A 60-year-old man with metastatic supraglottic laryngeal cancer treated with nivolumab, an immune checkpoint inhibitor (ICI), developed abdominal aortitis and retroperitoneal fibrosis. The nivolumab was discontinued and no steroids were initiated. A literature search found 19 other cases of ICI-associated aortitis. BACKGROUND: IgG4-related disease (IgG4-RD) is a chronic fibrotic disease mediated by immunity recognized by clinicians in recent years. When the kidney is involved, it is called IgG4-related kidney disease (IgG4-RKD). IgG4-related tubulointerstitial nephritis (IgG4-TIN) is a representative manifestation of IgG4-RKD. IgG4-TIN can cause obstructive nephropathy complicated by retroperitoneal fibrosis (RPF). Cases of IgG4-TIN complicated with RPF are rare. Retroperitoneal fibrosis (RPF) causing large vessel stenosis and thrombosis is rare but well-described. We describe a 50-year-old man with rapid progression of central venous thrombosis in the presence of RPF and exogenous testosterone use. Therapeutic anticoagulation was initiated and catheter directed thrombolysis was performed after placement of an inferior vena cava (IVC) filter. We demonstrate a case, in which endoscopic ultrasonography-guided fine-needle biopsy (EUS-FNB) was useful for determining the diagnosis of lesions of retroperitoneal fibrosis. In our case, accessing the retroperitoneal lesions by conventional percutaneous biopsy procedures was not feasible due to the difficulty of avoiding the inferior vena cava and ureter. We believe that our case demonstrates a unique approach for performing histological analysis in a challenging case.

A 68-year-old man with a history of retroperitoneal fibrosis was referred to our clinic because of disabling bilateral lower extremity swelling and venous claudication, worse on the right side than on the left. He was noted to have a duplicated inferior vena cava and an iliac vein obliteration from retroperitoneal fibrosis. The patient underwent bilateral iliac vein stenting and had complete relief of symptoms through follow-up of 46 months with no recurrence.
