A systematic review of climate change impacts on sewer overflow.

Climate change has posed a serious threat to wastewater infrastructure, intensifying sewer overflow (SO) failures and associated public health risks. Innovative approaches for capturing and predicting climate change effects on SOs are continuously developed, providing valuable insights into the key processes and drivers of SOs and supporting the design of adaptation strategies. However, a comprehensive summary with rigorous evidence evaluation for the impacts of all potential climate scenarios on SOs is still lacking. This paper reviews 72 recent studies and proposes future perspectives on cli

Exceeding the limits of paediatric heat stress tolerance: the risk of losing a generation to climate inaction.

Greenhouse gas (GHG) emissions are creating unprecedented climate-driven extreme weather, with levels of heat and humidity surpassing human physiological tolerance for heat stress. These conditions create a risk of mass casualties, with some populations particularly vulnerable due to physiological, behavioural and socioeconomic conditions (eg, lack of adequate shelter, limited healthcare infrastructure, sparse air conditioning access and electrical grid vulnerabilities). Children, especially young children, are uniquely vulnerable to extreme heat-related morbidity and mortality due to factors

Maternal and child health services in India with special focus on perinatal services.

India has an excellent infrastructural layout for the delivery of MCH services in the community through a network of subcenters, primary health centers, community health centers, district hospitals, state medical college hospitals, and other hospitals in the public and private sectors. However, the health pyramid does not function effectively because of limited resources, communication delays, a lack of commitment on the part of health professionals, and, above all, a lack of managerial skills, supervision, and political will. The allocation of financial resources for the delivery of health ca

Social Fragility: Impact of Early Fertility and Domestic Violence in Colombia During the Pandemic.

Social fragility, defined as the inability of a society to manage risks and resolve conflicts without violence or external aid, is closely linked to early fertility and domestic violence, particularly among vulnerable populations. These challenges were exacerbated by the COVID-19 pandemic, which exposed regional disparities across Colombia, particularly in areas with weak social infrastructure and high dependence on public policies. This study integrates data from all Colombian departments on annual births, deaths by cause, domestic violence by perpetrator, and other demographic factors to con

Injury burden during an insurgency: the untold trauma of infrastructure breakdown in Baghdad, Iraq.

BACKGROUND: Injuries as a result of the ongoing violence in Iraq have been a topic of both lay and academic interest. However, to date, attempts to quantify injuries in the country have focused primarily on direct mortality, which represents only a small portion of overall injury burden and ignores those injuries indirectly resulting from the conflict-induced breakdown of Iraqi infrastructure. METHODS: We performed a cross-sectional household survey from October to November, 2009, within the Baghdad Governorate of central Iraq. Using two-stage cluster-sampling with a proportional-to-size meth

Next-generation biosensors for detecting bioaerosols: Challenges, innovations, and applications.

Bioaerosols, which include airborne bacteria, viruses, fungi, spores, and biological toxins, are an important but understudied component of atmospheric particulate matter, with serious consequences for public health, environmental safety, and biodefense. Traditional bioaerosol detection methods, such as culture-based assays and laboratory molecular approaches, are time-consuming, labor-intensive, and unsuitable for real-time airborne monitoring. As a result, next-generation biosensors have emerged as revolutionary instruments for detecting bioaerosols quickly, sensitively, and in situ. This st

Retrospective Assessment of Preparedness for Mosquito Control Post-Hurricane Florence in North Carolina.

OBJECTIVES: Ideally, mosquito control programs (MCPs) use surveillance to target control measures to potentially dangerous mosquito populations. In North Carolina (NC), where there is limited financial support for mosquito control, communities may suffer from mosquito-related issues post-hurricane due to lack of existing MCPs. Here, study objectives were to (1) investigate the emergency response of a subset of NC counties post-Hurricane Florence and (2) develop guidelines and policy recommendations to assist MCPs in post-hurricane mosquito control response. METHODS: A survey was administered

Avoiding Crisis Conditions in the Healthcare Infrastructure: 2 Case Studies in Statewide Collaboration.

In fall 2020, COVID-19 infections accelerated across the United States. For many states, a surge in COVID-19 cases meant planning for the allocation of scarce resources. Crisis standards of care planning focuses on maintaining high-quality clinical care amid extreme operating conditions. One of the primary goals of crisis standards of care planning is to use all preventive measures available to avoid reaching crisis conditions and the complex triage decisionmaking involved therein. Strategies to stay out of crisis must respond to the actual experience of people on the frontlines, or the "groun

Accelerating wastewater-based SARS-CoV-2 surveillance using a mobile laboratory in mixed infrastructure regions.

Wastewater-based epidemiology (WBE) is a powerful tool for early infectious disease detection, but its effectiveness depends on rapid sample processing to prevent RNA degradation and maximize detection lead time for public health responses. To address delays and logistical constraints in regions with limited laboratory infrastructure, we deployed a mobile laboratory to enable on-site, near-real-time sample processing and analysis. Evaluation of this approach for on-site wastewater processing and RNA extraction showed a significant reduction in time-to-results compared to centralized laboratory

Zika Virus Vaccine Development: Progress in the Face of New Challenges.

Zika virus (ZIKV) emerged at a global level when it spread to the Americas and began causing congenital malformations and microcephaly in 2015. A rapid response by academia, government, public health infrastructure, and industry has enabled the expedited development and testing of a suite of vaccine platforms aiming to control and eliminate ZIKV-induced disease. Analysis of key immunization and pathogenesis studies in multiple animal models, including during pregnancy, has begun to define immune correlates of protection. Nonetheless, the deployment of ZIKV vaccines, along with the confirmation…

A Perspective on the Global Pandemic of Waterborne Disease.

Waterborne diseases continue to take a heavy toll on the global community, with developing nations, and particularly young children carrying most of the burden of morbidity and mortality. Starting with the historical context, this article explores some of the reasons why this burden continues today, despite our advances in public health over the past century or so. While molecular biology has revolutionized our abilities to define the ecosystems and etiologies of waterborne pathogens, control remains elusive. Lack of basic hygiene and sanitation, and failing infrastructure, remain two of the g

[The return of sleeping sickness in an epidemic form: international action for drugs].

In Central Africa, the number of people suffering and dying from sleeping sickness is the same as it was in 1920, whereas the disease was under control in the 1950s. In Zaire (Congo), Gambiense sleeping sickness was virtually under control by 1960. Due to war, chaos, population movements, lack of resources, and collapse of public health, the situation is now as it was 80 years ago and some years ago it was even more bleak as the supply of drugs seemed to halt. Due especially to the action of Médecins sans Frontières, the availability of the four essential drugs pentamidine, suramine, melarsopr

Culturally-Tailored Health Literacy: A Pilot Study of VHV-Led Cannabis Harm Reduction Under Nursing Supervision in Rural Northern Thailand.

Thailand's cannabis policy transition created a knowledge vacuum regarding the boundaries between authorized medicinal use and non-medical consumption, thereby increasing misuse risks. This study evaluated a culturally-tailored medicinal cannabis health literacy program delivered by Village Health Volunteers (VHVs) under public health nursing supervision as a community-based harm reduction strategy. An exploratory sequential mixed-methods design was employed in Northern Thailand. Phase 1 surveyed baseline health literacy ( n = 462 ). Phase 2 involved focus groups to co-design the intervention

Neglected tropical disease control in post-war Sierra Leone using the Onchocerciasis Control Programme as a platform.

Strategic investments in the control of neglected tropical diseases (NTD) spearheaded by the US Government, the British Government and other bilateral donors such as foundations and key pharmaceutical partners have enabled the treatment of millions of people for the five targeted debilitating diseases (lymphatic filariasis, onchocerciasis, schistosomiasis, soil-transmitted helminthiasis and trachoma), paving the way for the potential elimination as public health problems of some of these diseases. Like many other countries, Sierra Leone has a high burden of these major NTDs. Despite the fragil
