Radiation exposure to surgeon and patient in intramedullary nailing of the lower limb.

Intramedullary nailing is probably the best method of treating long bone fractures in the lower limb. The operation is guided by fluoroscopy, for guide-wire insertion, fracture reduction and distal locking. No study so far has measured the scatter radiation to the patient's gonads during intramedullary, particularly femoral, nailing. The purpose here was to estimate the radiation hazard to the patients' gonads and surgeons' hands during intramedullary nailing for lower limb fractures. From April 1994 to June 1998, 184 consecutive patients had 224 nailings for lower limb fractures. Twenty-eight

Sex-based differences in the severity of radiation-induced arthrofibrosis.

As cancer survivorship increases, so does the number of patients that suffer from the late effects of radiation therapy. This includes arthrofibrosis, the development of stiff joints near the field of radiation. Previous reports have concentrated on skin fibrosis around the joint but largely ignored the deeper tissues of the joint. We hypothesized that fat, muscle, and the joint tissues themselves would play a more significant role in joint contracture after radiation than the skin surrounding the joint. To address this hypothesis, we irradiated the right hind flanks of mice with fractionated

Radiation-induced adaptive response for protection against micronucleus formation and neoplastic transformation in C3H 10T1/2 mouse embryo cells.

We have monitored the end points of cellular survival, micronucleus formation and neoplastic transformation frequency to assess adaptation to ionizing radiation in the C3H 10T1/2 mouse embryo cell system. Plateau-phase cells were pre-exposed to an adapting dose of 0.1 to 1.5 Gy low-dose-rate gamma radiation 3.5 h prior to an acute challenge dose of 4 Gy. No adapting dose improved clonogenic survival detectably, whether the cells were plated immediately after the acute exposure or held in plateau phase for 3.5 h before plating. However, all chronic adapting doses resulted in both a reduction in

Photobiology of melanin pigmentation: dose/response of skin to sunlight and its contents.

A randomized double-blind clinical trial involving twenty-two volunteers was conducted in two locations (Orlando, FL, and St. Paul, MN) to test the efficacy of the newly designed ultraviolet monitor badges (Sun Timers), described in another paper by us in this issue of the Journal, 1 and to establish the relationship between spectral band exposure dose and the biologic responses of erythema and pigmentation. Individuals with skin types II, III, and IV, exhibiting differences in reactivity to solar radiation, were exposed to varying doses of full-spectrum sunlight through templates mounted on t

Radiation dose proportions of localizer radiograph and bolus tracking in low-dose pediatric cardiothoracic computed tomography.

BACKGROUND: Optimization of localizer radiograph and bolus tracking doses is necessary, as their dose proportion may increase with a decreasing diagnostic scan dose in pediatric cardiothoracic computed tomography (CT). OBJECTIVE: To evaluate the radiation dose proportions of the localizer radiograph and bolus tracking in low-dose pediatric cardiothoracic CT. MATERIALS AND METHODS: For low-dose pediatric cardiothoracic CT, a posteroanterior localizer radiograph was acquired with 80 kV, and 35 mA or 20 mA in 852 infants (age<1 year). Propensity score matching was employed in comparing the 35 m

Trustworthy artificial intelligence in radiation oncology: cross-industry lessons for development, validation, and deployment.

Artificial intelligence (AI) is rapidly expanding across the radiation oncology workflow, with applications spanning imaging, contouring, treatment planning, quality assurance, outcome prediction, workflow automation, and clinical decision support. Although technical progress has accelerated substantially, successful clinical translation remains inconsistent. Many of the challenges limiting implementation are not unique to radiation oncology and have previously emerged across healthcare and other high-stakes industries. In this narrative review, we examine radiation oncology AI through the bro

Use of Radiation Therapy in the Management of Vulvar Cancers-Identification and Management of Acute and Late Toxicities.

Radiation therapy plays a critical role in the management of locally advanced vulvar cancers but can lead to a unique spectrum of side effects, with >25% of patients experiencing high-grade toxicities. The treatment phase requires meticulous perineal skincare and may require pharmacologic management of dysuria and cystitis, diarrhea, nausea, and dermatitis/mucositis. The addition of chemotherapy warrants close laboratory monitoring for hematologic and metabolic derangements.

Radiation exposure in patients with inflammatory bowel disease.

BACKGROUND AND AIMS: Radiological imaging is crucial for diagnosing and monitoring patients with inflammatory bowel diseases (IBD). With increasing awareness of radiation risks, imaging doses should be "as low as reasonably achievable," with a risk level that one should try to stay underneath 75 mSv. However, real-world data on cumulative exposure are limited. We retrospectively analyzed the radiation exposure of patients with IBD in a Belgian tertiary referral center. METHODS: Radiation exposure examinations were reviewed using the electronic health records. Annual (AED) and cumulative effec

Medical management: major lessons learned from the Chernobyl accident (the review).

Thirty-five years have passed since the moment of the disaster at the Chernobyl nuclear power plant. It is quite a sufficient period to assess the correctness of the organisation of medical care for victims, to summarise the results of monitoring the health status of various groups of persons involved in the accident, including its direct participants. Radiation from a massive source of relatively uniform gamma radiation and a heterogeneous source of beta radiation can cause affected people to develop acute radiation syndrome (ARS) of varying severity, including non-curable forms of the diseas

Mortality among U.S. Industrial Radiographers Exposed to Ionizing Radiation, 1969-2019.

A cohort mortality study was conducted of 123,401 industrial radiographers in the United States to estimate risks following protracted radiation exposures. The cohort was constructed from the Nuclear Regulatory Commission Radiation Exposure Information Reporting System and the Landauer, Inc. dosimetry databases. Workers were monitored between 1939 and 2011 and were exposed mainly to external gamma radiation from 192Ir and 60Co. Causes of death were obtained from the National Death Index and state mortality files with follow-up through 2019. The mean duration of follow-up was 27.7 years. Nearly

Evaluation of congenital extracranial masses by waterpath sonography.

Congenital scalp masses pose a diagnostic and management challenge. In the past the relationship of the lesion to the skull and intracranial contents have been assessed by skull x-rays and computed cranial tomography. Waterpath sonography is a non-invasive technique that utilizes no ionizing radiation for evaluating and monitoring congenital scalp masses. In two illustrative cases this method of imaging assisted in assessing the size, contents, and relationship of the congenital scalp masses to the intracranial contents. It enabled the surgeon to anticipate the avascular cystic nature of the c

Evaluation of deep space exploration risks and mitigations against radiation and microgravity.

Ionizing radiation and microgravity are two considerable health risks encountered during deep space exploration. Both have deleterious effects on the human body. On one hand, weightlessness is known to induce a weakening of the immune system, delayed wound healing and musculoskeletal, cardiovascular, and sensorimotor deconditioning. On the other hand, radiation exposure can lead to long-term health effects such as cancer and cataracts as well as have an adverse effect on the central nervous and cardiovascular systems. Ionizing radiation originates from three main sources in space: galactic cos

Bedside detection and monitoring of pulmonary embolism using electrical impedance tomography.

Pulmonary embolism (PE) is a common and potentially fatal obstructive disease of the pulmonary arteries; early diagnosis and continuous monitoring are particularly critical in critically ill patients. Electrical impedance tomography (EIT), a noninvasive and radiation-free imaging modality that enables real-time bedside monitoring, offers a promising approach for adjunctive diagnosis and perfusion assessment of PE, especially in patients who cannot undergo computed tomography pulmonary angiography (CTPA) due to instability or other contraindications. Building upon an overview of EIT imaging pri

Progressive Postnatal Pansynostosis in Crouzon Syndrome.

Crouzon syndrome is a known craniosynostosis syndrome with a heterogenous presentation. Suture closure can rapidly develop postnatally and involve multiple sutures, requiring vigilant clinical monitoring to prevent delay in diagnosis and developing intracranial hypertension. The authors present the case of a male patient with Crouzon syndrome who developed postnatal pansynostosis at 18 months of age when several missed appointments led to diagnosis after complete closure of all sutures and signs of elevated intracranial pressure. Posterior cranial vault distraction was performed soon after, an

Mechanism of Asbt (Slc10a2)-related bile acid malabsorption in diarrhea after pelvic radiation.

Background: Radiation is a mode of treatment for many pelvic malignancies, most of which originate in the gynecologic, gastrointestinal, and genitourinary systems. However, the healthy gut is unavoidably included in the irradiation volume, resulting in undesirable results that manifest as radiation-induced diarrhea (RID), which is the most common side effect of radiation therapy and significantly affects the patients' quality of life. This study aimed to investigate the potential mechanism of diarrhea after pelvic radiotherapy in rats based on the effect of radiation on bile acid homeostasis a

A 2D antiscatter grid and scatter sampling based CBCT method for online dose calculations during CBCT guided radiation therapy of pelvis.

Online dose calculations before radiation treatment have applications in dose delivery verification, plan adaptation, and treatment planning. We propose a novel CBCT imaging pipeline to enhance accuracy. Our approach aims to improve HU accuracy in CBCT images for more precise dose calculations. A quantitative CBCT pipeline was implemented, combining data correction strategies and scatter rejection, achieving high CT number accuracy. We evaluated the pipeline's effect using pelvis anatomy phantoms and found that dosimetric errors in quantitative CBCT-based dose calculations were minimal. In con

Tissue thickness estimation for high precision head-tracking using a galvanometric laser scanner - a case study.

Marker-less optical head-tracking constitutes a comfortable alternative with no exposure to radiation for realtime monitoring in radiation therapy. Supporting information such as tissue thickness has the potential to improve spatial tracking accuracy. Here we study how accurate tissue thickness can be estimated from the near-infrared (NIR) backscatter obtained from laser scans. In a case study, optical data was recorded with a galvanometric laser scanner from three subjects. A tissue ground truth from MRI was robustly matched via customized bite blocks. We show that Gaussian Processes accurate
