BACKGROUND: We report a unique case of synchronous sacrococcygeal chordoma in association with rectal invasive adenocarcinoma. Metastatic penile carcinoma is rare and usually originates from genitourinary tumors. The presenting symptoms or signs have been described as nonspecific except for priapism. Rectal adenocarcinoma is a very unusual source of metastatic penile carcinoma. We report a case of metastatic penile carcinoma that originated from the rectum. Symptomatic improvement occurred with palliative radiotherapy.

The case history of a patient whose barium enema radiographs confirmed the clinical diagnosis of a stenosing rectal carcinoma is presented. Two years previously, the patient had undergone sub-total gastrectomy for carcinoma of the gastric antrum. An abdominoperineal excision of the rectum was performed, and histological examination of the tumour showed it to be not primary large bowel in origin, but a metastatic manifestation of the original gastric cancer.

Endorectal sonography is a technique that has been developed recently to visualize the rectal wall and perirectal tissues with a high degree of clarity. Studies utilizing endorectal sonography in the preoperative staging of rectal carcinoma have reported an accuracy of between 67 and 92% in the visualization of the depth of tumor invasion in the rectal wall.
