BACKGROUND: With an aging global population, the incidence of revision total hip arthroplasty (rTHA) is expected to increase markedly. While patients undergoing primary total hip arthroplasty who require chronic anticoagulation (AC) have been associated with increased postoperative complications, less is known about the impact of chronic AC status on postoperative complications in the rTHA setting. This study sought to compare complication rates following aseptic rTHA between patients who were on chronic AC and those who were not.
METHODS: A large national database was utilized to retrospecti

BACKGROUND: Although periprosthetic joint infection (PJI) can affect multiple joints concurrently, the majority of patients with multiple prosthetic joints present with PJI of a single joint. Data regarding the optimal management of these patients are limited. We aimed to identify the prevalence, risk factors for a subsequent PJI, and clinical circumstances of PJI in patients with multiple prosthetic joints.
METHODS: We retrospectively reviewed the clinical records of 197 patients with ≥2 total joint prostheses in place who presented with PJI from 2000 to 2017. The average follow-up was 3.6 y

High operative mortality of infected thoracoabdominal aortic aneurysms (ITAA) is partly attributable to ischemic injury during aortic clamping. A case is presented of an 88-year old man who was admitted with imminent rupture of an ITAA. Axillobifemoral bypass grafting had been performed after removal of an infected abdominal aortic prosthesis six years earlier. In situ graft replacement was performed during 70 minutes of aortic clamping just below the pulmonary hilum without causing any but transient renal ischemic injury. Since the infrarenal aorta was absent after previous removal of an infe

Although robotic-assisted total knee arthroplasty (R-TKA) enhances the accuracy of component alignment, its relationship with postoperative infection risk remains uncertain. This study systematically compares infection rates following R-TKA versus manual TKA (M-TKA). A systematic search of four bibliographic databases was undertaken to retrieve comparative studies that evaluated postoperative infection rates in patients undergoing R‑TKA versus M‑TKA. Eligible studies were required to report at least one infection‑related endpoint, including those explicitly reporting zero infection events. In

INTRODUCTION: Cardiac implantable electronic device (CIED)-associated infections are a significant procedural complication. Antibacterial envelopes have emerged as a potential strategy to mitigate this risk, though the evidence regarding their prognostic benefit remains inconclusive.
OBJECTIVES: We performed a meta-analysis of randomized controlled trials (RCTs) and propensity score-matched cohorts evaluating the efficacy of the adjunctive use of antimicrobial envelopes in preventing CIED-related infections compared to standard prophylactic measures alone.
METHODS: We searched through PubMed

For infection-associated implant osteomyelitis (IAOM) caused by Staphylococcus aureus, copper-induced bacterial death holds great potential. However, inducing bacterial cuproptosis-like death using low concentrations of copper (Cu) ions remains a challenge. Therefore, a "herbal-piezoelectric heterojunction" (Herbal-Piezo-HJ) is designed by integrating berberine (Ber) with Cu ion-doped Strontium titanate (SrTiO3, STO) on a Ti substrate (STO-Cu@Ber). Ber and acoustoelectric therapy (SPT) synergistically enhance the efficacy of low-concentration Cu ions in inducing bacterial cuproptosis-like deat

INTRODUCTION: Prosthetic joint infections (PJIs) caused by mycobacteria are exceptionally rare and often underrecognized in clinical practice.
AREAS COVERED: Although most PJIs are due to common bacterial pathogens, Mycobacterium tuberculosis and nontuberculous mycobacteria (NTM) represent a unique subset with distinct microbiological and therapeutic challenges. The presence of biofilms further complicates their management, often requiring intensive pharmacological interventions and surgical strategies. This review analyzes over 300 reported cases of mycobacterial PJIs. The findings indicate

During maceration, a string of beads was found near the surgically modified right os coxa and femur of the remains of a 55-year-old woman donated to the Forensic Investigation Research Station at Colorado Mesa University. The remains were initially used in the study of decomposition of human remains, and then macerated to clean the skeleton. Documentation on this decedent noted three separate "hip replacement" surgeries, but no other details. The surgical modifications include significant alterations to the right os coxa and femur consistent with a Girdlestone procedure, generally used to trea

Cutibacterium acnes, part of normal skin flora, is increasingly recognized as an opportunistic pathogen capable of causing chronic prosthetic joint infections (PJI) associated with total hip and knee arthroplasty. However, there is a paucity of literature examining the pathogenesis of C. acnes during PJI. To study this, we developed an implant-associated osteomyelitis murine model in which 8-10-week-old C57BL6 mice were subjected to transtibial implantation of titanium or stainless-steel L-shaped pins contaminated with C. acnes. Postsurgery, mice were killed on Days 14 and 28 for terminal asse

Spinal implant infections are a serious complications of instrumented spinal fusion surgeries, carrying high morbidity and complex management challenges. Early postoperative infections may manifest with wound-healing issues, back pain, and fevers. Magnetic resonance imaging (MRI) is the preferred imaging modality, but can be limited by metal artifacts. For cases with stable implants, surgical debridement with implant retention combined with at least 12 weeks of antibiotics is currently considered appropriate treatment. Staphylococcal infections are ideally treated with biofilm-active antibioti

Effective treatment of orthopedic implant-associated infections (IAIs) remains a clinical challenge. The in vitro and in vivo studies presented herein evaluated the antimicrobial effects of applying cathodic voltage-controlled electrical stimulation (CVCES) to titanium implants inoculated with preformed bacterial biofilms of methicillin-resistant Staphylococcus aureus (MRSA). The in vitro studies showed that combining vancomycin therapy (500 µg/mL) with application of CVCES at -1.75 V (all voltages are with respect to Ag/AgCl unless otherwise stated) for 24 h resulted in 99.98% reduction in th

The Boston Keratoprosthesis (B-KPro) is a widely accepted modality of corneal restoration in eyes where traditional penetrating keratoplasty has little chance of success. It is the most commonly used keratoprosthesis worldwide. While the introduction of broad-spectrum antibiotic prophylaxis has virtually eliminated cases of bacterial endophthalmitis, fungal colonization and infections are a growing concern. This review of the literature summarizes risk factors for fungal infections in KPro eyes, rates of fungal infection and colonization, clinical presentation, causative organisms, management,

BACKGROUND: Surgical site infection following correction of pediatric scoliosis is well described. However, we are aware of no recent multicenter study describing the rates of surgical site infection, and associated pathogens, among patients with different etiologies for scoliosis.
METHODS: A multicenter, retrospective review of surgical site infections among pediatric patients undergoing spinal instrumentation to correct scoliosis was performed at three children's hospitals in the United States. Study subjects included all patients undergoing posterior spinal instrumentation from January 200

Infectious complications of cardiovascular implantable electronic devices (CIED) have risen out of proportion to overall increases in use. Although CIED can provide potentially life-saving benefits, device-associated infections can be a devastating complication. This article examines the epidemiology and microbiology of CIED infection and reviews specific host and procedural risk factors. Recent advances in the diagnosis of CIED infections and the multifaceted approach required for successful management in this complex patient population are also considered.

Patient-related risk factors for invasive Staphylococcus aureus infection overlap with those for periprosthetic joint infections (PJIs). We compared these factors and clinical characteristics between 17 exogenous and 40 haematogenous PJIs caused by S. aureus. Exogenous cases presented significantly more often with damaged periprosthetic soft tissue, whereas haematogenous cases more often had systemic signs of infection, such as fever, chills, and sepsis syndrome. However, comorbid conditions associated with S. aureus infection and/or PJIs did not differ between the two groups. These findings i

Prosthetic valve endocarditis is an uncommon manifestation of infection with Bartonella species. Herein, we report a case of Bartonella henselae endocarditis involving prosthetic mitral and aortic valves. The patient had a favorable outcome with combined medical and surgical therapy. Concomitant crescentic glomerulonephritis led to an initial mistaken diagnosis of Wegener's granulomatosis.
