome (CRPS) is a chronic pain condition characterized by intense pain, usually in a body region that has experienced trauma, and autonomic and inflammatory features. It most commonly develops after an arm or leg injury. Patients typically present with extreme hyperalgesia and/or allodynia. The Budapest Criteria are used to make this clinical diagnosis. Prompt diagnosis and aggressive management are critical because long-term outcomes are improved when treatment is initiated soon after symptom onset. The primary management options are rehabilitation and physical therap

Expected and reported pain in children undergoing ear piercing: a randomized trial of preparation by parents.

This study examined experimentally the effectiveness of preparatory information provided by parents in creating accurate expectations and reducing children's procedural pain. Ear piercing was used as an analogue to minor painful medical procedures. Sixty children, aged 5-12 years, requesting ear piercing and accompanied by their parents, were randomly assigned to a parental information or contact-control condition. Parents in the information group were asked to read their child a description of the procedures and sensations of ear piercing. Parents in the contact-control condition played pictu…

Transvaginal hybrid-NOTES vs. traditional laparoscopic sigmoid resection for diverticulitis: a short-term comparative study.

The aim was to compare short-term results of transvaginal hybrid-NOTES (NSR) with traditional laparoscopic technique in sigmoid resection (LSR) in cases of diverticulitis. Natural Orifice Transluminal Endoscopic Surgery has been evolved as a minimally invasive procedure to reduce the operative trauma due to the absence of specimen extraction through the abdominal wall causing less postoperative pain, and shorter hospital stay. Despite the increasing use and published case series of NSR for diverticulitis as a laparoscopic procedure with transvaginal stapling and specimen extraction, there are
